The medicines for Sha syndrome should be taken cold. The reason for coma and unresponsiveness is that the heat-toxin of Sha attacks the Heart, causing the Heart to lose its ability to govern consciousness, resulting in coma. When cold medicine enters the mouth and flows smoothly down through the diaphragm, the heat-toxin residing in the chest and epigastrium dissipates along with the action of the medicine, so the patient regains consciousness quickly. If coma persists, it must be due to food retention or blood-phlegm obstruction; in such cases, medication should be administered based on pulse and symptoms, employing methods to open blockages or purge downward, without fail leading to recovery. Here, a specific aspect of medication usage is presented, awaiting those with divine insight to understand it further. Use herbs like Jingjie (Schizonepetae Herba) and Fangfeng (Saposhnikoviae Radix) to disperse from the exterior; use Qingpi (Aurantii Fructus Immaturus Pericarpium) and Chenpi (Aurantii Fructus Ripens Pericarpium) to resolve stagnation in the middle jiao; use Zhishi (Aurantii Fructus Immaturus) and Dahuang (Rhei Radix et Rhizoma) to purge via the large intestine; use Mutong (Akebiae Caulis) and Zexie (Alismatis Rhizoma) to promote urination; use Zhangsha (Crataegi Fructus), Maiya (Hordei Fructus Germinatus), and Luobozhongzi (Raphani Semen) to treat food obstruction; use Yinhua (Lonicerae Japonicae Flos) and Honghua (Carthami Flos) to treat blood stasis; (Yinhua treats unresolved heat in the blood level.) Use Binglang (Arecae Semen) and Pengzhu (Curcumae Rhizoma) to treat accumulation and stagnation.
Avoid hot soups, hot wine, porridge soup, rice foods, and other similar items. Violating these prohibitions will cause mild cases to become severe, and severe cases to die immediately.
After Sha syndrome subsides slightly, if hunger is felt, sudden intake of food will cause recurrence. Endure for one or two days before eating to ensure complete safety.
The book *Shazhang Yuheng* discusses the treatment of Sha syndrome with great precision and detail, yet there are parts that are overly rigid. For instance, miscellaneous diseases such as Feng (Wind), Lao (Consumption), Gu (Distension), and Ge (Regurgitation/Obstruction) are all treated as Sha syndrome. Is it logical that everything observed is merely Sha? Here, only the major and critical principles are selected, arranged in order, and annotated, thereby encompassing the general methods for treating Sha syndrome. Recorded by Jin Zai.
The symptoms include abundant phlegm-saliva, rapid breathing and wheezing, a sawing sound in the throat, and pain resembling Hou'e (Goose Throat, a type of throat disease). However, unlike Hou'e which presents with swelling inside the throat, this condition lacks such swelling. It also resembles Ji Houfeng (Acute Wind-Throat), but while Houfeng pain is fixed in location, the pain here is not fixed. Furthermore, Hou'e does not present with Sha veins (explained previously), whereas this condition does. Apply the pricking method described earlier, and choose one of the blowing formulas listed subsequently for application.
Bingpeng San (Ice and Borax Powder) Treats throat swelling and pain caused by Sha syndrome.
Tianzhuhuang (Calamistris Reliquia) [2 qian], Pingsha (Borax) [2 qian], Zhusha (Cinnabaris) [2 fen], Bingpian (Borneolum Syntheticum) [2 fen, Yuanmingfen (Natrii Sulfas) [8 li].
Grind together into a fine powder, store in a porcelain bottle, and seal with wax. (For use when breathing is difficult.) Blow the powder into the patient's throat.
Jiuku Dan (Pill for Saving from Suffering) A formula for treating Sha Qi depression and stifling.
Zhishi (Aurantii Fructus Immaturus) [1 liang], Luobozhongzi (Raphani Semen) [1 liang], Wuyao (Linderae Radix) [8 qian], Lianqiao (Forsythiae Fructus) [8 qian], Yujin (Curcumae Longae Rhizoma) [2 qian].
Grind into powder, take with clear tea that has cooled slightly.
Jingjie Yinhua Tang (Schizonepeta and Honeysuckle Decoction) This is a formula for treating blood stasis.
Jingjie (Schizonepetae Herba), Yinhua (Lonicerae Japonicae Flos), Honghua (Carthami Flos), Qiancao (Rubiae Cordifoliae Radix et Rhizoma), Danpi (Moutan Cortex), Chishao (Paeoniae Radix Rubra) [each 1 qian], Baijili (Tribuli Fructus) (remove spines, grind to powder) [8 fen], Wuyao (Linderae Radix) [5 fen], Xiangfu (Cyperi Rhizoma) (crushed) [3 fen].
Add two cups of water, decoct until seven-tenths remain, and take warm.
Appendix: Formulas for Treating Various Sha Pains
Mix half well-water and half river-water, and drink. Drink clarified mud-water. Mix white sugar with plum juice. Take powder of Can Sha (Bombyx Batryticatus) mixed with boiled water after cooling. The above are used for Sha syndrome without food retention or obstruction.
Emetic Method
For treating new food blocking Sha toxin: Mingfan (Alumen) [4 fen], dissolve in one bowl of plain water, cool, and drink. Another prescription: Salt [a pinch], mix with one bowl of plain water, cool, and drink. Both prescriptions require drinking a large amount to induce vomiting; taking less will be ineffective. (Note: Using slightly more Alumen is also acceptable.)
