Coptis rhizome (Coptidis Rhizoma) stir-fried with wine, 1.5 qian. Lotus root with skin (Nelumbinis Rhizoma) sliced and decocted first, 2 liang. This illness involves a total of four main prescriptions. The first prescription mainly clears phlegm-heat. The second prescription mainly pacifies wind-yang. The third prescription mainly clears phlegm-heat. The fourth prescription mainly pacifies wind-yang. (They appear similar but are substantially different.)
Ms. Xu suffered from dysentery during pregnancy. The physician administered warming and tonifying herbs, resulting in severe chest and abdominal pain, syncope, and throat ulceration. Swallowing water or food was obstructed. Mengying diagnosed the pulse as surging and rapid, and the tongue as crimson and dry. He urgently applied Xile San to the throat and administered a decoction containing Rhinoceros horn (Rhinocerotis Cornu), Scrophularia root (Scrophulariae Radix), Sea sponge (Spongilla), Bamboo shavings (Bambusae Caulis in Succo), Fritillaria bulb (Fritillariae Cirrhosae Bulbus), Gardenia fruit (Gardeniae Fructus), Aster root (Asteris Radix), Anemarrhena rhizome (Anemarrhenae Rhizoma), Dendrobium stem (Dendrobii Caulis), Sophora root (Sophorae Flavescentis Radix), Belamcanda rhizome (Belamcandae Rhizoma), Honeysuckle flower (Lonicerae Japonicae Flos), and Melia fruit (Meliae Fructus). After the fetus was expelled, already decayed, the throat and abdominal ailments healed subsequently. Continuation treatment used sweet-cool herbs to clear heat and preserve fluids. (The condition is excess-heat with yin deficiency, with liver wind counterflowing upward. The severe chest and abdominal pain indicates wind-yang counterflow invading lung and stomach. The prescription focuses on pacifying wind-yang. Pounded rhinoceros horn decocted first, 4 qian; Scrophularia slices soaked and decocted, dregs removed, 1 liang; Sea sponge decocted first, 2 liang; Ginger-processed bamboo shavings, 3 qian; Fritillaria bulb pounded, 4 qian; Black gardenia peel, 3 qian; Aster fluff, 1.5 qian; Anemarrhena rhizome stir-fried with wine, 3 qian; Dendrobium stem decocted first, 1 liang; Sophora root stir-fried with wine, 1 qian; Ginger-processed belamcanda rhizome, 3 qian; Jizhou honeysuckle flower, 1.5 liang; Melia nut pounded and decocted first, 2 qian.)
Grandmother of Xu Peizhi, over seventy years old, had long suffered from strangury and leukorrhea, and frequently developed wind-spots. Mengying palpated her pulse as wiry and slippery, tongue crimson and mouth dry. He often used Rhinoceros horn (Rhinocerotis Cornu), Rehmannia root (Rehmanniae Radix), Erzhi Wan (Ligustri Lucidi Fructus and Ecliptae Herba), Scutellaria root (Scutellariae Radix), Artemisia annua herb (Artemisiae Annuae Herba), Vitex root (Viticis Negundo Radix), Scrophularia root (Scrophulariae Radix), Turtle plastron (Testudinis Carapax et Plastrum), and Cuttlebone (Sepiae Endoconcha) to quell acute manifestations. He modified Ganlu Yin to regulate chronic conditions. People suspected the medicine was too cooling. Mengying said, "Tailor the clothes to the body. The patient has vigorous yang and abundant qi and blood. Observing the pulse and complexion, treatment must be so." The patient then stated, "More than ten years ago, I accidentally suffered from metrorrhagia and widely took warming tonics, thus forming this ailment." It was then understood that although congenital yang qi was sufficient, it was酿 (brewed/induced) into disease by medication. At the end of autumn, she suffered chills and fever resembling malaria, irritability, insomnia, extreme thirst, easy hunger, and inability to eat. Mengying palpated the pulse as significantly wiry and rapid, and prescribed methods to clear the lungs, dispel phlegm, soften the liver, and replenish fluids. Gradually, she improved. In winter, upon inquiry about the origin, he discussed it as "Tuoying" (Loss of Essence) in the elderly. He formulated based on "tonifying qi for blood loss." After three or four doses, appetite increased sharply. Soon after, bleeding was heavy, sleep and appetite ceased. Mengying was consulted again and maintained the previous opinion. She gradually recovered. (Long-term strangury and leukorrhea indicate blood deficiency causing wind movement. Wiry and slippery pulse indicates wind-yang stirring phlegm. Crimson tongue and dry mouth indicate yin deficiency with blazing heat. Pounded rhinoceros horn 4 qian, decocted first in 8 zhong; Large Rehmannia root soaked and decocted, dregs removed, 8 qian; Ligustrum lucidum fruit pounded, 5 qian; Eclipta herb, 4 qian; Scutellaria root stir-fried until charred, 3 qian; Fresh Artemisia annua herb, 1.5 qian; Fragrant Vitex root, 3 qian; Blood turtle plastron pounded and decocted first, 4 liang; Scrophularia slices soaked and decocted, dregs removed, 1 liang; Vinegar-processed cuttlebone pounded and decocted first, 4 qian. Clearing lung, dispelling phlegm, softening liver, replenishing fluids formula. Irritability, insomnia, etc., indicate wind-yang harming the lung. Prescription focus is pacifying wind-yang. Large Rehmannia root 8 qian, Scrophularia slices 1 liang, both soaked and decocted, dregs removed; Jizhou honeysuckle flower 1.5 liang; Dendrobium stem decocted first, 1 liang; Raw gypsum decocted first, 1.6 liang; Soybean sprouts, 3 qian; Sea sponge decocted first, 2 liang; Water chestnut, 1 liang; Inula flower wrapped and decocted first, 3 qian; Oyster shell pounded and decocted first, 2 liang; Soybean sprouts treat chills and fever like malaria.)
Mrs. Wang, over fifty, initially suffered from redness in the left eye, which gradually progressed to fever. The physician administered warming and dispersing herbs, leading to diarrhea and cold extremities. Subsequent administration of tonics caused the old mass in the lower abdomen to attack and hurt. The situation was critical. Mengying palpated the pulse as very wiry and choppy, tongue crimson and thirsty. He prescribed Cistanche stem (Cistanches Stem), Tangerine seed (Citri Reticulatae Semen), Angelica root (Angelicae Sinensis Radix), Corydalis rhizome (Corydalisis Rhizoma), Turtle plastron (Testudinis Carapax et Plastrum), Pyrite (Hematitum), Cuttlebone (Sepiae Endoconcha), Poria sclerotium (Poria), Gardenia fruit (Gardeniae Fructus), Melia fruit (Meliae Fructus), Evodia fruit (Evodiae Fructus), and Coptis rhizome (Coptidis Rhizoma). After several doses, she stabilized. (This illness involves internal movement of liver wind causing syncope. When liver wind moves, qi of the whole body counterflows upward, causing the old lower abdominal mass to attack and hurt. Wiry pulse indicates yin deficiency and liver heat; choppy pulse indicates deficiency of yang within yin. Heavy use of turtle plastron and pyrite to subdue and anchor. Blood turtle plastron pounded, 4 liang; Pyrite pounded, 5 qian; Vinegar-processed cuttlebone pounded, 4 qian; these three simmered first in 8 zhong; Dried Cistanche stem, 3 qian; Tangerine seed pounded and added later, 1 qian; Box-body Dongquiang, 2 qian; Corydalis rhizome, 1.5 qian; Yunling (Poria), 3 qian; Black gardenia peel, 3 qian; Melia nut pounded and decocted first, 3 qian; Light Evodia fruit, 6 fen; Wine-processed Coptis rhizome, 8 fen.)
Ms. He delivered in early winter. The next day, she had one loose stool, followed immediately by fever, spasms, syncope, delirium, and madness. Mengying examined her: pulse wiry and slippery, lochia still flowing. He said this was latent summer-heat before delivery, triggered by postpartum blood deficiency and phlegm stagnation. He administered a large dose of Rhinoceros horn (Rhinocerotis Cornu), Antelope horn (Saiga Tataricae Cornu), Scrophularia root (Scrophulariae Radix), Bamboo leaf (Bambusae Folium), Anemarrhena rhizome (Anemarrhenae Rhizoma), Trichosanthes root (Trichosanthis Radix), Gardenia fruit (Gardeniae Fructus), Melia fruit (Meliae Fructus), and Honeysuckle flower (Lonicerae Japonicae Flos). Red rashes appeared all over the body, spasms stopped, and consciousness cleared. She was then regulated with clearing and purifying methods and recovered. (Latent summer-heat before delivery, triggered by postpartum blood deficiency and phlegm stagnation, stirred up wind-yang. Pounded rhinoceros horn 4 qian; Antelope horn tip 4 qian; Melia nut pounded 3 qian, all decocted first in 8 zhong; Scrophularia slices soaked and poured, dregs removed, 8 qian; Fresh bamboo leaves, 2 qian; Anemarrhena rhizome stir-fried with wine, 3 qian; Nanhuafen (Trichosanthes root), 5 qian; Gardenia peel stir-fried with wine, 3 qian; Jizhou honeysuckle flower, 1.5 liang. Prescription focus is pacifying wind-yang to cleanse phlegm-heat.)
Ms. Hu, just coming of age, suffered from vertigo in previous years. Mengying palpated a slippery pulse and treated it as phlegm. One or two doses did not cure it. Another doctor claimed it was deficiency and administered tonics with good effect. She believed it was an excess condition. However, in winter this year, the illness recurred. She directly took tonics. Half a month later, sleep and appetite were lost. She was startled by sounds, shivered, sweated spontaneously, and limbs were cold as ice. It was thought to be long-standing deficiency about to collapse. She begged Mengying for help. Pulse extremely fine and rapid, eyes red, constipation, chest below blocked as if obstructed. He firmly refuted it as not a deficiency syndrome. Phlegm-fluids pathology, when suddenly tonified, may seem comfortable. Only those with insight are not deceived by the disease. He administered Inula flower (Inulae Flos), Oyster shell (Conchae Ostreae), Bamboo shavings (Bambusae Caulis in Succo), Fritillaria bulb (Fritillariae Cirrhosae Bulbus), Clam shell (Conchae Arcae), Trichosanthes root (Trichosanthis Radix), Mulberry leaf (Mori Folium), Gardenia fruit (Gardeniae Fructus), Trichosanthes peel (Trichosanthis Pericarpium), Allium macrostemon bulb (Allii Macrostemonis Bulbus), Coptis rhizome (Coptidis Rhizoma), and Immature bitter orange (Aurantii Fructus Immaturus). After several doses, she stabilized. Vertigo persisted, so he removed Oyster shell, Allium macrostemon, Trichosanthes peel, and Immature bitter orange, adding Scrophularia root (Scrophulariae Radix), Chrysanthemum flower (Chrysanthemi Flos), Erzhi Wan (Ligustri Lucidi Fructus and Ecliptae Herba), and Sanjia (Oyster shell, Turtle plastron, Soft-shelled turtle carapace). Continuous服用 for a month allowed her to get out of bed. (This syndrome is yin deficiency with phlegm excess. Slippery pulse and loss of appetite indicate phlegm excess. Extremely fine and rapid pulse indicates yin deficiency with heat. Constipation and chest fullness indicate stomach excess. Red eyes indicate fire generated by phlegm stagnation. Warming tonics add heat and brew phlegm. Dispelling phlegm cleanses heat. Inula flower wrapped and decocted first, 3 qian; Raw oyster shell pounded and decocted first, 1.2 liang; Ginger-processed bamboo shavings, 3 qian; Fritillaria bulb pounded, 4 qian; Raw clam shell pounded and decocted first, 5 qian; Ginger-processed Trichosanthes root, 4 qian; Winter mulberry leaves, 2 qian; Ginger-processed gardenia peel, 3 qian; Ginger-processed Trichosanthes peel, 3 qian; Western Allium macrostemon pounded, 1.5 qian; Ginger-processed Coptis rhizome, 8 fen; Stir-fried immature bitter orange, 1.5 qian. Later removed oyster shell, Allium macrostemon, Trichosanthes peel, immature bitter orange; added Scrophularia slices 8 qian, Hangbaiju (Chrysanthemum) 2 qian, Ligustrum lucidum fruit pounded 5 qian, Eclipta herb 4 qian, Raw oyster shell pounded 4 liang, Blood soft-shelled turtle carapace pounded 2 liang, Blood turtle plastron pounded 1 liang.)
Mrs. Wang, after suffering from dysentery in early autumn, had persistent loose stools. Then she became pregnant, fearing miscarriage. She continuously took tonics. By mid-winter, both eyes were red and obscured, qi counterflow disturbed sleep, epigastric pain refused pressure, phlegm-cough prevented eating, extreme thirst without urination. Mengying palpated a very slippery and rapid pulse. He said this was a disease brewed by warming tonics. The dysentery in autumn originally belonged to pathogenic factors of damp-heat from summer-heat. Since it was not cleared and resolved, it lingered as loose stools. Since conception, four months had passed. Fear of miscarriage led to harsh tonification, reversing the descending function of lung and stomach, causing chest blockage and pain, wheezing and coughing preventing lying down. Fearing upper wheezing and lower diarrhea leading to collapse, more tonification abolished regulatory mechanisms. Urinary obstruction and lack of appetite, turbid qi congested to clear orifices, causing blindness. He prescribed Glehnia root (Glehniae Radix), Clam shell (Conchae Arcae), Loquat leaf (Eriobotryae Folium), Winter melon seed (Benincasae Semen), Sea floating stone (Lithostroma), Inula flower (Inulae Flos), Perilla seed (Perillae Fructus), Apricot kernel (Armeniacae Semen Amarum), Coptis rhizome (Coptidis Rhizoma), Immature bitter orange (Aurantii Fructus Immaturus), Sea floating stone (Lithostroma), Scutellaria root (Scutellariae Radix), Gardenia fruit (Gardeniae Fructus), heavily adding Fritillaria bulb (Fritillariae Cirrhosae Bulbus). After two doses, she felt hungry, could get out of bed, and see. (Warming tonics brew phlegm and burn yin fluids, hence Glehnia root and Sea floating stone are used in the phlegm-dispelling formula. If yin is not restored, hot phlegm cannot transform. Beiglehnia root 8 qian; Raw clam shell and Sea floating stone each 5 qian, decocted first together; Ginger-processed loquat leaf brushed and wrapped, 3 qian; Raw winter melon seed, 4 qian; Inula flower wrapped and decocted first, 3 qian; Raw perilla seed ground, 2 qian; Ginger-processed Coptis rhizome, 8 fen; Stir-fried immature bitter orange, 1.5 qian; Light sea floating stone decocted first, 2 liang; Ginger-processed Scutellaria root, 1.5 qian; Ginger-processed gardenia peel, 1.5 qian; Fritillaria bulb pounded, 8 qian.)
Huang Lüjí suffered from vomiting and pain. Mengying had cured him. In mid-winter, it recurred. Other doctors treated him for seven days without grain intake. Bowels and bladder were obstructed, flesh wasted away rapidly. Mengying was consulted again. He prescribed Inula flower (Inulae Flos), Oyster shell (Conchae Ostreae), Bamboo shavings (Bambusae Caulis in Succo), Poria sclerotium (Poria), Evodia fruit (Evodiae Fructus), Coptis rhizome (Coptidis Rhizoma), Persimmon calyx (Kaki Calyx), Melia fruit (Meliae Fructus), and Corydalis rhizome (Corydalisis Rhizoma). One dose showed improvement, three doses cured. (This illness is liver yang insulting stomach, lung failing to descend, spleen also weak. Inula flower wrapped and decocted first, 3 qian; Raw oyster shell pounded and decocted first, 2 liang; Ginger-processed bamboo shavings, 3 qian; Yunling (Poria), 3 qian; Light evodia fruit, 6 fen; Ginger-processed Coptis rhizome, 1 qian; Dried persimmon calyx ten pieces; Melia nut pounded and decocted first, 3 qian; Corydalis rhizome, 1.5 qian.)
Xu Zhongyun suffered from abdominal pain and lack of appetite. Doctors gave Ginseng root (Ginseng Radix), Aconite root (Aconiti Lateralis Radix Preparata), Dried ginger (Zingiberis Rhizoma), and Atractylodes macrocephala rhizome (Atractylodis Macrocephalae Rhizoma). Pain and distension worsened daily, unable to take even water, lethargic. Mengying palpated wiry and fine pulse, yellow greasy coating. He administered Immature bitter orange (Aurantii Fructus Immaturus), Magnolia bark (Magnoliae Officinalis Cortex), Evodia fruit (Evodiae Fructus), Coptis rhizome (Coptidis Rhizoma), Gardenia fruit (Gardeniae Fructus), Melia fruit (Meliae Fructus), Cyperus rhizome (Cyperi Rhizoma), Tribulus fruit (Tribuli Fructus), and Corydalis rhizome (Corydalisis Rhizoma). Two doses brought bowel movement, pulse recovery, coating retreat, and return of appetite. (Wiry pulse indicates liver heat; fine pulse indicates yin deficiency with concurrent yang deficiency. Stir-fried immature bitter orange, 1.5 qian; Processed magnolia bark, 1 qian; Light evodia fruit, 6 fen; Ginger-processed gardenia peel, 3 qian; Ginger-processed Coptis rhizome, 1 qian; Melia nut pounded and decocted first, 3 qian; Raw cyperus rhizome, 8 fen; Raw tribulus fruit, 3 qian; Corydalis rhizome, 1.5 qian.)
Bi Shi suffered from phlegm-cough disturbing sleep. Doctors gave tonics and astringents until tears and mucus ceased. Eyes closed, unaware of hunger, bowels and bladder obstructed, dry cough without phlegm, qi counterflow with spontaneous sweating. Mengying palpated right cun pulse deep and slippery, left hand pulse fine, rapid, and wiry. It was elderly yin deficiency, warm pathogen in lung not cleared, imprisoned by tonics.
