The *Neijing* (Classic of Internal Medicine) states: The symptoms of Lung Abscess (Fei Yong, 肺痈) include panting and distension in both hypochondriac regions. This is because the Lung governs Qi and is associated with respiration. Its collateral branches emerge horizontally from the Lung system to the axilla. Therefore, when pathogenic factors accumulate in the Lung, they cause upward panting and urgency, and downward distension in both hypochondriac regions. Treatment should aim to drain the pathogenic toxins from the Lung organ.
Lung Abscess presents with coughing and expectoration of pus and blood. A rapid and excess pulse indicates Lung Abscess; a deficient pulse indicates Lung Flaccidity Syndrome (Fei Wei, 肺痿). If there is coughing but natural saliva in the mouth, and a slippery tongue coating, this is Floating Cold (Fu Han, 浮寒), not Lung Flaccidity Syndrome. If the mouth feels dry, coughing causes hidden pain in the chest, and the pulse is extremely slippery and rapid, this is Lung Abscess. Question: When a patient coughs and has rebellious Qi, how does the physician know it is Lung Abscess through pulse diagnosis? There should be pus and blood; if vomited, death ensues. Later, indeed, pus was vomited and the patient died. What does the pulse resemble? How to differentiate it? The physician says: The Cun (Inch) position pulse is faint and rapid (according to *San Yin Fang*, the Cun position pulse is rapid and excess, slippery upon pressure). Faint indicates Wind; rapid indicates Heat. Faint leads to sweating; rapid leads to aversion to cold. Wind invades the Ying (Defensive) level, so exhalation cannot enter deeply; Heat penetrates the Xue (Nutritive) level, so inhalation cannot exit. Wind damages the skin and hair; Heat damages the blood vessels. Wind lodges in the Lung, causing dry mouth, panting, and distension, dry throat without desire to drink, copious turbid saliva, and intermittent chills. Where the Heat passes, blood coagulates and stagnates, accumulating to form an abscess and pus. The expectorated material resembles rice porridge. In the early stage, it can be saved; once pus is formed, it is difficult to treat. The Cun position pulse is rapid, and the Zu Yang (Foot-Yangming) position pulse is tight. Cold and Heat struggle against each other, causing chills, trembling, and coughing. The Zu Yang position pulse is floating and slow, indicating normal Stomach Qi. This belongs to Lung Abscess. The physician says: Chills and fever, with a slippery and rapid Cun position pulse, while diet and daily activities remain normal, indicate an abscess or swelling disease. Physicians who do not recognize this may treat it as Cold Damage (Shang Han, 伤寒), which will not lead to recovery. How to know there is pus? Where is the pus located? How to distinguish its location? The physician says: If the pus is in the chest, it is Lung Abscess. The pulse is rapid, and coughing produces pus and blood. Assuming the pus has not yet formed, the pulse is naturally tight and rapid; if only rapid, the pus has already formed.
Generally, for Lung Abscess, a short and choppy pulse is favorable (Shun, 顺); a floating and large pulse indicates a poor prognosis (Si, 死). The complexion should be white; if excessively red, it also indicates a poor prognosis (Si, 死).
Hidden pain at the Zhongfu (LU-1) acupoint indicates Lung Carbuncle (Fei Ju, 肺疽). Slight elevation of the flesh above indicates Lung Ulcer (Fei Chuang, 肺疮). Zhongfu is an acupoint located 1.6 cun below Yunmen (LU-2), where the arterial pulse can be felt in the depression between the breast and ribs. This serves as a diagnostic indicator. In the early stage, it can be saved; once pus forms, most cases are fatal. To determine if pus exists, simply examine the pulse. If the pulse is faintly tight and rapid, no pus has formed; if the pulse is very tight and rapid, pus has formed.
Both Lung Abscess and Lung Flaccidity Syndrome are difficult to treat. Lung Abscess originates from the Upper Jiao, with guest heat lingering in the Lung organs, causing coughing and internal pain in the chest and diaphragm, leading to lung tissue necrosis and abscess formation. The expectorated pus resembles spoiled paste and is foul-smelling; this is difficult to treat. The pulse is tight before the Guan (Bar) position and faint/small after it, indicating deficiency below and excess above. Ancient practitioners used Huangqi (Astragalus Root) as the principal herb and Ganjie Tang (Licorice and Platycodon Decoction) as an assistant. However, few patients recovered, likely due to lung perforation.
*Rumen Shiqin* records: In Wushui, a wealthy family had two sons, the elder thirteen and the younger eleven. Both enjoyed eating one to two jin of purple cherries daily, consuming them for half a month each year. After one or two years, the younger son developed Lung Abscess, and the elder son developed Lung Flaccidity Syndrome; they died successively. Dairen often said: Is human death determined by fate or heaven?
