Zhang (29 years old): Menstruation occurs early with abnormal color. Pricking pain in the skin and interstices, located irregularly. Morning diarrhea with incomplete evacuation. Never given birth to children. Due to a lack of joy in mood, often feeling sorrowful and depressed. Qi stagnation leads to obstruction of Qi and Blood circulation throughout the body, causing intermittent blockage and bi-syndrome in the collaterals. Treatment follows the principle of using unblocking agents. (Qi and Blood stagnation due to sorrow and depression)

Yao (22 years old): Chronic cough with coldness in the back. Morning sweating. Cough worsens when lying on the right side. Menstrual cycle gradually delayed. Pulse is rapid and deficient. Appetite decreases, no desire to eat. This is emotional depression injuring Lung Qi, dragging on to form deficiency consumptive disease. It is not suitable for using cold-natured Lung-clearing herbs. (Late menstruation; Long-term cough due to emotional injury, Lung Qi deficiency)
Astragali Radix (Huangqi), Cinnamomi Ramulus (Guizhi), Paeoniae Radix Alba (Baishao), Glycyrrhizae Radix et Rhizoma Preparata (Zhigancao), Jujubae Fructus (Nanzao), Saccharum (Yitang).
The Lung is the pathway for Qi entering and exiting. Internal injury causes pathogenic factors from the five zang organs to rebel upwards into the Lung, resulting in cough. Here, chronic cough, back coldness, and morning sweating are entirely due to Lung Qi injury. The gradual delay of menstruation indicates not only that Qi and Blood do not circulate smoothly, but also blood dryness and Liver closure can be imagined. A rapid pulse indicates deficient fire. In deficiency patterns, cold-natured clearing herbs should not be used. Moreover, decreased appetite and food refusal indicate severe weakness of Middle Qi; how can bitter-cold herbs be used again to damage the Stomach? Prescribe Huangqi Jianzhong Tang. To injure the Lung is to benefit its Qi; are not Guizhi and Baishao excellent products for astringing Yin and harmonizing Blood?
Qin (21 years old): Qi rushing causing heart pain and vomiting of saliva. Sensation of Qi sinking. Diarrhea in the lower abdomen. Menstruation arrives late. The color is either pale or purple. The disease is in the Chong Mai (Penetrating Vessel). Treat from both Jueyin and Yangming meridians. (Liver invading Stomach)
Coptidis Rhizoma (Chuanlian), Foeniculi Fructus (Xiaohui), Toosendan Fructus (Chuanlianzi), Angelicae Sinensis Radix Tail (Guiwei), Pinelliae Tuber Preparated (Chao Banxia), Poria (Fuling), Cinnamomi Ramulus (Guizhi), Citri Reticulatae Pericarpium (Juhong).
Hua (23 years old): Depression injures Liver and Spleen. This is due to unsatisfactory mood, leading to disharmony of Qi and Blood. Use Xiaoyao San minus Baizhu, add Shanzha and Xiangfu. Do not let it stay in the Middle Jiao; the key is to promote the free flow of Qi and Blood. Now menstruation is delayed; stagnation and bi-syndrome should be unblocked. Also, Qi is weak and fails to control Blood. Furthermore, deep spring is a time when Qi leaks out; one must assist with Qi-tonifying herbs to be lawful. (Depression injuring Liver and Spleen) Guipi Tang.
Second visit: Previously, depression injuring Liver and Spleen was well treated with Xiaoyao San and Guipi Tang. Now, due to anger, Qi rushes in the lower abdomen, passing through the Stomach up to the diaphragm, causing swelling and obstruction in the throat, cold limbs, and even coma. All these are due to Liver Wood rebelling; severely, it leads to Jue syndrome. The Liver houses Ministerial Fire internally; the disease comes rapidly, all signs of extreme movement. It is Liver Function overacting. One should restrain its function. Previous solid tonification methods cannot be used. Anwei Lizhong Wan minus Huangbai and Xixin.
Qian (Age not specified): Pulse is choppy. Epigastric fullness and decreased appetite. Menstruation is late. Abdominal pain with a bearing-down sensation.
Bupleuri Radix (Chaihu), Paeoniae Radix Alba Preparata (Chao Baishao), Scutellariae Radix (Huangqin), Curcumae Aromatae Rhizoma (Yujin), Cyperi Rhizoma (Xiangfu), Poria (Fuling), Perillae Caulis (Sugeng), Massae Fermentatae Medicinales (Shenqu).
Second visit: Various symptoms have not subsided; the abdomen hurts but does not bear down. Xiaoyao San minus Baizhu and Gancao, add Yujin, Xiangfu, and Shenqu.
Xu (18 years old): Amenorrhea with alternating chills and fever. Loose stools, abdominal pain. Jiawei Xiaoyao San minus Zhizi.
Certain patient: Blood deficiency with internal heat. Menstruation does not arrive. Jiawei Xiaoyao San minus Baizhu.
Certain patient: Delayed menstruation with abdominal pain. Wind-rash, blood in collaterals is unsettled. Long-term stagnation transforms into heat. The method should be to unblock and drain. (Heat in blood collaterals causing abdominal pain) Liangge San minus Mangxiao, add Mudanpi and Chi Shao.
Sun (29 years old): Lower part of the extraordinary vessels is damaged. Delayed menstruation with abdominal pain. First use Danggui Jianzhong Tang. Then discuss treatment methods for the eight extraordinary vessels. (Deficiency-cold stagnation in extraordinary vessels) Danggui Jianzhong Tang.
Second visit: Chronic cough, chills and fever upon exertion. Danggui Jianzhong Tang minus Jiang.
Xie (30 years old): Can eat but cannot transport. Mass-associated diarrhea. Menstrual period irregular. Dryness and pain in the lower abdomen. Numbness in the lower jiao. Qi rushing to the heart, vomiting and rebellion. Abdominal borborygmus, burning sensation in the heart. Both the eight extraordinary vessels and the qi Jing vessels are diseased.
Ren Shen (Ginseng Radix et Rhizoma), Poria (Fuling), Artemisiae Argyi Folium (Aiye), Cyperi Rhizoma Preparated (Zhi Xiangfu), Cistanches Herba (Dan Congrong), Osteumi Euphorbiae (Dan Guzhi), Cinnamomi Cortex (Rougui), Angelicae Sinensis Radix (Danggui), Cornu Cervi Degelatinatum (Lujiaoshuang), Foeniculi Fructus (Xiaohui), Magnesium Silicatum (Zishiying), Yimucao Extract Pills (Yimu Gao Wan).
Wang (31 years old): Pulse slow on the right, choppy on the left. Menstrual blood pale and late. Vomiting phlegm, water, and food. Married for over three years without conception. This is long-term stagnation condensing phlegm and obstructing Qi. It is essential to promote free flow. Formulate prescription from Yangming and Jueyin meridians.
Pinelliae Tuber (Banxia), Citri Reticulatae Pericarpium (Guangpi), Poria (Fuling), Magnoliae Officinalis Cortex (Nongpo), Atractylodis Rhizoma (Maozhu), Evodiae Fructus (Dan Wuzhu), Cyperi Rhizoma (Xiao Xiangfu), Crataegi Fructus (Shanzha Rou), Ginger Juice Pills (Jiangzhi Fa Wan).
Second visit: In mid-March, after using acrid-warm formula to scatter depression, phlegm and stasis were discharged naturally, chest became comfortable, vomiting eased. Now prefers warm food, dislikes cold. Menstruation delayed by over fifty days. When it arrives, it will likely be pale and scant. Propose warming and nourishing Chong and Ren vessels, cultivating vital Qi. Bazhen Tang minus Baizhu, Gancao, and Dihuang, add Xiaohui, Rougui, Qiai, Xiangfu, Zishiying, and Placenta Hominis (Hechejiao) pills.
Zhu (26 years old): Menstruation occurs twice a month, or does not come for several months. No conception for five years. Lower jiao limbs are often cold. This is damage to Chong and Ren vessels, lacking storage. Treat with warming and benefiting Kidney and Liver. (Deficiency-cold of Liver and Kidney)
Ren Shen (Ginseng Radix et Rhizoma), Placenta Hominis (Hechejiao), Rehmanniae Radix Preparata (Shudi - processed with Amomi Fructus Shaoren), Angelicae Sinensis Radix Root (Guishen), Paeoniae Radix Alba (Baishao), Chuanxiong Rhizoma (Chuanxiong), Cyperi Rhizoma (Xiangfu), Poria Cum Pinellia (Fushen), Cinnamomi Cortex (Rougui), Artemisiae Argyi Folium Carbonized (Aitan), Foeniculi Fructus (Xiaohui), Magnesium Silicatum (Zishiying), Yimucao Extract Pills (Yimu Gao Wan).
Cheng (37 years old): Infertility for thirteen years, various phantom diseases occurred. Patient describes late menstruation. Three days before onset, whole body muscles, bones, and collaterals feel pulling, sore, and painful, unable to stretch. In women, menstrual blood from all collaterals gathers in the Sea of Blood to descend. The Sea of Blood is the Chong Mai. Men store essence, women connect to the uterus. Infertility, irregular menstruation, and Chong Mai disease. Abdomen is Yin; Yin deficiency generates heat. Limbs and back are Yang; Yang deficiency generates cold. Ultimately, it is all deficiency damage from postpartum non-recovery. Confused by "treating disease as seen," suffering lifelong infertility and lingering illness. Liver Blood and Yin are deficient, Wood-Fire resides internally. Ancients, when warming the lower jiao, must assist with cooling Liver and firming Yin herbs. Do not stick to the idea that late menstruation is Qi stagnation, blindly throwing strong Qi-breaking drugs to rob Yin. (Chong Mai Liver Yin deficiency)
Placenta Hominis (Hechejiao), Rehmanniae Radix (Shengdi), Lycii Fructus (Gouqi), Astragali Complanati Semen (Shayuan), Eucommiae Cortex (Sheng Duzhong), Anemarrhenae Rhizoma (Baiwei), Crataegi Fructus (Shanzha), Phellodendri Cortex (Huangbai), White Flower Motherwort Herb (Baihua Yimucao).
Zhu (Age not specified): The Classic says: "When Yang Wei is diseased, one suffers chills and fever." Because there was little snow last year, winter lost its storing function. Mid-spring was damp, earth Qi rose and leaked. With a constitution of long-term Liver-Kidney Blood deficiency, Spring generation power is shallow. The eight extraordinary vessels belong to Liver and Kidney, they are the framework of the body. The eight vessels lack binding and securing functions; Yin weak leads to internal heat, Yang micro leads to external cold. Spine often hurts. Menstruation irregular. Sea of Blood gradually dried up. Dragging on becomes deficiency consumptive disease. The situation is revealed. Ju Fang Xiaoyao San, although a holy medicine for gynecology, focuses mainly on Liver and Spleen meridians. Due to depression causing damage, Wood and Earth mutually injure, Qi and Blood blocked. In harmonizing Qi and Blood, assist with Chaihu slightly ascending to guide Shaoyang vital Qi. The depression in Upper and Middle Jiao can be smoothed. Now, disease worsens at dusk, calm at dawn, obviously indicating injury to Liver-Kidney Supreme Yin, eight vessels not constraining. Thus fever without sweat. Supreme Yin is profound; ancients said Yin diseases do not have sweat. Should follow Zhongjing's example of sweet medicine, do not take acrid-aromatic Yang-assisting herbs. (Liver-Kidney Extraordinary Vessels Yin deficiency)
Glycyrrhizae Radix et Rhizoma Preparata (Zhigancao), Ecliptae Herba (Ejiao - Note: Text says Ejiao, usually Donkey-hide gelatin, here likely refers to Ejiao), Rehmanniae Radix (Xi Shengdi), Paeoniae Radix Alba (Sheng Baishao), Ophiopogonis Tuber (Maidong), Ostreae Concha (Muli).
Certain patient: Yin deficiency with internal heat. Menstruation irregular. (Yin deficiency)
Male Black Bone Hen (Xiong Wuguji), Rehmanniae Radix (Xiao Shengdi), Ecliptae Herba (Ejiao), Paeoniae Radix Alba (Baishao), Lycii Fructus (Gouqi), Asparagi Tuber (Tiantong), Poria (Fuling), Leonuri Fructus (Chongweizi), Ligustri Lucidi Fructus (Nvzhenzi), Longan Aril (Guiyuan).
Ten ingredients above, decoct with Qinghao juice, child's urine, and pure wine into paste, add honey and make pills.
Zhang (43 years old): Alternating chills and fever every other day. Abdominal pain during menstruation. (Dysmenorrhea; Qi and Blood stagnation due to depression)
Rehmanniae Radix (Xiao Shengdi), Moutan Cortex (Danpi), Anemarrhenae Rhizoma (Zhimu), Polygonati Rhizoma (Huafen), Trionycis Carapax et Squama (Sheng Biejia), Lycopii Herba (Zelan).
Certain patient (20 years old): Abdominal pain first, then menstruation arrives. Mostly Qi stagnation. Morning diarrhea with abdominal borborygmus, also a Spleen-Stomach disease. Different from lower jiao mass-diarrhea.
Chuanxiong Rhizoma (Chuanxiong), Angelicae Sinensis Radix (Danggui), Cyperi Rhizoma (Xiangfu), Prepared Costus Root (Wei Guangmuxiang), Crataegi Fructus (Zharou), Poria (Fuling).
Certain patient (26 years old): Chills, fever, no sweat. Abdominal pain before menstruation. Dry and itchy throat with rebellious cough. No desire for food. This is depression injuring Qi and Blood, disease of the eight extraordinary vessels. Temporarily give Zelán Decoction.
Angelicae Sinensis Radix Root (Guishen), Lycopii Herba (Zelan), Salviae Miltiorrhizae Radix (Danshen), Paeoniae Radix Alba (Baishao), Biota Seed (Baiziren), Poria Cum Pinellia (Fushen).
Zhou (17 years old): Virgin with irregular menstruation, varying intervals. Abdominal pain always precedes menstruation, worse than usual. Greatly reduced diet. Started in early summer. After autumn, lower jiao often cold. Abdominal borborygmus. Alternating diarrhea and constipation. Examining pulse and complexion, it is due to living environment easily causing depression and anger, Liver Qi deviating horizontally.
