Book: Yao Zhenbai's Medical Cases Chapter: Main Text (3/20) Category: Medical Discussion
[Warm Diseases]
[Spring Warmth]
Patient Chen XX, female, 25 years old, from Kunming City. Spring of 1939.
First Visit: Fever for several days, pulse is deficient and rapid, tongue body is deep red and curled/shrunken making it difficult to extend, tongue coating is black and cracked like a tortoise shell. Facial complexion is dirty, teeth are scorched, consciousness is confused with delirious speech, hands and feet are convulsing. Spontaneous sweating profusely without stopping, incontinence of urine and feces. This is a deteriorated stage of Spring Warmth disease. Due to prolonged fever, the Yin of the Liver and Kidney is depleted, fluids are dried up leading to internal wind stirring, and the condition is critical. I tentatively prescribe the following formula to urgently rescue the Yin, hoping for a slim chance of recovery.
Prescription: Raw Guiban (Tortoise Shell) 18g, Sheng Dadi (Rehmannia Root) 15g, Hei Xuanshen (Scrophularia Root) 9g, Da Cundong (Ophiopogon Tuber) 9g, Sheng Hangshao (White Peony Root) 9g, Cu Biejia (Softshell Turtle Shell) 15g, Ye Huanglian (Wild Coptis Rhizome) 3g, Sheng Muli (Oyster Shell) 12g, Sheng Gancao (Licorice Root) 3g, Dong Ejiao (Donkey-hide Gelatin) 15g (melted and mixed), Jizi Huang (Chicken Egg Yolk) 1 piece (mixed in portions).
Add three spoons of Tongbian (Child's Urine) to each dose. Due to the patient's confusion and difficulty in swallowing, instructed to administer via frequent dripping feeding method.
Second Visit: After taking the medicine, consciousness slightly recovered, convulsions gradually stopped, spontaneous sweating decreased. Able to eat a small amount of thin porridge, slight cough with phlegm stagnation. Pulse has turned deficient, wiry, slippery, and rapid; tongue body is deep red but can be extended, black cracked coating has reduced but remains dry with little fluid. The wind pathogen is gradually calming, but Yin fluids have not recovered, and latent heat is not cleared. Treatment continues to nourish Yin, generate fluids, and clear heat.
Prescription: Dong Ejiao 24g (melted), Zhi Guiban (Prepared Tortoise Shell) 21g, Cu Biejia 15g, Hei Xuanshen 9g, Ye Huanglian 3g, Da Cundong 9g, Sheng Hangshao 9g, Guang Jieluo (Citrus Reticulata Peel Veins) 9g, Sheng Gancao 3g, Dancai (Edible Mussel) 5 pieces, Jizi Huang 1 piece (mixed in portions).
Third Visit: After two doses of the above prescription, consciousness gradually cleared, but speech was slurred. Fever subsided, but tidal fever persisted in the afternoon. Cough with spontaneous sweating, mental fatigue, bowel and bladder control restored, though urine remained yellow-red. Appetite acceptable, pulse slippery and rapid, tongue red with black coating, cracking lessened, slight moisture present. Continue with previous method, instructed to take four doses. Prescription:
Sheng Dadi 21g, Cu Biejia 15g, Sheng Hangshao 9g, Digupi (Cortex Lycii) 15g, Chao Zhimu (Prepared Anemarrhena Rhizome) 4.5g, Guang Jieluo 9g, Fushenmu (Poria with Coconut Wood) 15g, Ye Huanglian 3g, Xian Lugen (Fresh Reed Rhizome) 30g, Sheng Gancao 3g, Zhuru (Bamboo Shavings) 6g, Jizi Huang 1 piece (mixed in portions).
Fourth Visit: After two doses, consciousness and speech were clear, tidal fever, spontaneous sweating, and cough all reduced. Convulsions completely stopped. Desire for food, able to sit up, tongue body red and moist, black cracked coating gone, pulse still slippery and rapid. Still use original formula with modifications.
Prescription: Sheng Dadi 15g, Hei Xuanshen 9g, Da Cundong 9g, Digupi 12g, Xian Lugen 21g, Sheng Gancao 3g, Jing Zhuru 6g, Jizi Huang 1 piece (mixed in portions).
Fifth Visit: After taking the above, tidal fever receded, spontaneous sweating stopped, cough ceased, appetite good, bowel and bladder normal. Tongue red and moist, pulse slippery and slightly rapid. Condition has turned from critical to safe. Continue with the following formula for aftercare. Prescription:
Sheng Dadi 12g, Kong Shashen (Glehnia Root) 12g, Guang Yuzhu (Polygonatum Rhizome) 9g, Tianmendong (Asparagus Tuber) 9g, Jinshihu (Dendrobium Stem) 9g, Da Cundong 9g, Sheng Hangshao 9g, Fushenmu 15g, Jing Paye (Perilla Leaf) 3 slices, Guang Jieluo 9g, Sheng Gancao 3g, Zhuru 6g.
[Commentary] This case of Spring Warmth involved heat pathogens lingering in the lower Jiao blood level for a long time, damaging true Yin, drying up fluids, stirring wind, and depriving the mind of nourishment, leading to a critical state. If one recklessly uses wind-chasing and dispersing herbs, there is a risk of "separation of Yin and Yang"; conversely, clearing heat and purging fire alone is ineffective. Following the *Neijing* principles of "moisten what is dry" and "astringe what is scattered," I decisively applied *Da Ding Feng Zhu* (Great Wind-Calming Decoction) from *Wen Bing Tiao Bian* to nourish Yin and subdue Yang. Wild Huanglian from Yunnan clears heart heat and resolves toxicity; Tongbian assists the "Three Shells" (Guiban, Biejia, Muli) in guiding fire back to its source. Through successive adjustments, full recovery was achieved.
[Big Head Plague (Warm Toxin)]
Patient Yang XX, male, 25 years old, clerk, from Kunming City. Spring of 1938.
First Visit: Patient initially had headache, fever, no aversion to cold, restlessness at night. Subsequently high fever, severe headache, thirst wanting to drink, red face, swollen eyes, nasal obstruction, dry throat, worse in the afternoon. Constipation, scanty dark urine. Self-medicated with Sufeng Wan and Yinqiao San with no effect. Days later, appearance changed drastically: face and head swollen and red, eyes narrowed, nose small, rapid breathing, continuous high fever and drinking. Pulse: surging, large, wiry, rapid. Tongue: deep red tip, yellow dry coating. This is Wind-Heat Warm Toxin attacking the clear orifices, diagnosed as "Big Head Plague." Urgent treatment requires dispelling wind, clearing heat, purging fire, and resolving toxin. Prescription:
Bohe (Mint) 6g, Hangju (Chrysanthemum) 6g, Sangye (Mulberry Leaf) 9g, Yinhua (Honeysuckle Flower) 9g, Lianqiao (Forsythia Fruit) 9g, Huangqin (Scutellaria Root) 6g, Huanglian (Coptis Rhizome) 4.5g, Gancao (Licorice Root) 3g, Sheng Dahuang (Rhubarb Root) 9g, Sheng Shigao (Gypsum Fibrosum) 18g, Renzhonghuang (Human Excrement Filter) 6g, Xian Jingzhuye (Fresh Bamboo Leaf) 50 pieces.
Second Visit: Breathing calmer, fever/thirst/restlessness slightly reduced, stool passed, urine still dark. Face/head swelling persists, cough with phlegm. Disease trend slightly receded, but warm toxin remains heavy. Continue original formula with modifications.
Prescription: Bohe 6g, Sangye 9g, Yinhua 9g, Lianqiao 9g, Huangqin 6g, Huanglian 4.5g, Mabo (Fly Agaric Mushroom) 9g, Jiangcan (Silkworm) 9g, Gancao 3g, Sheng Shigao 18g, Zhe Beimu (Fritillaria Cirrhosa Bulb) 9g (suspended), Xian Jingzhuye 50 pieces.
Third Visit: After two doses, head/eye swelling gradually subsided, facial wrinkles appeared. Restless heat calmed, drinking reduced. Cough with phlegm, bowel/bladder regulated. Pulse wiry-rapid, tongue red with yellow-greasy coating. Wind-heat warm toxin gradually dissipating. Prescription:
Sheng Shigao 18g, Huashi (Talcum) 9g, Hanshui Shi (Calcitum) 9g, Yinhua 9g, Lianqiao 9g, Bohe 6g, Lugen (Reed Rhizome) 30g, Mabo 6g, Chantui (Cicada Slough) 6g, Gancao 3g, Xian Jingzhuye 50 pieces.
Fourth Visit: After three consecutive doses, facial edema subsided, dry wrinkles appeared. Bowel/bladder unobstructed, cough resolved, thirst stopped. Pulse fine-wiry-slippery, tongue red, coating thinner. Warm toxin nearly cleared, Yin fluids not yet restored. Prescription:
Bai Xuanshen (White Scrophularia) 9g, Da Cundong 9g, Chuan Beimu (Sichuan Fritillaria) 9g (suspended), Sheng Shigao 15g, Mabo 6g, Chantui 3g, Lianqiao 6g, Gualou (Trichosanthes Fruit) 9g, Gancao 3g.
Fifth Visit: After three doses, facial swelling completely gone, wrinkles itchy, peeling/desquamation. Diet and bowel/bladder normal, cough reduced. Pulse still fine-wiry, tongue red-moist. Disease significantly reduced. Prescribe for clearing, moistening, and regulating. Full recovery followed. Prescription:
Xi Shengdi (Fine Rehmannia) 12g, Jing Xuanshen 9g, Da Cundong 9g, Bai Fuling (White Poria) 12g, Hang Baishao (White Peony) 6g each, Fen Danpi (Moutan Bark) 6g, Dong Sangye (Winter Mulberry Leaf) 9g, Gualou Qiao (Trichosanthes Rind) 9g, Sheng Gancao 3g, Lv Xiaodou (Green Mung Bean) 9g (suspended), Sigualuo (Luffa Fiber) 9g.
[Commentary] Heavy latent heat combined with seasonal pathogens caused dysfunction of ascending/descending and transformation, leading to warm toxin stagnation and blocking of clear orifices. Treatment involved light wind-dispelling, opening upper Jiao, detoxifying/purging fire, regulating middle Jiao, sweet-cold generating fluids, and clearing/moistening lower Jiao. Followed by nourishing Yin, clearing collaterals, and sweeping toxins.
[Summer Wind (Japanese Encephalitis Type B)]
Patient Li XX, male, 25 years old, worker. Consulted at Infectious Disease Hospital in July 1958.

[Commentary] Yangming heat excess scorches Qi and Yin, seemingly requiring urgent purgation to preserve Yin. However, due to summer-heat sinking and extreme heat generating wind, premature purgation would cause complications. Thus, first used standard method to clear heart, resolve summer-heat, awaken brain, extinguish wind. Later, guided by circumstances, performed gentle purgation so pathogens had no foothold. During this critical struggle between pathogen and healthy Qi, the sequence of treatment must be extremely cautious.
[Damp-Warmth]
Patient Yang XX, male, 36 years old, cadre. Consulted at a hospital in August 1958.
Illness began with headache, fever, spontaneous sweating, epigastric stuffiness, nausea, poor appetite, thirst without desire to drink, loose stools multiple times daily, scanty yellow urine, abdominal borborygmus and bloating. [Yangming Warm Disease]
Patient Peng XX, male, 32 years old, from Kunming City. August 1946.
First Visit: Initial headache, fever, body heaviness, vexatious thirst wanting to drink, abdominal distension/chest tightness. Took acrid-warm exterior-releasing agents, sweat occurred but fever did not subside, mouth/throat dry, thirst worsened, constipation, dark urine. Western medicine and enemas ineffective. Used Liangge San and Da/Xiao Chengqi Tang to purge fu-organ excess, but stool still blocked, heat intensified, craving cold drinks, vexatious delirium. Family anxious, invited me. Illness duration over ten days, abdominal fullness/chest tightness, rapid breathing. Six pulses surging-large-rapid, Guan position hard-solid, tongue coating thick-yellow-dry, center black-dry, base deep-red tip red. Diagnosis: Yangming Warm Disease. Mistaken sweating depleted fluids, repeated purgatives failed to clear fu-excess because of chronic Liver/Kidney deficiency and internal Yin consumption. Prolonged fever dried fluids, intestines dry. Must use Yin-nourishing, Qi-firming, Blood-moistening, "adding water to move the boat" approach. Prescription:
Bai Renshen (White Ginseng) 9g (decocted separately and mixed), Hai Shen (Sea Cucumber) 2 strips (simmered first), Jing Yuanshen 15g, Xi Shengdi 30g, Maidong (Ophiopogon Tuber) 15g, Danggui (Angelica Sinensis) 18g, Sheng Dahuang 9g, Yuanmingfen (Mirabilite) 9g, Sheng Gancao 3g, Shengjiang Zhi (Ginger Juice) 2 teaspoons per dose.
Second Visit: Next morning after taking medicine, borborygmus, flatus, then passed over ten pieces of black lacquer-like dry stool, chest/abdominal fullness instantly reduced. Breathing peaceful, consciousness recovered. Body heat, slight sweat, still dizzy/fatigued, no appetite. Pulse regular/harmonious/slow, tongue coating thin-yellow with fluid. Post-purgation, Ying-Wei harmonizing, Qi/Yin recovering. Change to Yiwei Tang with modifications:
Fei Shashen (Sand Ginger) 15g, Da Cundong 9g, Da Shengdi 24g, Jinshihu 9g, Guang Yuzhu 15g, Hang Shaoyao (Peony) 9g, Fushen 15g, Sheng Gancao 3g, Bingtang (Rock Sugar) 9g.
Third Visit: After three doses, all symptoms retreated, movement free, diet improving. Pulse harmonious, tongue moist coating thin. Continue above formula for regulation, cured after five doses.
[Yao's Comment] The patient was a bathhouse worker. Based on clinical symptoms and pulse/tongue, he met criteria for Yangming Fu-Excess requiring purgation. However, illness lasted ten days, repeated purgatives (Liangge San, Da/Xiao Chengqi Tang, enemas) failed. Reasons: 1. Chronic Liver/Kidney deficiency, insufficient Middle Qi. 2. Prolonged fever scorched Yin; although Liangge/Chengqi used, Huangqin/Huanglian bitter-cold damaged healthy Qi, Zhishi/Houpo consumed Qi/depleted fluids, hence purgation failed or was incomplete. 3. Long illness, constitution weak, healthy Qi unable to transport medicine. Critical condition, if not urgently purging to preserve Yin, difficult to save. I followed Warm Disease methods: Renshen tonifies Qi, Danggui nourishes Blood, Haishen/Shengdi large doses nourish Yin, Xiaofen/Dahuang purge, mixed with Ginger Juice to prevent cloying, reaching Middle/Lower Jiao directly, achieving purgation goal, full recovery.
[Winter Warmth]
Patient Zhao XX, male, 12 years old, student, living in Kunming. Winter 1932.
First Visit: Child habitually likes spicy/sweet foods, heavy latent heat. Illness for over twenty days, initial fever/thirst, no aversion to cold, vexatious panting. Maternal grandfather knowledgeable in medicine gave Yinzha San, Puji San, fever didn't subside, cough not resolved, chest distension/constipation, scanty dark urine, vexatious dry throat. Then gave Shuangjie San, Zhichi Tang, stool still blocked, fever increased, abdominal distension, rapid breathing, anorexia, thirst worsened. I visited, winter season, child body hot as fire, breath full/scorched lips, dirty face/dry teeth, confusion/delirium. Pulse rapid, Guan wiry-hard-solid, tongue red, coating dry-yellow little fluid. Severe Winter Warmth. Liver/Lung accumulated heat, Heart fire blazing, plus Yangming Fu accumulation unblocked, Triple Burner congested.
Method: Clear and purge, sweep Fu-excess. Expect battle-sweat after medicine, otherwise nasal bleeding or maculopapular rash may occur. Tentatively prescribe for expert review.
Prescription:
Sheng Shigao 12g, Hang Juhua (Chrysanthemum) 6g, Dong Sangye 9g, Sheng Dahuang 9g, Xi Zhishi (Immature Bitter Orange) 6g (stir-fried suspended), ZiTongcao (likely Bai Tongcao) 3g, Jiao Shanzha (Hawthorn) 9g, Fei Huashi 9g, Hanshui Shi 9g, Chu Bohe (Mint) 4.5g, Xian Jingzhuye 50 pieces as guide.
