Chen Qiuzha suddenly passed several sheng of black blood from the bowels, followed by profuse vomiting of bright red blood. He then exhibited sweating, coma, cold limbs with convulsions, restlessness, confusion, and delirious speech. Mengying examined his pulse: the left hand pulse was faint as if absent, while the right hand pulse was wiry (Xian) and rapid (Shu) upon pressure. The patient was elderly with a long-standing deficiency of Yin (Yin Fen). The massive loss of Liver Blood (Gan Xue) caused a sudden hyperactivity of Wind-Yang (Feng Yang). This was likely caused by anger combined with the intake of hot-natured medicines. His wife stated that he had been somewhat depressed recently, had taken Ginger-Jujube Decoction (Jiang Zao Tang) for a long time during winter, and drank a bottle of Zhongyao liquor. Mengying said, "Indeed." He prescribed a formula containing American Ginseng (Xi Yang Shen), Rhinoceros Horn (Xi Jiao), Rehmannia Root (Sheng Di), Honeysuckle Flower (Yin Hua), Mung Bean (Lu Dou), Gardenia Fruit (Zhi Zi), Scrophularia Root (Yuan Shen), Poria (Fu Ling), Antelope Horn (Ling Yang), and Imperata Rhizome (Mao Gen), mixed with warm child's urine (Tong Sou) for administration. (The pulse focuses on the deep level; rapidity upon pressure indicates Yin deficiency with internal heat. This formula is based on the mutual reference of pulse and symptoms. Dosage: American Ginseng 3 qian, sliced Rhinoceros Horn and secondary tip of Antelope Horn each 4 qian, simmered in charcoal fire for 8 minutes; Scrophularia slices 1 liang, large Rehmannia 8 qian, soaked and decocted, dregs removed; Silver Flower 1.5 liang, raw Mung Bean 1 liang, black Gardenia peel 3 qian, Cloud Poria 3 qian, fresh Imperata Rhizome 5 qian, one large wine glass of child's urine, decocted removing head and tail, then mixed.) Externally, heated iron was quenched in vinegar for inhalation; Dragon Bone (Long Gu) and Oyster Shell (Mu Li) were ground into powder and applied to stop sweating; Raw Aconite (Sheng Fu Zi) was mashed and applied to Yongquan (KI-1) acupoint to guide floating Yang back to its source. After two doses, bleeding stopped, and the left pulse gradually recovered. Turtle Shell (Gui Ban) and Softshell Turtle Shell (Bie Jia) were added (Blood Turtle Shell pounded 4 liang, Blood Softshell Turtle Shell pounded 2 liang, these two ingredients simmered in charcoal fire for 6 minutes first). After three more doses, mental clarity returned, and various ailments subsided. He could slightly consume congee. Then Rhinoceros Horn and Antelope Horn were removed, and Ophiopogon Tuber (Mai Dong), Asparagus Tuber (Tian Dong), Privet Fruit (Nv Zhen Zi), and Eclipta Herba (Han Lian Cao) were added. (Flower Ophiopogon 4 qian, sliced Tian Dong 6 qian, pounded Nv Zhen Zi 5 qian, Han Lian Cao 4 qian.) Sleep and appetite improved daily. Half a month later, black dry feces were passed. After twenty days, the color of urine and stool normalized. Tonifying medicines were given to regulate and cure him.
Yao Lingyu's wife suffered from chronic wheezing and coughing, and contracted Spring-Warmth disease (Chun Wen). The physician knew her original essence was long depleted and administered warming tonics. This led to spasms, syncope, coma, deafness, and delirium. Her face was blue, tongue crimson, with phlegm-cough and insomnia. Mengying diagnosed her pulse as still wiry and slippery. He said, "Although the condition is dangerous, the vital energy has not perished." He prescribed Rhinoceros Horn, Antelope Horn, Scrophularia Root, Glehnia Root (Sha Shen), Anemarrhena Rhizome (Zhi Mu), Trichosanthes Root (Hua Fen), and Gypsum (Shi Gao) to clear heat, extinguish wind, save Yin, and generate fluids. (Sliced Rhinoceros Horn and secondary tip of Antelope Horn decocted first, 4 qian each; Scrophularia slices soaked and decocted, dregs removed, 1 liang; Bei Sha Shen 8 qian stir-fried with wine, Anemarrhena 4 qian, Nan Hua Fen 4 qian, raw Gypsum decocted first, 1.6 liang.) Assisted by Cistanche (Rong Rong), Purple Quartz (Shi Ying), Softshell Turtle Shell, Melia Seed (Jin Ling Zi), Inula Flower (Xuan Fu), Fritillaria Bulb (Bei Mu), and Bamboo Sap (Zhu Li) to subdue Yang, suppress rebellion, unblock collaterals, and eliminate phlegm. (Pale Cistanche 1.5 qian, used as a counter-agent for subduing Yang; crushed Purple Quartz 3 qian, crushed Blood Softshell Turtle Shell 2 liang, crushed Melia Seed 3 qian, wrapped Inula flower decocted first 3 qian, these four ingredients simmered in charcoal fire for 8 minutes first; crushed Chuan Bei Mu 1 liang, ginger-infused Bamboo Sap 2 large wine glasses mixed in. Using salty and bitter subduing agents to guide medicinal power downward, combining with Bei Mu and Zhu Li to achieve descending with ascending effects.) After three doses, the condition stabilized. Subsequently, Rhinoceros Horn, Antelope Horn, and Gypsum were removed, and Rehmannia Root (Sheng Di) was added. (Large Sheng Di soaked and decocted, dregs removed, 8 qian.) She recovered after ten days.
Ma Xianggu suffered from urinary retention, nearly dying. Mengying examined him: the pulse was firm, the body robust, with thirst and yellow coating. (A firm pulse indicates Yang excess; Yang excess can tolerate bitter-cold medicines.) He administered Anemarrhena, Phellodendron Bark (Huang Bai), Gardenia, Melia Seed, Rhinoceros Horn, Dodder Seed (Tu Si Zi), Trichosanthes Kernel (Lou Ren), and Ginger-processed Bamboo Sheath (Zhu Ru), sending it down with Danggui Longhui Wan. He recovered without recurrence. (Wine-stir-fried Anemarrhena 3 qian, wine-stir-fried Huang Bai 1 qian, black Gardenia peel 3 qian, crushed Melia seed decocted first 3 qian, sliced Rhinoceros Horn simmered in charcoal fire for 8 minutes 4 qian, crushed Lou Ren 4 qian, ginger-processed Zhu Ru 4 qian, medicine sent down with 3 qian of Longhui Wan. The Liver is the organ of Wind; extreme Liver heat causes Wind to rise. When Wind rises, the Qi of the whole body fails to descend. Urinary retention nearing death is due to rising internal Wind. Rhinoceros Horn calms internal Wind.)
Xie suffered from coughing and difficulty lying on the left side. Mengying palpated his pulse: the right Cun position was choppy and slippery. He said, "This is Lung deficiency with phlegm stored in the collaterals." He administered Eelgrass Stem (Wei Jing), Loofah Network (Si Gua Luo), Raw Concha Meretricis (Sheng Ge Fen), Fritillaria, Winter Melon Seed (Dong Gua Zi), Poria, Polygonatum Rhizome (Yu Zhu), Loquat Leaf (Pi Ye), Bird's Nest (Yan Wo), and Pear Flesh. He recovered. (Pulse choppy indicates deficiency; pulse slippery indicates phlegm excess. Ginger-processed Reed Rhizome 8 qian, Si Gua Luo 3 qian, raw Ge Fen wrapped and decocted first 4 qian, Chuan Bei Mu pounded 8 qian, raw Dong Gua Zi 4 qian, Cloud Poria 3 qian, fat Yu Zhu 3 qian, ginger-processed Pi Ye brushed and wrapped 3 qian, Bird's Nest wrapped and decocted first 3 qian, pear flesh with skin 4 qian.)
Grandmother of Xu Shuchao suffered from Malaria. Mengying treated her: the pulse was wiry, slippery, and rapid. There was epigastric stuffiness and constipation, sweating when closing eyes, thirst, and lack of appetite. Some worried about Qi collapse in an elderly person. Mengying said, "This is Warm pathogen transforming with abundant phlegm. Tonics must absolutely not be used." He prescribed Anemarrhena, Scutellaria Root (Huang Qin), Trichosanthes, Apricot Kernel (Xing Ren), Forsythia Fruit (Qiao), Fritillaria, Inula, Bamboo Sheath, Coptis Rhizome (Lian), Dendrobium Stem (Shi Hu), and Xue Geng (Snow Soup) as a formula. It took effect gradually. (Wiry pulse indicates Liver heat; slippery indicates phlegm; rapid indicates Yin deficiency with heat. Ginger-processed Anemarrhena 3 qian, ginger-processed Huang Qin 1 qian, ginger-processed Chuan Lian 0.8 fen, crushed Lou Ren 3 qian, bitter Xing Ni entered next 2 qian, Qiao Ke 3 qian, pounded Chuan Bei Mu 4 qian, wrapped Inula decocted first 3 qian, ginger-processed Zhu Ru 3 qian, Shi Hu decocted first 1 liang, pale Sea Cucumber (Hai Zhe) decocted first 2 liang, whole Water Chestnut 1 liang. Sweating when closing eyes indicates Yin deficiency with floating Yang. If Yin does not return, Yang does not settle. If Yang does not settle, Qi does not descend. If Qi does not descend, phlegm does not move. Hence, Shi Hu and Xue Geng are used.)
Pu Aitian, over sixty years old, suddenly suffered from epistaxis. Various methods failed to stop it. Mengying diagnosed: dark complexion with red luster, pulse wiry, flooding, and hollow. Inquiry revealed extensive use of Yang-assisting medicines in winter. This constitution features excessive Heat and Yin deficiency. He prescribed a large dose of Rhinoceros Horn, Scrophularia, Imperata Rhizome, Privet Fruit, Eclipta, Dendrobium, Poria, Alisma Rhizome (Ze Xie), Asparagus Tuber, and Anemarrhena. (Pulse condition shows Heat excess and Yin deficiency. Sliced Rhinoceros Horn decocted first 8 minutes 4 qian, Scrophularia slices 1 liang, sliced Tian Dong 8 qian, fresh Mao Gen soaked in boiling water, dregs removed 5 qian, pounded Nv Zhen Zi decocted first 5 qian, Han Lian Cao 4 qian, Shi Hu decocted first 1 liang, raw Ze Xie 1 qian, wine-stir-fried Anemarrhena 4 qian. One dose brought peace. Subsequent nourishing medicines filled the deficiency and restored health. Unstopping epistaxis leads to rising Wind and loss of Liver Blood, so saving fluids and extinguishing Wind is essential.)
Wife of Xu Shaoqing contracted external pathogen in early summer. Physician He used clearing and resolving methods ten times consecutively without relief. Mengying was invited. The pulse was wiry, flooding, empty, and large, especially on the left. Great thirst, profuse sweating, ability to eat but speaking nonsense, red face, cold feet, and all-night insomnia. Mengying said, "Although the syndrome belongs to Warmth, true Yin has been deficient since long ago. Long-term mental strain has caused Heart Yang to float outward and Internal Wind to rage. Fortunately, warming dispersants were not used, so there is no cause for fear." He prescribed a large dose of Dragon Bone, Oyster Shell, Rhinoceros Horn, Pearl, Turtle Shell, Softshell Turtle Shell, Fritillaria, Bamboo Sap, Bamboo Leaf, Cinnabar (Chen Sha), Wheat (Xiao Mai), Scrophularia, Salvia Root (Dan Shen), Rehmannia, and Ophiopogon. (Ten clearing treatments targeted the upper while the lower was deficient, hence no improvement. Wiry, flooding, empty, large pulse especially on the left indicates Yin deficiency in the Blood aspect. Great thirst indicates Heat scorching Lung Yin. Profuse sweating indicates Yin deficiency with Yang floating. Ability to eat indicates Wind consuming. Delirium indicates Liver Heat invading the Ying level. Cold feet indicate Heat pathogen injuring the Lung, preventing Qi from descending. All-night insomnia is definitive evidence of rising Wind and floating Yang. Decoct pounded Long Gu 1 liang, pounded Mu Li 6 liang, sliced Rhinoceros Horn 4 qian, pounded Blood Gui Ban 4 liang, pounded Blood Bie Jia 2 liang, whole Chen Sha pounded 1.2 liang. These six ingredients simmered in charcoal fire for 8 minutes first, using the decoction to replace water for brewing medicine. Pounded Chuan Bei Mu 4 qian, ginger-processed Zhu Li one large wine glass mixed in, Bamboo Leaf entered next 2 qian, Bei Xiao Mai 4 qian, Zi Dan Shen 3 qian, Yuan Shen slices 1 liang, large Sheng Di 8 qian, flower Mai Dong 5 qian, soaked in boiling water, dregs removed.) Externally, heated iron quenched in vinegar for inhalation, Oyster Shell powder applied to stop sweat, Raw Aconite mashed and applied to Yongquan acupoint (to guide floating Yang). Gradually improving, but Yin is hard to restore. Frequent administration of sweet, soft, nourishing, and sedating medicines allowed him to get out of bed after more than a month. Menstruation occurred in mid-summer, but emotional distress easily caused fright. He prescribed Mai Dong, Ren Shen, Shu Di, Shi Ying, Fu Shen, Long Yan, Gan Cao, Xiao Mai, Da Zao, and San Jia to conclude treatment. (Pounded Mu Li 6 liang, pounded Blood Bie Jia 4 liang, pounded Blood Gui Ban 2 liang, pounded Zi Shi Ying 3 qian. These four ingredients simmered in charcoal fire for 6 minutes first, using decoction to replace water. Pediatric treatment made the condition worse: stiff limbs, red face, vomiting, yellow coating, thirst with clear urine, occasional syncope. He administered Xiao Xian Xiong Tang plus Gypsum, Anemarrhena, Trichosanthes Root, Bamboo Leaf, Loquat Leaf, Fritillaria, and Xue Geng. (Considering clear urine and occasional syncope, Phlegm-Heat is entirely in the Qi level of the Lung meridian. Flooding and rapid pulse indicate Yin deficiency with heat, hence Xue Geng is used. Pinellia combined with Gypsum releases the exterior with pungent-cool; combined with Anemarrhena and Coptis, it resolves phlegm with pungent-bitter. Ginger-processed Chuan Lian 4 fen, processed Ban Xia pounded and entered next 1.5 qian, crushed Lou Ren 1.5 qian, raw Gypsum decocted first 5 qian, ginger-processed Anemarrhena 1.5 qian, Nan Hua Fen 1.5 qian, fresh Zhu Ye 8 fen, pounded Chuan Bei Mu 3 qian, ginger-processed Pi Ye brushed and wrapped one piece, whole Water Chestnut 4 qian, pale Hai Zhe decocted first 8 qian.) Fe Boyuan suffered from irritability and insomnia. The physician saw a white coating and administered warming medicines, leading to madness and convulsions. Many treatments failed. Mengying observed: left pulse wiry, fine, and rapid; right pulse choppy and slippery. This is a Yin-deficient constitution with blazing Heart Fire, stirring Liver Wind, and abundant Phlegm in the middle. First, Rhinoceros Horn, Antelope Horn, Mulberry Leaf (Sang Ye), and Chrysanthemum Flower (Ju Hua) were used to extinguish Wind. Scrophularia, Moutan Cortex (Dan Pi), Lotus Plumule (Lian Xin), and Child's Urine were used to clear Fire. Bamboo Sheath, Fritillaria, and Xue Geng were used to transform Phlegm. (Left fine-rapid indicates Yin deficiency in the Blood aspect. Left wiry-fine combined with madness and convulsions indicates Yin deficiency with blazing Heart Fire and stirring Liver Wind. Internal Wind did not surge suddenly, but diarrhea occurred. Pulse was fine and wiry, with thirst, drinking, abundant phlegm, lack of appetite, and insomnia. Mengying consulted and administered White Head Worm Decoction (Bai Tou Weng Tang) plus San Jia, Shi Hu, Poria, and Bamboo Sheath, bringing peace. Subsequently, strong tonics concluded treatment and cured her. (Differentiation lies in lack of appetite and insomnia. Raw Gypsum decocted first 1.6 liang, wine-stir-fried Anemarrhena 4 qian, Nan Hua Fen 1 liang, Spring Chai Hu entered next 3 qian.)
Daughter-in-law of Wu Yunxiang often suffered from abdominal distension and reduced meals, gum bleeding, and leg pain. Long-term treatment was ineffective, and muscles gradually wasted. Mengying diagnosed the pulse: wiry, fine, and rapid. Although Liver Qi was stagnant, Yin deficiency and Ying heat existed. How could pungent-unblocking and warming-moving medicines be used? He administered Sour Plum (W Mei), Coptis Rhizome, Melia Seed, White Peony Root (Bai Shao), Gardenia, Quince (Mu Gua), Polygonum Multiflorum (Shou Wu), and Softshell Turtle Shell, along with Bamboo Sheath and Fritillaria. He recovered. (Wiry-rapid pulse indicates Yin deficiency with heat; fine indicates Yin deficiency with concurrent Yang deficiency. Abdominal distension and reduced meals indicate Liver Yang insulting the Stomach. Gum bleeding and leg pain indicate Liver-Stomach Heat knotting. Muscle wasting indicates Liver Wind consuming. Wife of Gu Yunluo suffered from Hong Kong Foot (gout/erysipelas-like condition) for a long time, recurring despite multiple treatments. Eventually, whole-body tendon spasms extended to the vertex, gum pain, numb teeth, sore waist, dizzy eyes, dry mouth, poor appetite, and insomnia. Mengying diagnosed the pulse: hollow, wiry, and rapid. True Yin was greatly depleted. Although the legs hurt, there was never redness or swelling. Hong Kong Foot medicines are useless. He prescribed Two Di (Shu Di and Sheng Di), Two Dong (Tian Dong and Mai Dong), Two Zhi (Nv Zhen Zi and Han Lian Cao), Anemarrhena, Phellodendron, Mulberry Leaf, Chrysanthemum, Gardenia, Melia Seed, Artemisia (Qing Hao), Anemarrhena (Bai Wei), Turtle Shell, Softshell Turtle Shell, and Lotus Root. The condition worsened daily with rebellious Qi and lack of appetite. Mengying diagnosed the pulse: left flooding and rapid, right wiry and slippery. Although Yin deficiency exists, clearing Phlegm-Heat from Lung and Stomach should come first. He administered Bai Hu Tang plus Sha Shen, Hua Fen, Winter Melon Peel, Loquat Leaf, Gardenia, Bamboo Sheath, and Reed Rhizome. Edema subsided immediately.
