Xie Puxiang had a constitution that was inherently Yin-deficient. Suddenly, he suffered from pain in the Huantiao acupoint (GB30). Initially, the pain extended down to the left leg, then shifted to the right leg, and even caused spasms (muscle cramps) in both feet, rushing upward to the abdomen. Occasionally, the pain started from the breast and flowed downward to the thigh. He cried out day and night, with cold limbs and spontaneous sweating, finding it difficult to turn over. Seeing his complexion lacked luster, physicians indiscriminately administered tonifying agents. Mengying palpated the pulse: wiry and slightly slippery. The patient feared heat and felt hunger, had scanty urine and hard stools, a red tongue but no thirst. This indicated Yin deficiency with phlegm-qi stagnation in the Jueyin channel. The prescription consisted of Cistanche Herba (Rongshu), Vicia sativa Seed (Shushi), Bamboo Shavings (Zhuru), Luffa Fruit Network (Sigualuo), Citrus Aurantium Unripe Fruit (Juhe), Fennel Fruit (Huixiang) stir-fried with Danggui, Evodia Fruit (Wuyu) stir-fried with Scutellaria Root (Huanglian), Sichuan Pepper (Chuanjiao) stir-fried with Mume Fructus (Wumei), Corydalis Rhizome (Yanhusuo) stir-fried with Toosendan Fruit (Lianshi), Jellyfish (Hai Zhe), and Water Chestnut (Biqi). One dose brought immediate relief; after several doses, the condition stabilized. Subsequently, Hujian Wan (Tiger Hidden Pill) plus Gentianae Macrophyllae Radix (Qinjiao) was given, leading to recovery. (The pathology was Yang deficiency within Yin, complicated by phlegm-heat. Light Cistanche Herba 1.5 qian. Liangtoujian 3 qian. Sigualuo 3 qian. Ginger-processed Zhuru 3 qian. Jihe powder added 1 qian. Danggui processed with Huixiang decoction 2 qian. Chen Jianzhou’s son contracted Spring-Warmth disease. From the onset, there was mental agitation and confusion, fear, insomnia, and very rapid pulses. Mengying stated that the Warm pathogen had directly entered the Ying (Nutritive) level. Shenxi Dan (Divine Calamity Elixir) was prescribed, assisted by Zixue San (Purple Snow Powder). After two doses, the patient recovered. In summer, Wu Shouzhan’s son and Gao Ruozhou’s daughter, Hu Qiuren, contracted Warmth diseases. From the onset, they exhibited limb twitching, delirious speech, and confused mental state. Mengying used this same method for all, and they recovered rapidly. People often insist on using diaphoresis (sweating) as the treatment for initial exterior contraction, not realizing that diseases have no fixed form, and the value of medicine lies in its appropriateness. (Rhino Horn Xi Jiao, ground and taken with water 2 qian. Kuqin (Scutellaria Root) wine-fried 3 qian. Shichangpu added later 2 qian. Large Rehmannia Root (Dishengdi) 1 liang, soaked, water discarded. Bao Jizhong suddenly felt cold in late spring, with wheezing, sweating, and impending syncope. Mengying observed the pulse: deep, wiry, rough-slippery, and slightly rapid. This indicated a chronic Phlegm-Fluid retention, likely caused by the mistaken use of warming tonics. It was reported that last winter, taking Hu’s Shengji Tang (Kidney Qi Decoction) provided comfort, and he had not expectorated phlegm for a long time. Mengying said: The disease is in the Lung; only when Lung Qi spreads can Phlegm move. Although it is a chronic condition, it is vastly different from Shaoyin Water Flooding. The initial comfort was because Aconite (Fuzi) and Cinnamon Twig (Guizhi) in the formula are fierce and advance relentlessly, forcing the Phlegm to retreat. Combined with heavy, turbid descending herbs like Rehmannia, which protected the rebellious nature of the Phlegm, the violent Qi eventually revealed itself. The soft, greasy texture obstructed the pivot mechanism. The governance of Qi failed, leading to scanty urination and defecation, leaving the Phlegm with no exit. The more it hid, the more it accumulated until sudden eruption blocked the pathway of Clear Yang ascending and Descending. Thus, the danger was extreme. One must distinguish this sharply from Shaoyin Deficiency Wheezing. Do not fear deficiency and wrongly tonify. Prescribed Xiebai (Allium Macrostemon Bulb), Gualou (Trichosanthes Fruit), Zhishi (Immature Bitter Orange), Xingren (Apricot Seed), Xuanfuhua (Inula Flower), Zheshi (Hematite), Juhong (Citrus Peel), Banxia (Pinellia Tuber), Ziwan (Aster Root), Zhuru (Bamboo Shavings), Lugeng (Reed Rhizome), Ge Fen (Clam Shell Powder), and Xue Geng Soup. The condition stabilized. Subsequently, Lung Qi was regulated to clear residual Phlegm, and full recovery took a month. (Fresh Xiebai pounded 1.5 qian. Jiang Gualou Pi 3 qian. Fried Zhishi 1.5 qian. Wang Zoushi’s younger brother contracted Spring-Warmth. Initially, he had delirium and mania. After continuously taking large doses of clearing and resolving formulas, he became comatose and silent, with ice-cold limbs, closed eyes, urinary incontinence, and refusal to drink. Physicians were helpless. Mengying palpated the pulse: wiry, large, slow, and slippery. The tongue coating was yellow and greasy throughout, with foul odor emitting directly. Chengqi Tang (Purgative Decoction) plus Yinhua, Shihu, Huangqin, Zhuru, Xuanshen, and Shichangpu was administered. Many black, tarry stools were passed, and mental clarity slightly improved. Slight fluid intake was allowed. The next day, Mangxiao (Mirabilite) and Dahuang (Rhubarb) were removed. Hai Zhe, Lufu (Radish), Huanglian, and Shigao (Gypsum) were added. After two doses, chills resolved, limbs warmed, coating receded, and porridge was tolerated. Recovery occurred without further effort. Mengying subsequently treated patients surnamed Ye and Li, who recovered after one or two purgations. Only Mrs. Zheng (wife of Wu) required over ten purgations to recover. (This case follows the treatment principles of *Zhengshu Lun*. Differentiation: despite large doses of clearing/resolving herbs, the tongue remained yellow/greasy with foul odor, confirming Excess Heat binding in the Stomach Fu. Yuanmingfen (Mirabilite) 1 qian. Chu Xiaoshu suffered from delayed menstruation with alternating chills and fever. Physicians diagnosed it as consumption (deficiency) and indiscriminately gave warming tonics. This led to abdominal distension, anorexia, leukorrhea, strangury, constipation, and painful, difficult urination. Mengying palpated the pulse: wiry, forceful, and rapid. This indicated Heat hiding in the Jueyin channel, and mistaken treatment causing Lung congestion. A formula to clear and descend, opening the upper jiao, was given, using Danggui Longhui Wan (Codium Dragon Gallbladder Pill). After two doses, chills/fever ceased, appetite returned, and all symptoms resolved within ten days. (Differentiation of Heat hiding in Jueyin: wiry, forceful, rapid pulse; painful, difficult urination. Differentiation of mistaken treatment causing Lung congestion: abdominal distension, anorexia. Fresh Lugeng 2 liang. Sheng Dongguazi 4 qian. Jiang Kuqin 3 qian. Fried Zhishi 1.5 qian. Jiang Piyeshua Bao 3 qian. Jin Yinhua 1 liang 5 qian. Fresh Shihu decocted first 1 liang. Mrs. Wen contracted intermittent malaria in early summer while eight months pregnant. After several attacks, high fever and coma set in, with lower back pain threatening miscarriage. Mengying palpated the pulse: surging, slippery, and rapid. Tongue coating was yellow, greasy, and turbid. Huangqin, Zhimu, Zhuru, Zhuye, Yinhua, Sangye, Sigualuo, Shihu, Shigao, and Shichangpu were prescribed. One dose resulted in cure. (Slippery pulse indicates Phlegm-Heat. Surging-slippy indicates Excess Heat complicated by Yin deficiency. Wine-fried Kuqin 1.5 qian. Wine-fried Zhimu 3 qian. Jiang Zhuru 3 qian. Fresh Zhuye 2 qian. Jin Yinhua 1 liang 5 qian. Winter Sangye 4 qian. Fresh Shihu decocted first 1 liang. Shigao decocted first 8 qian. Shichangpu added later 2 qian.)
Zhu Jiamu’s father suffered from intermittent malaria, aged over seventy. Mengying palpated the pulse: wiry and slippery. Epigastric stuffiness and yellow coating were present. He said: Do not worry. A formula to clear heat and resolve phlegm was given. After several doses, he recovered rapidly. Mrs. Li (wife of Li Mou), over sixty years old, chronically suffered from Phlegm-Cough. Recently, she developed afternoon fever, anorexia, headache, and inability to eat. Previous treatments failed. Mengying saw a slippery, rapid pulse, indicating internal Phlegm-Fire and external invasion of Warmth. Two doses of Gypsum-based formula reduced fever and restored appetite. Several more doses cured both the acute and chronic conditions. (Zhu’s Father Formula: Jiang Chuanhuangbai 1.5 qian. Lianniu pounded, decocted first 3 qian. Fried Zhishi 1.5 qian. Song Shi’s wife had recurrent colds. Just as she was recovering, sudden abdominal distension rose to the epigastrium, accompanied by wheezing, diarrhea, urinary retention, inability to swallow fluids, spontaneous sweating, and orthopnea. Mengying prescribed Sangzhi, Shigao, Xuanfuhua, Zheshi, Xingren, Houpo, Huangqin, Banxia, Huanglian, and Tongcao. The illness vanished after one cup. (Wine-fried Sangzhi 3 qian. After labor injury, Wu Mou developed fever and jaundice. Mengying observed a rough, rapid pulse, diagnosing severe Damp-Warmth. Jaundice appeared immediately. Urgent clearing and resolving measures were taken. Stools gradually became loose, urine very red, indicating Damp-Heat moving downward, and fever subsided. Seven to eight days after stopping medication, fever returned with delirium, deafness, convulsions, and incontinence. Mengying diagnosed Damp-Heat disturbing the Ying level. Xuanshen, Xi Jiao, Shichangpu, Lianqiao, Zhuru, Zhuye, Yinhua, and Shigao were used to release the Wei level and clear the Ying level, assisted by Niuhuang Wan (Calculus Bovis Pill) and Zixue Dan (Purple Snow Elixir). Recovery occurred despite buttock skin necrosis, managed with sheepskin gold paste. (Wu’s Damp-Warmth Clearing Formula: Wine-fried Xiyinchen 1.5 qian. Dongguapi 3 qian. Yun Fuling 3 qian. Zhu Shushu’s wife contracted a cold. Wu Mou gave two doses of exterior-releasing herbs, resulting in red rashes and gradual mental clouding. Ye Mou, knowing her chronic deafness and visual impairment indicated Yin deficiency, switched to Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction) for two doses, leading to incontinence and convulsions. Mengying examined her and said: Though thin and Yin-deficient, allowing pathogens to disturb Ying, she is not damp-heavy, so treatment is possible. However, epigastric tenderness, hiccups, and constipation indicate Phlegm-Heat blocking the Qi level. Mistaken use of ascending herbs causes Jie Xiong (Chest Binding). Dihuang is cloying and contraindicated. Many cases this year showed death from chest obstruction due to either mistaken ascending herbs or premature cloying tonics. He prescribed Xi Jiao, Xuanshen, Zhuru, Chuanbei, Xuanfuhua, Gualou, Piyeye, Ziwan, Baiqian, and Shichangpu, adjusted with Zixue. After two doses, hiccups stopped, mind cleared, but throat pain and thirst appeared. Zixue and Baiqian were removed. Shegan, Shandougen, Zhimu, and Huafen were added. Xilei San was insufflated into the throat. Throat healed in two days, chest relaxed, rash faded, fever subsided. Xi Jiao, Ziwan, Shegan, and Shandougen were removed. Yinhua, Zhizi, Zhuye, Hai Zhe, and Biqi were added. Sleep and appetite improved. Constipation persisted. Mengying said: No abdominal pain, so for a deficient body, moistening tonification is suitable. Cistanche, Hemp Seed, Danggui, and Dihuang were added. After continued use, bowels moved, and she recovered. (Phlegm-Heat blocking Qi level, no reason to take cloying Dihuang. Ground Xi Jiao 2 qian. Xuanshen Pian soaked, water discarded 8 qian. Li Dechang’s mother contracted a cold in mid-summer. Diagnosed as Damp, dryness-promoting herbs were given, causing diarrhea. Suspecting Qi sinking due to age, Earth-tonifying herbs were used, leading to Qi rebellion, coma, excessive sweating, and retracted tongue. Mengying palpated a surging, rapid, uncontrolled pulse, worse on the right Chi position. Large doses of Xi Jiao, Shigao, Huangqin, Huanglian, Huangbai, Zhimu, Huafen, Zhizi, Shihu, Zhuye, Lianxin, Xuanshen, and Dihuang were prescribed. Cold snow water was used to mix Zixue for continuous administration over a day and night. The tongue protruded, throat/tongue became red and ulcerated, swallowing water was painful. Three yellows (Huangqin, Huanglian, Huangbai) were removed. Yinhua, Shegan, and Shandougen were added. Xilei San was insufflated. Pulse/symptoms harmonized after three days, thin gruel tolerated. Sweet-cool mild agents were given. After ten days, hard black stools passed, and she recovered. (Surging/rapid/uncontrolled indicates Excess Heat with Yin deficiency. Yu Langzhai was thin and weak, suffering from intermittent malaria with less chills and more fever. Urination/defecation were obstructed. Epigastric oppression was extreme. Tongue coating was greasy, no thirst. Pulse was slow-slippery and overflowing upward. Mengying said: Constitution is Yin-deficient, but Phlegm-Damp obstruction exists. Ascending/releasing herbs should not be used to aid rebellion, nor cooling/moistening herbs to obstruct the pivot. Huashi, Houpo, Zhuru, Xuanfuhua, Tongcao, Piyeye, Weijing, Yujin, and Lane ye were prescribed. Coating receded. Houpo and Yujin were removed. Lianqiao, Zhishi, and Banxia were added. Chest oppression opened, malaria decreased, bowels moved freely. Huashi, Banxia, Lianqiao, and Zhishi were removed. Shashen, Shihu, Chenpi, and Huangqin were added. Full recovery in ten days. (Slow pulse indicates Dampness. Overflowing slipperiness indicates Phlegm-Heat failing to descend. West Huashi decocted first 4 qian. Prepared Houpo 1 qian. Jiang Zhuru 3 qian. Dong Zheqing’s wife had cough during pregnancy, unresolved after birth. Night sweats decreased, appetite dropped, headache and dry mouth developed. She lay listlessly, unable to sit up easily. Pulse was deficient-wiry, rough-rapid. Tongue was bare, red, and coating-less. Mengying said: Headache and dry mouth are due to Yang rising without fluid support. Do not treat as a cold. This is Crotch-Vessel rebellion causing cough, not wind-cold. Cistanche, Ziyingui, Guiban, Fuling, Dongchongxiacao, Muli, Doubitao, Gancao, Xiaomai, Hongzao, and Ou were prescribed. Cough decreased, appetite increased, headache ceased. Shudi was added, leading to full recovery. (Crotch-Vessel connects to Liver. Headache/dry mouth is Liver Yang rebelling. Pulse indicates deficiency of Yin-within-Yang. Cistanche, Ziyingui, Fuling, Hongzao treat the pulse. Light Cistanche 1.5 qian. Ziyingui pounded, decocted first 3 qian. Xue Guiban 2 liang. Qingyunpu’s son suddenly suffered from unilateral hernia (scrotal swelling). Doctor prescribed Huixiang, Lupaba, Wuyao, and Lihé. Pain became unbearable. Mengying palpated hot skin and pulse. Said: Not a hernia. Testicular swelling/pain on the right indicates Damp-Heat seasonal pathogen. Treating as hernia would cause abscess. Following correct treatment, relief came after one cup. Three doses passed stool, fever subsided. Eating mutton caused recurrence. Clearing/resolving herbs were given again. Dietary restrictions advised, leading to cure. (Damp-Heat injuring Qi causes right-sided testicular swelling/pain. Dongguapi 4 qian. Fresh Lugeng 1 liang. Sheng Yiren pounded 8 qian. Wine-fried Xiyinchen 1 qian. Chuanlianniu pounded, decocted first 2 qian. Qingguo pounded, decocted first 2 pieces. Yun Fuling 3 qian. Chen Mugua 1.5 qian. West Huashi decocted first 4 qian.)
Wu Xianzhang, over sixty, contracted a cold. Doctors knew it was Damp-Warmth but treatment failed. He could still get up and manage affairs. Mengying palpated: Left Cun rapid/frequent, others large. Appetite slightly reduced. Loose stools, scanty urine. Tongue coating greasy-yellow, tip uniquely black. Mengying refused to prescribe, stating: The syndrome is not severe. Observing pulse and tongue, chronic mental exertion has caused Fire to flare internally, unchecked by Water. Self-burning is inevitable, especially with Damp-Warmth. He died a few days later.
Huang Chunguang, 78, suffered Damp-Warmth for over ten days. Pulse was intermittent. Hiccups persisted daily. Mengying diagnosed: Intermittent pulse yet wiry/thready with root indicates strong congenital vitality. Despite age, it is an Excess pattern. Deep disease with voice loss is serious. Obstructed urination/defecation and grey-greasy coating suggest Fu Qi not venting, blocked by Phlegm-Damp-Heat. Clearing/venting may work. Xuanfuhua, Zhuru, Zhizi, Lianniu, Piyeye, Xingren, Wuyu, Huanglian, Ziwan, Gualou, Hai Zhe, and Biqi were prescribed. Tongcao 1 liang decocted to cook herbs. Immediate relief. Several doses produced massive sticky phlegm vomiting and bowel movements. Porridge tolerated. Movement-induced cough/wheezing revealed lower deficiency. American Ginseng, Guiban, Muli, Rongshu, Shihu, Niuxi, Dongchongxiacao, Shiying, Fuling, and Danggui were given. Symptoms improved sequentially. Shaoren and fried Shudi were added for final recovery. (Excess differentiation: intermittent pulse with root. Fu Qi obstruction: obstructed urination/defecation, grey-greasy coating. Xuanfuhua wrapped, decocted first 3 qian. Jiang Zhuru 3 qian. Qian Wenyuan occasionally had Phlegm-Cough in spring. Su Ge caused aphonia. Other doctors gave large tonics, worsening to choking on water. Mengying diagnosed: Left Cun active/rapid, Chi fine, Guan wiry. Right rough. Heart Yang overactive, secretly consuming Ying Yin. Lung Metal scorched, clearing/descending failed. Water source lost, roots unprotected. Left rising too much, Right descending powerless. Qi meridians distorted, Blood collaterals blocked. Choking on water confirms this. Metal fearing Fire is worrying. Eventually, blood stasis was vomited, eating slightly relieved, but he still died from hidden pathogen.
Tang Xicheng, over sixty, initial cold stage. Whole body red rash. Tongue fully coated yellow. Headache, lower back pain. Loose stools, painful urination. Pulse wiry-fine and rough. Diagnosis: Yin deficiency, fatigue, heavy Damp-Warmth toxin. Clearing/resolving must preserve Yin. Xi Jiao, Lingyangjiao, Huangqin, Zhuru, Yinhua, Lianqiao, Sangye, Weijing, and Tongcao were prescribed.
