夫心与小肠相表里，故心热则小肠亦热，而小便必赤是方也。生地可以凉心血。甘草稍可以泻热毒，佐以木通则直去小肠膀胱矣。曰导赤散是泻丙丁火邪。从便而泄也。
29.内伤方法
内伤外感要分明，脉辨气口与人迎；外感风寒有余症，手背热兮鼻气频。
寒热齐作而无间，语言后重而先轻；恶风一切风都怕，恶寒向火似如冰。
内伤饮食为不足，手心壮热苦眩头；寒热间作口无味，语言懒佉短声音。
纵有些兮亦不怕，稍有温暖即无寒；须知不足中不足，补中益气妙如神。
补中益气赖参芪，白术甘草归陈皮；柴胡左旋升麻右，内伤外感两相宜。
中脾也，坤也，万物之母，气也。阳也。乾也。万物之父。过于困乏劳碌则百骸皆虚。必盗父母以得养。而中气大伤也，不有以补之，则形气不几于绝乎。故用白术 甘草之平补者，以补中用人参 黄芪之峻补者，以益气，土欲燥则当归随以润之。气故欲滞，则陈皮随以利之。而升麻 柴胡所以升乎甲胆乙肝之气也。盖甲乙者东方生物之始，甲乙之气升则土火木金水，必次第生也。
30.伤饮方法
湿热有毒如酒无，注入脾胃疾常有；积痛昏迷呕吐频，饮食少进脾家咎。
治法汗之利小便，上下分清其湿妙；葛花解醒汤不移，众人皆醉我独醒。
葛花解醒参干姜，茯苓泽泻术木香；青陈皮与砂神曲，豆蔻加来治酒伤。
夫葛花之寒，能解酒中之毒，茯苓泽泻之淡，能利酒中之湿。砂仁豆蔻木香生皮之辛。能行酒食之滞。干姜所以开胃止呕，神曲所以消磨炙腻。而人参白术之甘所以补益被伤之脾胃。
31.郁门方法
伏脉五郁已先知，木喜条达急吐之；火癝上炎须汗发，土惠停滞夺为奇。
金本清虚雍何塞，水性奔流折莫迟；血气湿热痰与食，又名六郁亦须知。
升者不升降不降，当变不变失其时；诸郁病根由此始，解郁汤头造化机。
解郁香附苍术芎，陈皮神曲白茯从；贝母翘栀苏梗枳，再劳国老在其中。
32.痰饮方法
痰之为病不胜言，稠则为痰稀饮详；理气治标虽捷法，扶脾治本更为良。
诸症来历如何认，各色临流浮与沉；二陈见景须加减，滚痰饮用细思量。
二陈甘草与陈皮，半夏茯苓白去皮；痰嗽阵中为上将，添兵减灶有回春。
夫痰者，人心之贼也。生于脾胃，不能自动，随气稍长，今之治痰，必先理气，此说固是正治标之论无加健去湿，治本之万全也；是方半夏辛温能燥湿，茯苓甘淡能渗湿，湿去痰无再生，陈皮温能利气，甘草辛平能益脾，二陈不惟治痰，能使大便润小便长。
33.咳嗽方法
肺为华盖号清虚，忽被邪魔入心居；有声无痰来作咳，无声有痰嗽难除。
有声有痰为咳嗽，令人五脏不聊驱；参苏四季随加减，丁香半夏作丸须。
参苏饮内枳前胡，半夏陈皮桔梗芦；甘草茯苓木香末，清金泻火病皆疏。
丁香半夏丸 人参 干姜 丁香 细辛 半夏 槟榔 脾胃温暖，则能运行，痰饮脾胃虚寒，则痰饮停于胸膈，肺气因之不利，乃作咳嗽。半夏之辛，所以燥脾，人参之甘所以养胃。干姜之温所以行痰。细辛之辛所以散饮。用槟榔取性重，可以随痰。经云所高者抑之是也。
34.喘急方法
上气喘急最难医，呼吸参差势可悲；肺实热邪雍促甚，肺虚寒思怯之微。
手足温兮生一线，四肢厥冷梦无疑；定喘汤头斟酌用，云泥虎鼠正几希。
定清汤苓苏子瓜，陈皮白果款冬花；甘草麻黄桑白杏，沉香半夏古来夸。
35.哮吼方法
哮吼原来肺窍痰，又缘外热内包寒；咽喉日夜无停响，声音古怪若弦弹。
心染浮滑汤丸愈，久治沉澁觅灵丹；治哮汤头兼发散，不必多方孟浪谈。
定哮汤用参阿胶，麻黄甘草桑白加；五味半夏罂粟壳，临期应变始为佳。
36.疟门方法
缘何疟疾许多名，不必多方枉费心；总是内伤与外感，内外失守疟来侵。
邪正不分寒热作，阴阳交闭厥成功；经云无痰不作疟，脾胃源头可肃清。
弦数热多须汗解，弦迟寒盛要温平；无汗当发散邪与，有汗当止正气寻。
柴苓通分利表里，清脾无碍妙如神；鬼哭灵丹虽截法，虚实新久要分明。
散邪汤内有麻黄，防风荆艾姜活全；紫苏芷芍芎甘草，浑来疟汗两倾盆。
柴苓汤即两汤名，分别阴阳问久新；和解疟疾通表里，无骄无謟乐升平。
清脾甘草果茯苓，白术青皮厚朴芩；半夏柴胡甘草伴，太阴疟鬼串山林。
热多寒少口苦咽干大小便赤涩，脉来弦数者此方主之。
夫清脾者，非清凉之谓。乃攻去其邪，中焦为一清也。故青皮厚朴去清脾中之痰。半夏茯苓清去脾中之湿，柴胡黄芩清去脾中之热。白术甘草清去脾中之虚。而草果仁又清膏盲之疾也。
七宝饮：常山 厚朴 陈皮 甘草 槟榔 草果 等分用酒煎露宿次早温服。此方脉来浮大弦滑者可用，若沉涩细微者勿用。
三解汤:柴胡 泽泻 麻黄去节 此治疟之剂时行长幼相似者，主之。此方但可泻实，虚者宜补其气血。
截疟验方：常山三钱 槟榔一钱 草果一粒 苍术一钱 半夏一钱 厚朴八分 青皮一钱 柴胡八分 乌梅五粒 红枣三粒 葱三条 灯心七条 生姜三片 水二碗煎一碗当日早时面朝东，足立齐齐服之即除。
37.痢门方法
请问如何能成痢，多因湿热气血滞；脏腑积聚生冷多，脾胃不和饮食致。
勿兮赤白与杂形，俱作湿热标本治；身凉脉细腹多痛，身热脉大口噤忌。
里急后重觅香连，木香导气须当备；四时发作有来因，不独秋间有如是。
加味香连丸：木香 黄连 厚朴 甘草 苍术。
木香导气芍槟榔，硝黄厚朴茯苓存；黄连归尾姜茶引，诸般痢症总相当。
38.泄泻方法
泄泻之名有许多，大都湿胜土多魔；阑门注下无清浊，风能湿胜脉微毛。
治法升提须利水，涩法急投无奈何；断流汤饮先分理，治泻丸丹亦极疴。
断流汤内桂防风，二术二苓泽泻兮；厚朴陈皮甘草芍，中流砥柱建肤功。
夫湿多成泄以致阳气下陷，此清浊倒置也。风能湿胜故用防风。燥能胜湿故用二术。淡能利湿故用二苓。土病水乘故用白芍。其桂朴之流又所以温经佐使也。
止泻丸：肉豆 粟壳 诃子 黄连 白附 食积泻腹痛甚而泻之后痛减肺细是也。用香砂平胃散去枳壳加白术茯苓。
五苓散：茯苓八分 白鼠八分 猪苓八分 泽泻八分 山药八分 陈皮八分 苍术八分 炒砂仁八分 肉蔻去油八分 诃子去壳八分 此方治湿泻水多。而腹不痛腹响雷鸣，脉细是也。
理中汤治寒泻症。人参 一钱 干姜一钱 甘草五分 藿香七分 良姜七分 生姜三片 白术一钱 桂五分 陈皮七分 茯苓七分 乌梅一粒 杏二枚。灯心一节。寒极手足冷，脉沉细加附子去良姜桂。腹痛加砂仁 厚朴 木香 去人参 呕哕恶心加灯心，半夏去良姜桂，泻不止加苍术 山药 。泻多不止加豆蔻诃子 附子 去良姜桂。虚汗加黄芪去藿香桂饱闷加厚朴 砂仁 去良姜人参 官桂。
39.霍乱方法
霍乱之症有何因，内外伤感病根深；忽然或吐而不泻，或吐不泻要认真。
心腹先吐腹先泻，心腹齐痛吐泻频；若兼转筋入腹死，湿霍乱兮虎狼名。
更有绞肠干霍乱，吐泻不得可忧惊；命在须臾无所祷，盐汤探吐多救人。
藿香正气三春用，五积隆冬注意寻；不伏水土形相似，本地砂泥最有灵。
藿香正气散方见风门。
五积陈皮甘草姜，芎苍芷芍茯苓详；半夏桔梗归厚朴，麻黄枳壳桂心良。
40.呕吐等门
声物俱出谓之呕，有物无声见吐医；有声无物名干呕，其声轻小短而微。
哕脘有声亦无物，声音重大浊长依；翻胃食已即来吐，也有朝食暮吐之。
噎膈思食难进口，恶心欲吐不吐奇；吞酸咽下酸津液，吐酸涌出水稀稀。
欸逆俗呼为打呃，哇声发作势跷蹊；嗳气即今之阿欠，稠痰湿热胃中迷。
蹧襟不外痰因火，痛不痛兮饥不饥；大都脾胃肺肝患，额汗口沫险无疑。
两便不闭无妨事，保中汤饮任驰驱；顺气和中加减用，切脉因时再制宜。
保中汤治饮食不下吐泻不止。
保中汤内藿香苓，黄连栀子半砂仁；茯苓陈皮炙甘草，龙胆加上效如神。
顺气汤中香附芩，陈皮甘草半砂仁；术枳连栀神曲簇，煨姜一片保安宁。
41.诸气方法
经云夫气体之充，与血周流配始终；内外感伤病疾作，公私抑郁利名穷。
有余是火燎原野，不及生寒方寸中；独参自古为良法，不是虚寒莫浪从。
独参汤 人参一两
气者万物之所资始也，天非此气不足以养万物，人非此气不足以生。故曰一息不运则机诚穷。一毫不续则宵怀判。是故病，以气为首务也。人参味甘性温，得天地中和之气，以成形，故用以补中和之气。使其一息尚存，则可以次第。而疗诸疾矣。烦躁加童便者。虚而有火也。身寒加附子者，回其孤阳也。虚实之辨，不可不察。独参但可以治虚耳。若实症危急当因时制宜也。
治气虚用四君字汤
人参 白术 茯苓 砂仁 陈皮 姜汁炒厚朴 当归 甘草各等分 生姜一片 枣二枚 虚甚加黄芪。
42.痞满方法
痞与否同不通泰，不是五积之痞块；输轮失常或误下，治湿法同消痞诀。
实痞便闭虚痞利，内虽满闷外无恙；木香化滞先尝试，黄连消痞后加临。
木香化滞当归稍，柴枳陈皮大腹包；白蔻红花姜半夏，中洲茅塞任君投。
黄连消痞丸：黄连土炒 黄芩七钱 枳实五钱 半夏 陈皮四钱 茯苓一两 猪苓三钱 泽泻 郁金 干姜。
养胃方法：香附 砂仁 木香 枳实 炒各七钱 白术 茯苓 半夏姜汁炒 陈皮各一钱 白蔻 藿香 厚朴姜汁炒 炙草二钱 生姜二片 枣一枚。
食后服：瘦人心下痞满，加黄连去半夏，血虚加当归 白芍 去半夏。痰膈加瓜篓仁 贝母 桔梗 竹沥 姜汁 少许白术 半夏 。脾泄加炒苍术 白芍 去半夏。
43.鼓胀方法
中满胀满即鼓胀，四肢不肿头无恙；脐凸筋青小水无，男从下起女从上。
按之不窟为逆症，不比水肿多般样；分消汤饮可回春。家传正气谁堪让。
分消汤
分消汤那内茯苓，苍朴白术枳砂仁；陈皮木香附泽泻，大腹皮兮可坦平。
此方气急加沉香，肿胀加菜头子。
家传正气汤
家传正气乌药奇，藿香附朴枳陈皮；半夏苍术大腹壳，理脾消胀任驱驰。
44.水肿方法
水肿之病出乎脾，世人不识乱猜疑；肾水脾土两要固，脾土一亏水无围。
泛滥并流四肢去，使人浮肿黄光辉；风肿气肿并血肿，阴水阳水亦须知。
风肿走注皮麻木，气肿随气稍长之；血肿之症如何识，皮间赤缕血痕见。
阳水自热阴水冷，阴水无如胃苓奇；所虑路歧平且坦，或加气喘命几希。
木香流气散
木香流气青陈皮，香附麦门大腹皮；木瓜通桂参苓术，半朴葛槟果藿沉。
实脾饮
实脾泽泻猪茯苓，苍朴白术枳砂仁；木香香附陈皮等，大腹皮加最有灵。
八正散
八正散肿车前子，瞿麦扁蓄山栀子；滑石木通炙大黄，阳水身热堪授与。
胃苓汤
胃苓汤即两汤名，甘草陈皮朴术平；加上白术和肉桂，猪苓泽泻猪茯苓。
45.积聚方法
积内阴血主有形，五脏传染不容情；心曰伏梁肝肥气，脾云痞气息蕡金。
肾乃奔豚各部踞，古来五积有声名；聚为阳气巳无形，六腑衍和触景成。
虽然业积无根蒂，东临肿痛又西临；复有痞块总名色，症瘕痃癖要分明。
症者微者不移动，腹中坚硬痛堪怜；瘕者假也能移动，忽聚忽散实难凭。
痃在剂隅和弹指，时常作痛吐难伸；癖居胁肋形髣髴，虽多荆棘暂时形。
总是阴阳不和致，痰饮食积死血真；七气汤中加减用，溃坚饮子再搜寻。
七气汤
七气汤中用益智，三菱莪术藿香寄；青陈皮桂桔梗加，香附甘草请先尝。
溃坚汤
溃坚汤内枳砂仁，半夏陈皮厚朴寻；香附当归木香术，山楂用尽积皆平。
左胁有块加川芎，右胁加青皮。肉食成块加姜汁炒黄连 。粉面食积加神曲。血块加桃仁 红花桂，去半夏山楂。痰块加枳实瓜篓海石，去山楂。胞胀加萝卜子槟榔，去术。壮健人加山楂。瘦人加人参少许。
46.五疸方法
疸症身黄不雅观，黄汗黄疸许多般；酒谷女劳虽有五，利水和中止两端。
多因汤饮小便闭，中焦温热吃紧关；茵陈汤可回天意，转瞬红颜奏喜欢。
茵陈汤
茵陈专治其五疸，黄芩连术草龙胆；猪苓泽泻栀青皮，驱逐湿热真勇敢。
47.补益方法
劳伤虚损补为君，万法千方治不同；说破五行生克理，实虚血气要知兮。
气虚不外四君子，血虚四物所当宗；六味地黄和小火，天王补心各一通。
48.虚劳方法
虚劳俗语即怯劳，多因酒色太煎熬；手足心热痰作嗽，遗精汗雨咯血多。
浑身且发蒸蒸热，真阴消烁落皮毛；劳瘵有虫能变化，熏陶传染袭沉疴。
名色难分五六七，细微软弱脉难逃；治怯滋阴须降火，肉脱龙丹怎奈何。
滋阴降火柏知全，两地川芎归麦门；甘草陈皮白芍术，一杯浑下息燎原。
49.失血方法
The Heart and Small Intestine are exteriorly-interiorly related; therefore, if Heart Heat is excessive, the Small Intestine will also be hot, resulting in dark red urine (Xiaobian Chi). In this formula, Shengdi Huang (Rehmanniae Radix) cools Heart Blood (Xinxue); Gancao Shao (Glycyrrhizae Radix et Rhizoma with tip) clears heat toxins; assisted by Mutong (Akebiae Caulis), it directly drains damp-heat from the Small Intestine and Bladder. This formula is called Daochi San (Guide Out Red Powder), which means draining the Fire Evil of Bing (Heart/Fire) and Ding (Small Intestine/Water), allowing the heat to exit via urination.
29. Methods for Internal Injury
Internal injury and external contraction must be clearly distinguished; differentiate via pulse at Qikou (Cunkou, governing internal conditions) and Yingchang (Neck artery, governing external conditions). External contraction by Wind-Cold presents with excess symptoms: heat in the back of the hands and frequent nasal breathing.
Chills and fever occur simultaneously without interval; speech starts heavy and becomes light; fear of wind implies all wind is feared; fear of cold feels like ice even near fire.
Internal injury from diet constitutes deficiency: strong heat in palms, bitter dizziness; chills and fever alternate, tastelessness in mouth, lazy speech with short voice.
Even if there is slight aversion to cold, it is not frightening; slight warmth eliminates the cold. Know that this is extreme deficiency within deficiency; Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) works miraculously.
Buzhong Yiqi Tang relies on Renshen (Ginseng Radix) and Huangqi (Astragali Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), Danggui (Angelicae Sinensis Radix), and Chenpi (Citri Reticulatae Pericarpium). Chaihu (Bupleuri Radix) ascends left, Shengma (Cimicifugae Rhizoma) ascends right; suitable for both internal injury and external contraction.
Spleen is Earth, Kun Trigram, mother of all things, represents Qi, Yang, Qian Trigram, father of all things. Overexertion and fatigue cause emptiness in all bones and joints, inevitably depleting parents' essence for nourishment, severely injuring Middle Qi. Without supplementation, form and Qi would nearly perish. Thus, use Baizhu and Gancao for mild tonification; use Renshen and Huangqi for vigorous tonification to augment Qi. Since Earth desires dryness, Danggui moistens accordingly. Since Qi tends to stagnate, Chenpi promotes flow accordingly. Shengma and Chaihu elevate the Qi of Jia Gallbladder and Yi Liver. Jia and Yi represent the beginning of biological growth in the East; when Jia-Yi Qi ascends, Earth, Fire, Wood, Metal, and Water generate sequentially.
30. Methods for Alcohol Injury
Damp-heat and toxicity, like wine, inject into Spleen and Stomach causing frequent diseases; accumulation pain, coma, frequent vomiting, reduced intake blame the Spleen family.
Treatment methods: induce sweating and promote urination, clarify upper and lower to remove dampness excellently; Ge Hua Jie Xing Tang (Kadsura Flower Awakening Decoction) remains unchanged; while others are drunk, I alone am awake.
Ge Hua Jie Xing Tang contains Renshen, Ganjiang (Zingiberis Rhizoma), Fuling (Poria), Zexie (Alismatis Rhizoma), Baizhu, Muxiang (Aucklandiae Radix), Qingpi (Tangerine Peel Green), Chenpi, Sharen (Amomi Fructus), Shenqu (Medicated Leaven), Doukou (Cardamomi Fructus) to treat alcohol injury.
Ge Hua's cold nature resolves alcohol toxicity; Fuling and Zexie's bland nature promotes alcohol dampness; Sharen, Doukou, Muxiang, and Chenpi's acrid nature moves food-alcohol stagnation; Ganjiang opens stomach and stops vomiting; Shenqu digests roasted/greasy foods; Renshen and Baizhu's sweet nature tonifies injured Spleen and Stomach.
31. Methods for Depression/Stagnation
Hidden pulse indicates Five Depressions known in advance; Wood likes free flow, urgent cases require emesis; Fire depression flares up, requires diaphoresis; Earth depression stagnates, purgation is miraculous.
Metal is originally clear and empty, why blocked? Water flows rapidly, cannot delay its restriction; Blood-Qi, Damp-Heat, Phlegm, and Food, also known as Six Depressions, must be known.
When ascending should not ascend, descending should not descend, change fails to occur losing timing; roots of various depressions start here, Jieyu Tang holds creation's mechanism.
Jieyu Tang uses Xiangfu (Cyperi Rhizoma), Cangzhu (Atractylodis Lanceae Rhizoma), Xiong (Chuanxiong Rhizoma), Chenpi, Shenqu, Baifu Ling (Poria with white part); Beimu (Fritillariae Bulbus), Qiao (Lonicerae Flos), Zhi (Gardeniae Fructus), Sugeng (Perillae Caulis), Zhishi (Aurantii Fructus Immaturus), plus Laoguo (Licorice).
32. Methods for Phlegm-Fluids
Phlegm disease is indescribable; thick is Phlegm, thin is Fluid; regulating Qi treats branch quickly, but supporting Spleen treats root better.
How to recognize origins of various symptoms? Observe floating or sinking pulses; Erchen Tang needs modification upon seeing scenes; Gutan Wan (Rolling Phlegm Pill) requires careful consideration.
Erchen Tang contains Gancao, Chenpi, Banxia (Pinelliae Rhizoma), Fuling (white skin removed); for paroxysmal phlegm cough it is the general; adding/reducing troops brings spring recovery.
Phlegm is a thief in the human heart. Born in Spleen-Stomach, unable to move independently, grows slightly with Qi. Current phlegm treatment必先理气 (first regulate Qi); this theory is correct for treating the branch, but adding strengthening and removing dampness makes root treatment perfect. In this formula, Banxia's acrid-warmness dries dampness; Fuling's sweet-blandness渗湿 (seeps out dampness); dampness gone, phlegm cannot regenerate; Chenpi's warmness benefits Qi; Gancao's acrid-p neutrality benefits Spleen. Erchen Tang not only treats phlegm but moistens large intestine and lengthens small intestine urination.
33. Methods for Cough
Lung is Canopy, called Clear and Empty, suddenly invaded by evil spirits; sound without phlegm is Cough (Ke), no sound with phlegm is Sough (Sou) hard to remove.
Sound and phlegm together is Cough-Sough, making five organs unable to drive away; Rensu Yin (Ginseng and Perilla Decoction) modified seasonally, Dingxiang Banxia Wan (Clove and Pinellia Pill) necessary.
Rensu Yin contains Zhike (Aurantii Fructus), Qianhu (Peucedani Radix), Banxia, Chenpi, Jiegeng (Platycodonis Radix), Lugen (Imperatae Rhizoma); Gancao, Fuling, Muxiang powder, clears Metal and drains Fire, diseases all disperse.
Dingxiang Banxia Wan contains Renshen, Ganjiang, Dingxiang (Caryophylli Flos), Xixin (Asari Radix et Rhizoma), Banxia, Binglang (Arecae Semen). Warm Spleen-Stomach enables movement; Cold-Deficiency Spleen-Stomach causes Phlegm-Fluids to stop in chest-diaphragm, Lung Qi thus unfavorable, producing cough. Banxia's acridity dries Spleen; Renshen's sweetness nourishes Stomach; Ganjiang's warmth moves Phlegm; Xixin's acridity scatters Fluids; Binglang's heavy nature follows phlegm down. Classic says "suppress what is high," this is so.
34. Methods for Panting/Emergency
Upper body panting emergency hardest to treat, breathing uneven pitiful; Lung Excess Heat Evil oppression severe, Lung Deficiency Cold thought timid subtle.
Hands-feet warm means one thread of life, four limbs cold dream no doubt; Dingchuan Tang (Stop Panting Decoction) used after deliberation, cloud-mud tiger-rat difference is slight.
Dingqing Tang contains Huangqin (Scutellariae Radix), Suzi (Perillae Fructus), Guazhou (Trichosanthis Fructus), Chenpi, Baiguo (Ginkgo Semen), Kuandonghua (Farfarae Flos); Gancao, Mahuang (Ephedrae Herba), Sangbaipi (Mori Cortex), Xingren (Armeniacae Semen Amarum), Chenxiang (Aquilariae Lignum Resinatum), Banxia, praised since ancient times.
35. Methods for Asthma/Roar
Asthma roar originally Lung orifice phlegm, also due to external heat enclosing internal cold; throat day-night no stopping sound, voice strange like string plucking.
Heart染 (infected) floating-slippery soup-pill cures; long-term treatment deep-slow seek spirit pill; Zhi Xiao Tang (Treat Asthma Decoction) combines dispersing, no need many sides rash talk.
Ding Xiao Tang uses Renshen, Ejiao (Donkey-hide Gelatin), Mahuang, Gancao, Sangbaipi; Wuweizi (Schisandrae Fructus), Banxia, Yingsuke (Papaveris Pericarpium), adapt at time best.
36. Methods for Malaria
Why malaria has so many names, no need many sides waste heart; always internal injury and external contraction, internal-external loss守 (guard) malaria invades.
