Chen Jishang suffered from strangury (Lin Zheng, 淋证: a syndrome characterized by frequent, urgent, and painful urination) for a long time without recovery. The condition persisted until the humid and hot summer season. Mengying examined him and said, "This is an easy matter to treat." He prescribed Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), and the patient recovered. (The causes of strangury are diverse. This case was due to the failure of Spleen Yang, with Qi failing to secure essence. The right pulse must be powerless upon heavy pressure. Prescription: Fried Codonopsis (Ludangshen, 潞党参) five qian, Fried American ginseng (Xishen, 西洋参) five qian, Fried licorice root (Fengcao, 粉草) three qian, Fried Atractylodes macrocephala (Baizhu, 白术) three qian, Fried Coix seed? No, Fried Cangzhu (Cangshu, 苍术) four qian, Fried scorched Chenpi (aged tangerine peel, one qian five fen), Fried dried ginger (Ganjiang, 干姜) five qian, Prepared Pinellia (Banxia, 半夏) five qian, Fried Psoralea corylifolia (Guzhi/Buguzhi, 故纸) ground five qian.)
Chen Heguan suffered from abdominal masses (Pi Ji, 痞积) causing abdominal distension, fever, and dry coughing. He had a good appetite but became emaciated and yellow. Pediatric medicines were widely administered without effect. Mengying prescribed Coptis chinensis (Huanglian, 黄连), White peony root (Baishao, 白芍), Oyster shell (Muli, 牡蛎), Turtle plastron (Biejia, 鳖甲), Chicken gizzard lining (Jijin, 鸡金), Five-grain insect (Wugu chong), Xiaticur (Xiati qu), Papaya (Mugua, 木瓜), Hawthorn fruit (Shanzha, 山楂), Toosendan fruit (Lianshi, 楝实), Tangerine peel (Chenpi, 橘皮), Platycodon root (Jiegeng, 桔梗), Inula flower (Xuanfu, 旋覆), Gardenia (Zhizi, 栀子), and Moutan bark (Danpi, 丹皮). After one dose, symptoms improved; after more than ten days, he recovered. (This case involved hyperactivity of both Liver and Stomach, with Liver Heat attacking the Spleen. Fever and dry cough indicated Yin deficiency with Liver Heat invading the Lungs. Good appetite indicated Stomach Heat causing hunger. Yellowish complexion, emaciation, and loose stools indicated the Spleen being overcome by the Liver, leading to impaired Spleen transportation. Red urine indicated excess heat in the Liver. The pathogenic heat ultimately resided in the Qi level of the Liver and Lungs. Prescription: Wine-fried Coptis chinensis (Chuanlianhuan/Lian, 川连) six fen, Wine-fried White peony root (Baishao, 白芍) one qian five fen, Bitter Platycodon root (Kujiegeng, 桔梗) one qian, Inula flower wrapped in silk (Xuanfuhua, 旋复花) eight fen, Black Gardenia skin (Heizhipi, 栀皮) one qian five fen, Pink Moutan bark (Fendanpi, 丹皮) one qian, Five-grain insect two qian, Scorched Hawthorn flesh pounded one qian, Tangerine peel seven fen. Second prescription removed Chuanlian, Baishao, Jiegeng, and Zhipi; added Toosendan fruit pounded and decocted first (Chuanlianshi, 川楝实) two qian, Honey-fried Chicken gizzard lining (Zhijijin, 炙鸡金) one qian five fen. Third prescription removed Xuanfu and Danpi; added Blood Turtle plastron (Xuemiebia, 血鳖甲) four qian, Raw Oyster shell (Shengmuli, 生牡蛎) eight qian. These two ingredients were first simmered over charcoal fire for six hours, and the decoction used to cook the herbs. Xiaticur pounded one qian, Aged Papaya (Chen mugua, 陈木瓜) two qian.)
Duan Yaoqing's mother, over seventy years old, suffered from cholera with muscle cramps (Huoluan zhuanjin, 霍乱转筋). Mengying prescribed his self-made Lianpo Yin (Coptis and Magnolia Drink), and she recovered after three doses. The original formula treats cholera caused by damp-heat stagnation and also promotes digestion and resolves phlegm. Original formula: Ginger-processed Magnolia bark (Jiangzhin Houpo, 姜制浓朴) two qian, Ginger-fried Elegant Coptis (Jiangchao Yalian, 姜炒雅连) one qian, Acorus (Shichangpu, 石菖蒲) one qian, Prepared Pinellia (Zhibanxia, 制半夏) one qian, Fried fragrant fermented soybean (Chaoxiangchi, 炒香豉) three qian, Black Gardenia skin (Heizhipi, 黑栀皮) three qian, Fresh reed rhizome (Huoshui lugen, 活水芦根) two liang.
Shi Songxi's wife suffered from chronic phlegm-cough (Tansou, 痰嗽) for a long time, which other doctors failed to cure. Mengying palpated the pulse and said, "This is not wind-cold." He prescribed Glehnia root (Beishashen, 北沙参), Prepared Rehmannia root (Shudi, 熟地), Lily bulb (Baihe, 百合), Ophiopogon tuber (Maidong, 麦冬), Polygonatum odoratum (Yuzhu, 玉竹), Fritillaria bulb (Beimu, 贝母), Aster tataricus root (Ziwan, 紫菀), Loquat leaf (Piyè, 枇叶), Orange peel fried in salt water (Yanshui chao juhong, 盐水炒橘红), and Bird's nest (Yanwo, 燕窝). After one dose, symptoms improved; after several doses, she recovered. (This case was Yin deficiency with Lung dryness. Previous doctors likely treated it as wind-cold, repeatedly using warm-dispersing agents, thereby damaging the Yin of the Upper Jiao Qi level. Food stagnation is an exterior excess syndrome; this case is an interior deficiency syndrome. Prescription: Glehnia root (Beishashen, 北沙参) eight qian, Large Prepared Rehmannia root soaked in boiling water, strained, and taken five qian, Lily stamen (Baihexu, 百合须) three qian, Flower Ophiopogon (Huamaidong, 花麦冬) three qian, Sichuan Fritillaria bulb pounded (Chuanbeimu, 川贝母) three qian, Plump Polygonatum (Fei yuzhu, 肥玉竹) two qian, Fresh Loquat leaf brushed and wrapped (Xian piye, 鲜枇叶) three qian, Light salt-water fried Tangerine peel (Danshui chao jupi, 淡盐水炒橘皮) one qian five fen, Bird's nest wrapped and decocted first (Yanwo bao xian, 燕窝包先) three qian.)
In early autumn, he again suffered from epigastric pain radiating to the shoulder tip. Previously considered as Liver Qi stagnation, he switched to taking Qi-breaking and reducing agents. Mengying said, "That is also incorrect." He prescribed Amomum villosum (Sharen, 砂仁), Fried Prepared Rehmannia root (Chao shudi, 炒熟地), Honey-fried Tangerine peel (Zhi juhong, 炙橘红), Toosendan fruit (Lianshi, 楝实), Corydalis rhizome (Yanhusuo, 延胡), Goji berry (Gouqi, 枸杞), Angelica sinensis (Danggui, 当归), Poria cocos (Fuling, 茯苓), Mulberry fruit (Sangshen, 桑葚), and Tribulus terrestris (Jili, 蒺藜). The treatment was highly effective, and conception followed soon after. (Yin deficiency makes the Liver and Lungs prone to dryness. This case involved Yin deficiency with Liver hyperactivity, where Liver Yang attacked the Stomach. Excess Liver Yang indicates insufficient Liver Yin, not true excess. Taking Qi-breaking agents further damaged Liver Yin. Prescription: Amomum villosum powder two fen mixed with fried large Prepared Rehmannia root (remove Amomum) eight qian, Lai Tangerine peel (Lai juhong, 赖橘红) one qian, Toosendan fruit pounded and decocted first (Chuanlianshi, 川楝实) four qian, Corydalis rhizome (Yanhusuo, 延胡索) one qian, Sweet Goji berry (Gan gouqi, 甘枸杞) three qian, Box Angelica sinensis body (Xiang guishen, 箱归身) two qian, White Poria cocos (Bai fuling, 白茯苓) three qian, Dry Mulberry fruit pounded and decocted first (Gan sangshen, 干桑葚) three qian, Raw Tribulus terrestris de-thorned (Sheng baiji, 生白蒺) one qian five fen.)
Shi Zhiqing suffered from an external contraction. Dr. Zhang continuously prescribed Chaihu (Bupleurum) and Ge Gen (Pueraria), and the fever gradually subsided. However, upon recurrence, the fever was more severe. Mengying first used Gardenia (Zhizi, 栀子), Fermented soybean (Douchi, 豆豉), Scutellaria baicalensis (Huangqin, 黄芩), and Coptis chinensis (Huanglian, 黄连) to clear and resolve the ascending and floating heat. (Generally, when excess heat is complicated by external contraction, initial misuse of warm-dispersing agents may temporarily reduce fever because the patient's inherent Yin fluid can still supply the demand for sweating induced by warm agents. After such sweating, Yin is damaged and heat flares up, making the fever worse. Physicians must be aware of this. Mengying first used Zhizi, Douchi, Huangqin, and Huanglian to clear the ascending heat. Once the pathogen entered the Stomach Fu organ, and the pulse became deep, solid, and slippery, he then used Chengqi Tang (Major Order the Qi Decoction) to purge. This is the fixed method of treating diseases in their early, middle, and late stages. Prescription: Black Gardenia skin (Heishanzhipi, 黑山栀皮) three qian, Light Fermented soybean (Dandouchi, 淡豆豉) three qian, Wine-fried Elegant Coptis (Jiu chao Yalian, 酒炒雅连) eight fen, Wine-fried Scorched Scutellaria (Jiu chao Kuning, 酒炒枯芩) two qian five fen, Fresh Loquat leaf brushed and wrapped (Xian pipie, 鲜枇杷叶) three qian, Ginger-processed Bamboo shavings (Jiang zhuru, 姜竹茹) three qian, Forsythia fruit (Lianqiao qiao, 连翘壳) three qian, Fresh reed rhizome (Huoshui lugen, 活水芦根) one liang, Deworm Reed Stem (Chai shihu, 钗石斛) pounded and decocted first five qian, Thin Tangerine peel (Bo juhong, 薄橘红) and Inula flower wrapped in silk (Xuanfu, 旋复), each one qian five fen.) Waiting for the pathogen to enter the Stomach Fu organ, with a deep, solid, and slippery pulse, he directly used Chengqi Tang to purge. (Prescription: Raw Rhubarb (Sheng dahuang, 生厢黄/大黄) four qian, soaked in boiling water, strained; Fried Immature Bitter Orange (Chao zhishi, 炒枳实) two qian; Toosendan fruit pounded and decocted first (Chuanlianshi, 川楝实) two qian; Acorus (Shichangpu, 石菖蒲) one qian; Lai Tangerine peel (Lai juhong, 赖橘红) one qian five fen; Aged Arisaema cum Bile (Chen danxing, 陈胆星) one qian, stewed and taken together.) After taking the second prescription, the next day there was high fever, profuse sweating, and intense thirst craving drinks. Mengying said, "This is the expulsion of intestinal filth revealing the heat in the channels." He prescribed Zhuye Shigao Tang (Leaf and Gypsum Decoction) for two doses, and the patient stabilized. (Prescription: Fresh Bamboo leaf (Xian zhuye, 鲜竹叶) two qian, Raw Gypsum (Sheng shigao, 生石膏) one liang six qian, decocted first; Wine-fried Anemarrhena (Jiu chao Zhimu, 酒炒知母) three qian; Black Gardenia skin (Heishanzhipi, 黑山栀皮) three qian; Fresh Loquat leaf brushed and wrapped (Xian pipie, 鲜枇杷叶) three qian; Fresh reed rhizome (Huoshui lugen, 活水芦根) two liang; Fried Immature Bitter Orange (Chao zhishi, 炒枳实) three qian; Silkworm frass (Wancansha, 晚蚕砂) five qian; Aged Papaya (Chen mugua, 陈木瓜) one qian five fen.) Subsequently, he was regulated by nourishing Yin and replenishing fluids, leading to recovery.
Zhu suffered from dysentery. He had tried all sorts of exterior-releasing, purging, and cloying/nourishing medicines, and his condition became critical. Mengying examined him and found mental confusion, deafness, epigastric fullness, dry mouth, body heat, extreme hardness and pain around the umbilicus, passage of five-colored stool dozens of times a day, painful and difficult urination, limb convulsions, and frequent syncope. Mengying said, "This is pathogenic damp-heat that was not cleared and resolved. Exterior release and purging have damaged the healthy Qi, allowing the pathogen to transmit to the Jueyin channel. If cloying tonics are used to tonify and trap the pathogen, it will become intractable. Healthy Qi is deficient, pathogenic factor is excess; the danger is extreme." He prescribed Baitouweng Tang (Peony Head Decoction) plus Toosendan fruit (Lianshi, 楝实), Cistanche (Rongrong, 苁蓉), Scutellaria (Qin, 芩), Coptis (Lian, 连), Gardenia (Zhi, 栀), White Peony (Shao, 芍), Honeysuckle (Yinhua, 银花), Dendrobium (Shihu, 石斛), Mulberry leaf (Sangye, 桑叶), Tangerine leaf (Juye, 橘叶), Antelope horn (Lingyangjiao, 羚羊角), Oyster shell (Muli, 牡蛎), Cuttlefish bone (Haizhe, 海 ), Turtle plastron (Biejia, 鳖甲), and Chicken gizzard lining (Jijin, 鸡金). Large doses were administered frequently. After one dose, convulsions and syncope halved; after four doses, they ceased; after ten days, stool color normalized; urination became clear and abundant; food intake gradually increased; after half a month, the hardness around the umbilicus disappeared completely. He then switched to nourishing Yin for regulation, and recovered after more than a month. (In treating any disease, one must first know the ease or difficulty of Qi ascending and descending. Medicines follow Qi movement; without Qi moving with the medicine, efficacy cannot be achieved. The original case discussion is very correct, but the key to efficacy in this difficult case lies in the phrases "limb convulsions" and "frequent syncope." Limb convulsions and frequent syncope indicate Liver Wind rising without descending. If Wind does not subside, Qi does not descend; if Qi does not descend, the medicinal Qi has no basis to move downward. Therefore, sedating-Yang and bitter-cold agents in the formula must not be neglected lightly. Treat specific syndromes with specific methods; treat acute conditions urgently. This is the principle and method. Prescription: Baitouweng (Baitouweng, 白头翁) three qian, Toosendan kernel pounded and decocted first (Chuanlianhé, 川楝核) four qian, Light Cistanche (Dan rongrong, 淡苁蓉) three qian, Wine-fried Scorched Scutellaria (Jiu chao Kuning, 酒炒枯芩) three qian, Wine-fried Elegant Coptis (Jiu chao Yalian, 酒炒雅连) one qian, Black Gardenia skin (Heizhipi, 黑栀皮) one qian five fen, Wine-fried White Peony (Jiu chao Baishao, 酒炒白芍) one qian five fen, Jinan Honeysuckle (Ji yinhua, 济银花) one liang five qian, Deworm Reed Stem (Chai shihu, 钗石斛) pounded and decocted first one liang, Winter Mulberry leaf (Dong sangye, 冬桑叶) four qian, Fresh Tangerine leaf added later (Xian juye, 鲜橘叶) seven slices, Antelope horn ground and taken (Lingjiao mo chong, 羚角磨冲) two qian, Light Cuttlefish bone (Dan haizhe, 淡海 ) two liang, Honey-fried Chicken gizzard lining ground (Zhi jijin yan, 炙鸡金研) five qian, Raw Oyster shell (Sheng muli, 生牡蛎) eight liang, Blood Turtle plastron (Xue biejia, 血鳖甲) two liang. These two ingredients were pounded and first simmered over charcoal fire for eight hours, and the decoction used to cook the herbs.)
Old Man Wang suffered from phlegm-cough, loss of appetite, asthma preventing lying down, penile retraction, constipation, and mental instability ("heart shaking unable to grasp") in mid-autumn. The situation was extremely critical. Mengying diagnosed, "The root is about to detach; this is not a typical disease." He prescribed Bawei Dihuang Tang (Eight-Ingredient Rehmannia Pill) minus Moutan bark and Alisma, combined with Shengmai San (Generate the Pulse Powder), plus Purple Quartz (Zishiying, 紫石英), Lead (Qingqian, 青铅), Dragon Bone (Longgu, 龙骨), Oyster shell (Muli, 牡蛎), Walnut meat (Hetaorou, 胡桃肉), Toosendan fruit (Lianshi, 楝实), and Cistanche (Rongrong, 苁蓉). After bowel movement, symptoms decreased. Then Cistanche and Ophiopogon were removed, and after ten days, he recovered. (Recognizing this case as Yin deficiency with floating Yang depends entirely on "penile retraction, constipation, heart shaking unable to grasp." Prescription: Large Prepared Rehmannia root (Da shudi, 大熟地) eight qian, Huai Yam (Huai shanyao, 淮山药) four qian, Cornus officinalis fruit (Shanyurou, 山萸肉) three qian, White Poria cocos sliced dry (Bai fuling ganqie, 白茯苓干切) three qian, Flower Ophiopogon (Huamaidong, 花麦冬) three qian, Schisandra berry pounded (Wuweizi, 五味子) three qian, Wine-fried Anemarrhena (Jiu chao Zhimu, 酒炒知母) one qian five fen, Wine-fried Phellodendron bark (Jiu chao Chuanhuangbai, 酒炒川黄柏) one qian, plus Purple Quartz pounded (Zishiying, 紫石英) five qian, Lead (Qingqian, 青铅) one liang, Dragon Bone (Longgu, 龙骨) two liang, Oyster shell (Muli, 牡蛎) eight liang. These four ingredients were first simmered over charcoal fire for eight hours, and the decoction used to cook the herbs. Whole-skin Walnut meat (Lianyi hetaorou, 连衣胡桃肉) five qian, Toosendan kernel pounded and decocted first (Chuanlianhé, 川楝核) three qian, Light Cistanche (Dan rongrong, 淡苁蓉) three qian.)
Shao Ketingsuffered from phlegm-cough in winter, with facial swelling and slight asthma. Doctors administered warming and tonifying Qi-grasping medicines, worsening the asthma and cough. Saliva flowed freely, penis and scrotum swelled, legs became swollen and hard down to the heels, unable to stand slightly. Opening the mouth caused suffocating asthma, daring not speak. Tilting the head back caused coughing and sore throat, unable to lie down even slightly. Sputum was yellow, thick, and bloody. Urine was slightly yellow and long. Mengying observed a string-like, slippery, and forceful pulse, and said, "This is solitary Yang blazing internally in an elderly person, with seasonal dry-fire pressing externally. The external illness might be treatable, but True Yin may not recover. Moreover, his stool has always been like sheep dung, indicating dried-up fluids. Further administration of warming tonics would be adding fuel to the fire." He prescribed Baihu Tang (White Tiger Decoction) combined with Xiebai San (Drain White Powder) plus American Ginseng (Xiyangshen, 西洋参), Fritillaria (Beimu, 贝母), Snowdrop fritillary bulb (Huafen, 花粉), and Scutellaria (Huangqin, 黄芩), administered in large doses. He also instructed frequent drinking of crushed Northern Pear juice to moisten the throat and alleviate the rebellious fire above. After several doses, the fire gradually subsided. (This case was Yin deficiency with phlegm excess. Differentiation relies on the string-like, slippery, and forceful pulse. Correlating pulse and symptoms, the formula used Raw Gypsum decocted first (Sheng shigao, 生石膏) one liang, Wine-fried Anemarrhena (Jiu chao Zhimu, 酒炒知母) three qian, Raw Licorice root (Sheng fengcao, 生粉草) three qian, Raw Mulberry root bark (Sheng sangpi, 生桑皮) four qian, Fresh Cortex Lycii (Xian digupi, 鲜地骨皮) five qian, American Ginseng (Xiyangshen, 西洋参) three qian, Sichuan Fritillaria bulb pounded (Chuan beimu, 川贝母) one liang, Southern Snowdrop fritillary bulb (Nan huafen, 南花粉) four qian, Wine-fried Scorched Scutellaria (Jiu chao kuning, 酒炒枯芩) three qian. Also, Northern Pear with skin crushed and drunk raw to moisten the throat, unlimited quantity and time.)
He switched to Weijing Tang (Reed Rhizome Decoction) combined with Qingzao Jiufei Tang (Clear Dryness and Rescue the Lung Decoction) plus Cuttlefish bone (Haizhe, 海 ), Clam shell (Geke, 蛤壳), Indigo naturalis (Qingdai, 青黛), Water chestnut (Biqi, 荸荠), and Bamboo sap (Zhuli, 竹沥). After more than ten days, having consumed a hundred jin of pears, the asthma finally stopped. (Prescription: Fresh reed rhizome (Huoshui lugen, 活水芦根) two liang, Raw Winter melon seed (Sheng donggua zi, 生冬瓜子) five qian, Raw Coix seed pounded (Sheng yiren, 生苡仁) eight qian, Glehnia root (Beishashen, 北沙参) eight qian, Raw Licorice root (Sheng fengcao, 生粉草) three qian, Flower Ophiopogon (Huamaidong, 花麦冬) four qian, Sweet Almond (Tian xingren, 甜杏仁) three qian, Honey-roasted Loquat leaf brushed and wrapped (Mi zhi pipie, 蜜炙枇叶) three qian, Winter Mulberry leaf (Dong sangye, 冬桑叶) four qian, Light Cuttlefish bone (Dan haizhe, 淡海 ) four liang, Raw Clam shell pounded and decocted first (Sheng geke, 生蛤壳) one liang, Flying Indigo naturalis (Fei qingdai, 飞青黛) one qian, Water chestnut with skin smashed (Lianpi biqi, 连皮荸荠) two liang, Fresh Bamboo sap taken together (Xian zhuli, 鲜竹沥) three liang.) Subsequently, Tortoise plastron (Guiban, 龟板), Turtle plastron (Biejia, 鳖甲), and Rhinoceros horn (Xijiao, 犀角) were added, cooked in pork broth instead of water, to greatly nourish Yin and subdue Yang. Fire descended, urine became red like logwood juice, and all symptoms resolved. Lower limb swelling subsided progressively with the disease. Over a month, over two hundred jin of pears were consumed. (Previous formula added Blood Tortoise plastron pounded (Xue guiban, 血龟板) four liang, Blood Turtle plastron pounded (Xue biejia, 血鳖甲) two liang, pounded and first simmered over charcoal fire for eight hours, Rhinoceros horn shaved and ground (Bang xijiao, 镑犀角) one qian, Dry pork skin (Gan zhuroupi, 干猪肉皮) half a jin. Rapid fire decoction, skim off floating oil, use broth to cook herbs. Consumed over two hundred jin of pears in a month; special study recommended. Specific syndrome, specific treatment; acute condition, urgent treatment. How can medicine be judged by ordinary theories?) Coinciding with heavy snow and severe cold, he caught a slight chill while changing clothes, causing mild abdominal pain. He drank two bowls of ginger-sugar soup himself, and the asthma and cough recurred, with dry mouth, sore throat, intense thirst, cracked tongue, and insomnia. He reused the previous formula, decocting Green bean (Lvdou, 绿豆) clear soup to replace water for cooking the herbs, and only then did he gradually stabilize. (Green beans should be used in quantity of one sheng.)
Lu, whose wine business was extremely tight and who suffered misfortune, developed a strange disease over time: emaciation, itching, lice emerging from the skin, wriggling when scratched, unresponsive to treatment. Mengying diagnosed, "Sorrow and labor have injured Yang. The spreading sores transform into insects." He prescribed Qi-tonifying medicines plus Chinese Fir wood (Shanmu, 杉木) and Mulberry branch (Sangzhi, 桑枝), leading to recovery. (Yang deficiency must have pulse evidence. Note the phrase "unresponsive to treatment." If it were an excess syndrome, standard anti-parasitic drugs would have worked early. Prescription: Fried Codonopsis (Chao Ludangshen, 炒潞党) five qian, Fried American ginseng (Chao Xishen, 炒西) five qian, Processed Atractylodes lancea (Zhi Maozhu, 制茅术) four qian, Processed Atractylodes macrocephala (Zhi Baizhu, 制白术) three qian, Fried Licorice root (Chao Fengcao, 炒粉草) three qian, Fried Albizia bark (Chao Hehuanpi, 炒合欢皮) three qian, Yun Poria spirit (Yun fushen, 云茯神) three qian, White Poria cocos sliced dry (Bai fuling ganqie, 白茯苓干切) three qian, Quisqualis fruit with shell pounded (Shijunzi lianke, 使君子连壳) three qian, Fried Chinese Fir bark (Chao Shanmupi, 炒杉木皮) three qian, Wine Mulberry branch (Jiu Sangzhi, 酒桑枝) three qian.)
Chen Jishang's mother suddenly suffered from vomiting, lasting all night. Mengying prescribed a strong bitter-cold formula of Scutellaria (Qin, 芩), Coptis (Lian, 连), Gardenia (Zhi, 栀), and Toosendan (Lian, 楝), resulting in excellent recovery. (Acute diseases are rarely deficiency, especially when vomiting lasts all night. Recognize this as excess heat in the Qi level, key in "all night non-stop." Prescription: Wine-fried Scorched Scutellaria (Jiu chao Kuning, 酒炒枯芩) two qian, Wine-fried Elegant Coptis (Jiu chao Yalian, 酒炒雅连) one qian, Toosendan kernel pounded and decocted first (Chuanlianhé, 川楝核) three qian, Black Gardenia skin (Heizhipi, 黑栀皮) three qian, Sichuan Fritillaria bulb pounded (Chuan Beimu, 川贝母) three qian, Raw Winter melon seed (Sheng Donggua zi, 生冬瓜子) four qian, Raw Coix seed pounded (Sheng Yiren, 生苡仁) eight qian, Acorus added later (Shichangpu, 石菖蒲) one qian, Fried Immature Bitter Orange (Chao Zhishi, 炒枳实) one qian, Aged Arisaema cum Bile stewed and taken together (Chen Danxing, 陈胆星) one qian, Ginger-processed Bamboo shavings (Jiang Zhuru, 姜竹茹) three qian, Inula flower wrapped in silk (Xuanfuhua, 旋复花) three qian, Scorched Olive pounded and decocted first (Ku Qingguo, 枯青果) three pieces, Raw Hematite pounded and decocted first (Sheng Zheshi, 生赭石) one liang together.)
Zhang Zhengfeng's wife developed fever immediately after childbirth. After two doses of Shenghua Tang (Generate the New Decoction), fever worsened, and red rashes appeared. Gu Tingquan gave clearing and resolving agents, but three doses were ineffective. He planned to administer Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) and consulted Mengying. Mengying noted a string-like, slippery, and rapid pulse, red and hot face, restlessness, chest fullness, sadness, limb swelling and pain, dozens of white lumps like broad beans on both elbows, one on the tongue, pain obstructing drinking, and no bowel movement for ten days. Mengying said, "This is not only latent summer-heat before pregnancy but also accumulated toxin. Misusing Shenghua Tang aided the pathogen. Fortunately, immediate clearing and resolving prevented coma. Xijiao Dihuang Tang is correct, but I fear it may not be sufficient. I suggest adding American Ginseng (Xiyangshen, 西洋参), Talc (Huashi, 滑石), Anemarrhena (Zhimu, 知母), Honeysuckle (Yinhua, 银花), Human excrement ash (Renzhongbai, 人中白), Trichosanthes seed (Louren, 萎仁), Bamboo resin (Zhuhuang, 竹黄), Fritillaria (Beimu, 贝母), Mulberry leaf (Sangye, 桑叶), and Gardenia (Zhizi, 栀子)." (Red rash with string-like, slippery, rapid pulse indicates pathogen in the Qi level of the Upper Jiao. Prescription: Rhinoceros horn shaved and ground (Bang xijiao, 镑犀角) eight fen, Large Rehmannia root soaked in boiling water, strained (Da Shengdi, 大生地) eight qian, American Ginseng (Xiyangshen, 西洋参) three qian, Western Talc (Xi Huashi, 西滑石) four qian, Jinan Honeysuckle added later (Ji Yinhua, 济银花) eight qian, Southern Snowdrop fritillary bulb (Nan Huafen, 南花粉) four qian, Human excrement ash ground and taken (Renzhongbai, 人中白) eight fen, Ginger-juice fried Trichosanthes seed ground (Jiangzhi chao Louren, 姜汁炒萎仁) three qian, Heavenly Bamboo resin (Tian zhuhuang, 天竹黄) three qian, Sichuan Fritillaria bulb pounded (Chuan Beimu, 川贝母) one liang, Winter Mulberry leaf (Dong Sangye, 冬桑叶) four qian, Black Gardenia skin (Hei Zhipi, 黑栀皮) three qian.) Although the patient was thirsty, she preferred hot drinks, which is common when hot-phlegm obstructs Qi mechanism in the chest. Hence, many phlegm-resolving drugs were used in the formula. After four doses, bowel movement occurred, frequent expectoration of thick phlegm, symptoms decreased, and diet improved. Specialists were ineffective. After ten days, fire flared up again. Mengying continued with sweet-cold therapy. Skin peeled like snake skin, nails renewed. Subsequently, tonifying True Yin led to recovery.
Ye Zhou-san suffered from coughing with rebellious Qi, left-sided headache, thirst, lack of appetite, and watery diarrhea. Mengying said, "Liver Yin and Stomach fluids are mutually deficient, with seasonal dry pathogen pressing externally." He employed methods to nourish Yin, extinguish wind, clear dryness, and replenish fluids. Gradual improvement occurred. After two months, stool became dry and formed, and he recovered. (Left-sided headache indicates Liver Yin deficiency. Thirst and lack of appetite indicate Stomach Yin damage with rebellious heat. Watery diarrhea indicates heat seeking exit, rushing downward without forming stool. After two months of Yin-nourishing drugs, stool became dry because Yin and Yang harmonized and residues were processed. Previously, solitary Yang without Yin could not process residues. Prescription: Baitouweng (Baitouweng, 白头翁) three qian, Wine-fried Phellodendron bark (Jiu chao Chuanhuangbai, 酒炒川黄柏) one qian, Toosendan kernel pounded and decocted first (Lianhé, 楝核) three qian, Flower Ophiopogon (Huamaidong, 花麦冬) four qian, Raw Licorice root (Sheng fengcao, 生粉草) three qian, Deworm Reed Stem (Chai Shihu, 钗石斛) pounded and decocted first one liang, Fresh Bamboo leaf (Xian Zhuye, 鲜竹叶) two qian, Water chestnut with skin (Lianpi Biqi, 连皮荸荠) two liang, Light Cuttlefish bone (Dan Haizhe, 淡海 ) one liang, Aged Papaya (Chen Mugua, 陈木瓜) three qian, Blood Turtle plastron (Xue Biejia, 血鳖甲) two liang pounded and first simmered over charcoal fire for eight hours.)
Zheng vomited a bowlful of blood. Mengying palpated a洪 (Surging) and slippery pulse at the Right Guan position, spontaneous sweating, thirst, and bleeding upon slight movement. He prescribed Baihu Tang (White Tiger Decoction) plus American Ginseng (Xiyangshen, 西洋参) and Charred Rhubarb (Dahuang tan, 大黄炭). One dose brought sudden recovery. (Surging pulse indicates deficiency; slippery pulse indicates excess. Reflect on "bleeding upon slight movement"; this case is mostly excess with little deficiency. Prescription: Raw Gypsum decocted first (Sheng Shigao, 生石膏) eight qian, Wine-fried Anemarrhena (Jiu chao Zhimu, 酒炒知母) three qian, American Ginseng (Xiyangshen, 西洋参) three qian, Charred Rhubarb (Dahuang tan, 大黄炭) one qian, soaked in herbal decoction and taken, dregs discarded.)
Mengying palpated Kang Bo-hou's pulse: slippery and rapid, with intermittent beats on the Left and hurried beats on the Right. The Liver area showed occasional Sparrow-pecking pulse, and the Qi Gate showed Solving-Rope pulse. His face looked like smoked yellow, with sweat on the head, coarse breathing seemingly discontinuous, inability to sit or lie still for a moment, difficult and dark-smelling urination, hardness and tenderness under the heart resembling an inverted bowl, purple edges and yellow coating on the tongue but not very dry. Asked about his suffering, he replied: "Thirsty with sweet/greasy sensation, no desire to eat, closing eyes causes Qi rising and asthma, unbearable irritability, chest discomfort indescribable." Mengying said, "This is due to mistaken tonification of damp-heat, leaving no outlet, filling the Three Jiao, blocking Qi mechanism, and stagnating fluids into phlegm-drink. Doctors seeing cold limbs and sweating do not realize the disease stems from obstruction." (Middle omitted.) Generally, strange pulses and symptoms belong to phlegm. With such chest fullness and no phlegm expectorated, it is because phlegm blocks Qi, and Qi fails to move phlegm. It is appropriate to add Allium macrostemon (Xiebai, 薤白) and Trichosanthes seed (Louren, 萎仁) to Wendan Tang (Warm the Gallbladder Decoction) to open Yang in the chest. Combining with Xiaoxianxiong Tang (Minor Trap-the-Chest Decoction) is a holy method for treating phlegm-fullness. Add Gardenia and Fermented Soybean (Zhizi, 栀子; Douchi, 豆豉) to drain long-stagnant heat and remove discomfort. Assist with Mentha (Lancao, 兰草) to cleanse stale Qi and awaken Spleen/Stomach. (From slippery-rapid, left-intermittent, right-hurried for twenty-one lines, all indicate damp-heat mistaken tonification, transforming excess into deficiency, easy to rise but hard to descend. Must ponder sentence by sentence. Prescription: Pinellia curd (Banxiaqu, 半夏曲) one qian five fen, Light salt-water fried Tangerine peel (Danshui chao Chenpi, 淡盐水炒陈皮) one qian, White Poria cocos (Bai fuling, 白茯苓) one qian five fen, Fried Immature Bitter Orange (Chao Zhishi, 炒枳实) one qian, Ginger-processed Bamboo shavings (Jiang Zhuru, 姜竹茹) four qian, Ginger Gardenia skin (Jiang Zhipi, 姜栀皮) three qian, Western Allium macrostemon smashed and added later (Xi Xiebai, 西薤白) one qian five fen, Ginger-juice fried Trichosanthes seed ground (Jiangzhi chao Louren, 姜汁炒萎仁) three qian, Ginger-juice fried Elegant Coptis (Jiangzhi chao Yalian, 姜汁炒雅连) eight fen, Jian Mint leaves (Jian Lanye, 建兰叶) added later three qian, Fried Fermented Soybean (Chao Xiangchi, 炒香豉) one qian five fen.) Two consecutive doses reduced all symptoms, and pulse harmonized slightly. The patient mistakenly took Rhubarb pills twice and half a dose of Chengqi Tang. Mengying urgently stopped them, saying, "Fear of deficiency and tonifying is wrong, but haste and reckless purging is also erroneous. Damp steaming becomes heat, burning fluids into phlegm. Disease did not happen overnight. Treatment should focus on dispersing from upper and lower, unlike heat transmitting to Fu organs which can be cleared in one sweep." The next day, lower body swelled. Using previous method plus Scutellaria combined with Xie Xin Tang (Drain Heart Decoction) intention, paired with Xue Geng Tang (Snow Soup Decoction). (Previous formula removed Zhizi and Douchi, added Wine-fried Scorched Scutellaria (Jiu chao Kuning, 酒炒枯芩) one qian five fen, Light Cuttlefish bone (Dan Haizhe, 淡海 ) eight qian, Water chestnut with skin smashed (Lianpi Biqi, 连皮荸荠) one liang.) Phlegm was gradually expectorated, fullness subsided daily, but swelling progressed from abdomen to feet and genitals. Mengying said, given the situation, quick resolution is hard, but addressing the Three Jiao ensures safety. Previous medicine lacked strength, proposing Hejian's Guiling Ganlu Yin (Cinnamon, Poria, and Dew-Drop Drink) intention. Others advised sticking to previous medicine. Next day, significant blood in phlegm. Mengying said, "Damp-heat steaming continues, moving from Qi to Ying level." He prescribed Anemarrhena and Phellodendren Rehmannia Decoction plus Rehmannia root (Shengdi, 生地), Rhinoceros horn (Xijiao, 犀角), Turtle plastron (Biejia, 鳖甲), White Peony (Baishao, 白芍), Coix seed (Yiren, 薏仁), Fritillaria (Beimu, 贝母), Dendrobium (Shihu, 石斛), Imperata rhizome (Maogen, 茅根), Ophiopogon (Maidong, 麦冬), Talc (Huashi, 滑石), Gardenia (Zhizi, 栀子), Lotus root juice (Ou zhi, 藕汁), and Child's urine (Tongni, 童溺) to stop bleeding. (World fears consumption hematemesis; this case is damp-heat disturbing Ying, belonging to excess syndrome, blood loss is not worrying. Prescription: Wine-fried Anemarrhena (Jiu chao Zhimu, 酒炒知母) three qian, Wine-fried Phellodendron bark (Jiu chao Chuanhuangbai, 酒炒川黄柏) one qian five fen, Large Rehmannia root soaked in boiling water, strained (Da Shengdi, 大生地) eight qian, Rhinoceros horn shaved and ground (Bang Xijiao, 镑犀角) one qian, Blood Turtle plastron pounded and decocted first (Xue Biejia, 血鳖甲) eight qian, Wine-fried White Peony (Jiu chao Baishao, 酒炒白芍) one qian five fen, Raw Coix seed pounded (Sheng Yiren, 生苡仁) eight qian, Sichuan Fritillaria bulb pounded (Chuan Beimu, 川贝母) three qian, Deworm Reed Stem (Chai Shihu, 钗石斛) pounded and decocted first one liang, Fresh Imperata rhizome (Xian Maogen, 鲜茅根) five qian, Flower Ophiopogon (Hua Maidong, 花麦冬) four qian, Western Talc decocted first...)
