When mania (Kuang) first begins, the patient initially exhibits sadness, forgetfulness, distress and anger, and frequent fear. This arises from prolonged poverty, worry, and hunger. Treatment involves selecting points on the Hand Taiyin Lung Meridian and Hand Yangming Large Intestine Meridian until the blood changes color (from dark to bright red), followed by selecting points on the Foot Taiyin Spleen Meridian and Foot Yangming Stomach Meridian.
When mania first begins, presenting with initial sadness, forgetfulness, distress and anger, and susceptibility to fear, it results from long-term poverty, worry, and hunger leading to depression that suddenly erupts after a long period. Therefore, points on the Lung and Large Intestine meridians are selected to unblock stagnant Qi, and points on the Spleen and Stomach meridians are adjusted to harmonize Yin and Yang.
When mania erupts, the patient sleeps little, feels no hunger, considers themselves virtuous, wise, and noble, abuses others, and does so day and night without rest. Treatment involves selecting points on the Hand Yangming Large Intestine Meridian, Hand Taiyang Small Intestine Meridian, Hand Taiyin Lung Meridian, and the Shaoyin Kidney Meridian under the tongue. Observe the vessels; if they are full and vigorous, puncture them; if not full, leave them alone.
Symptoms such as inability to sleep, lack of appetite, and abusive speech are all manifestations of excessive Yang pathogenic factors. Puncture the vessels that appear full to drain the pathogen; if they do not appear full, do not puncture and abandon the treatment. The selection of the Large Intestine, Small Intestine, Lung, Kidney, and Heart meridians is because Yang pathogens are hyperactive, and treatment aims to drain the pathogen through the upper and lower Yin-Yang meridians.
Patients with mania who speak nonsense, are easily startled, laugh excessively, enjoy singing and music, and walk aimlessly without stopping, have contracted this due to great fright. Treatment involves selecting points on the Hand Yangming Large Intestine Meridian, Hand Taiyang Small Intestine Meridian, and Hand Taiyin Lung Meridian.
This condition arises because fright injures and disturbs the Heart and Liver organs, causing wind and fire to rage wildly, thus manifesting these symptoms. Points on the Lung and Large Intestine meridians are selected to promote Metal Qi to restrain Wood pathogen (Liver), and the Small Intestine Fu organ is targeted to drain Fire from the Heart.
Manic patients who see things that are not there, hear sounds that are not there, and frequently shout, suffer from a deficiency of Qi. Treatment involves selecting points on the Hand Taiyang Small Intestine Meridian, Hand Taiyin Lung Meridian, Hand Yangming Large Intestine Meridian, Foot Taiyin Spleen Meridian, and bilateral points on the head.
Due to the decline of Yuan Qi (Original Qi) and the scattering of the Shen (Spirit) caused by mental overexertion, hallucinations occur. The Qi fails to circulate smoothly, leading to frequent shouting. Treatment involves unblocking the meridians to regulate Qi and calm the Spirit.
Manic patients who eat large amounts, often see spirits or ghosts, laugh frequently, but keep their emotions inwardly suppressed, have contracted this due to excessive joy. Treatment involves selecting points on the Foot Taiyin Spleen Meridian, Foot Taiyang Bladder Meridian, and Foot Yangming Stomach Meridian, followed by points on the Hand Taiyin Lung Meridian, Hand Taiyang Small Intestine Meridian, and Hand Yangming Large Intestine Meridian.
Joy causes Qi to relax; excessive joy leads to the dissipation of Heart Shen, resulting in mania, where the dispersion of Qi is even more severe than usual. Points on the exterior and interior of the Six Meridians are selected to regulate Yin and Yang Qi and restore balance.
If mania has recently onset but presents with severity disproportionate to its newness, first select the arterial pulses at Ququan (LR8) on both sides and other vessels showing vigorous pulsation. Puncture to bleed; the condition should subside shortly. If it does not cease, apply further treatment according to method, and moxa the patella area for 20 moxibustion units (zhuang).
When mania newly appears but becomes severe despite being recent, it is due to internal flourishing of Liver Wind. One should first select the arterial pulse at Ququan (LR8) of the Foot Jueyin Liver Meridian on both sides, and other vessels with vigorous pulsation, puncturing to bleed. The condition should subside shortly. If it does not stop, puncture again according to method, and moxa the patella bone to unblock the Yang of the Du Mai (Governing Vessel).
