Pulse is wiry and there is thirst; Shaoyang (Lesser Yang) cold-heat pathogens have invaded the Stomach, seizing body fluids. Xiaochaihu Tang (Minor Bupleurum Decoction) can be used to harmonize the righteous Qi and resolve the pathogen.
Xiaochaihu Tang: remove Banxia (Pinelliae Rhizoma), add Huafen (Trichosanthis Radix) and Baishao (Paeoniae Radix Alba).
The right pulse is soft and the left is wiry; the onset of cold-heat symptoms is gradually becoming earlier. There is thirst with a desire to drink. This indicates that Stomach Yin fluids are being damaged day by day, while Liver Wood and Heart Fire remain blazing. The treatment should generate fluids and nourish the Stomach to support the righteous Qi, using acrid and sour flavors to harmonize the stagnant heat of Liver Wood and Heart Fire.
Chaihu (Bupleuri Radix), Renshen (Ginseng Radix et Rhizoma), Maidong (Ophiopogonis Radix), Juhong (Citri Reticulatae Pericarpium Rubrum), Huangqin (Scutellariae Radix), Zhimu (Anemarrhenae Rhizoma), Baishao (Paeoniae Radix Alba), Wumei (Mume Fructus).
Another prescription: Sheng Biejia (Testudinis Carapax et Plastrum), Zhimu (Anemarrhenae Rhizoma), Wumei (Mume Fructus), Chao Taoren (Persicae Semen), Danpi (Moutan Cortex), Caoguo (Tsaoko Fructus), Baishao (Paeoniae Radix Alba).
Another prescription: Renshen (Ginseng Radix et Rhizoma), Zhimu (Anemarrhenae Rhizoma), Jinshihu (Dendrobii Caulis), Chuanlian (Coptidis Rhizoma), Wumei (Mume Fructus), Fuling (Poria).
Lack of sensation of hunger or fullness, loose stools three times. Cold-heat pathogens invade the Middle Jiao, causing disharmony between the Spleen and Stomach. Pure tonification is not suitable at this stage. It is proposed to adopt the principle of Six Beast Decoction (Sishou Yin).
Renshen (Ginseng Radix et Rhizoma), Caoguoren (Tsaokos Fructus), Zhigancao (Prepared Glycyrrhizae Radix et Rhizoma), Fuling (Poria), Guangpi (Tangerine Peel), Mugua (Chaenomelis Fructus).
In individuals with Yin deficiency constitution, cold and heat are unbalanced, leading to malaria chills and fever. Severe damage to Stomach Yin causes vomiting. After the Summer Solstice, summerheat pathogens are contracted. The treatment should generate fluids and harmonize Yang to calm the Stomach appetite.
Nen Zhuye (Young Bamboo Leaf), Jinshihu (Dendrobii Caulis), Guang Juhong (Citri Reticulatae Pericarpium Rubrum), Zhimu (Anemarrhenae Rhizoma), Zhi Banxia (Prepared Pinelliae Rhizoma), Mugua (Chaenomelis Fructus).
Fluid injury followed by malaria, presenting with chills, fever, irritability, and thirst.
Guizhi Baihu Tang (Cinnamon Twig and White Tiger Decoction), added with Huafen (Trichosanthis Radix).
Pulse is wiry and sharp as a blade, accompanied by irritability, thirst, epigastric fullness, vomiting, and upward movement of roundworms. This suggests that Stomach Qi is already compromised, summerheat pathogens have re-entered, and the Triple Burner's Qi transformation is obstructed, allowing hostile Qi to counterattack. Moreover, improper post-illness management has led to a condition of root deficiency and branch excess. For now, administer methods to calm the Stomach and direct rebellious Qi downward, hoping for relief from vomiting and slight smoothing of the airway, after which further discussion can take place.
Chuanlian (Coptidis Rhizoma), Wumei Rou (Mume Fructus Flesh), Zhishi Juice (Aurantii Fructus Immaturus Juice), Chuanjiao (Zanthoxyli Pericarpium), Sheng Baishao (Raw Paeoniae Radix Alba), Shengjiang (Fresh Ginger).
Summerheat pathogens have not subsided, and Stomach Qi is deficient. Roundworm rebellion causes middle jiao fullness, and vomiting of saliva indicates Jueyin (Terminal Yin) pathology invading the Stomach, with signs of impending Stomach Qi collapse. Administer methods to calm the Stomach.
After administering the method to calm the Stomach, vomiting and rebellion slightly subside. At night, sleep is restless with disturbed consciousness. Before dawn, there are chills and aversion to cold. This indicates recurring chills and fever, damaging both Yin and Yang, with a trend of dispersion and loss. It is proposed to use the Rescue Rebellion Method (Jiuni Fa) to stabilize and secure Yin and Yang, allowing them to return to their proper positions, thereby reducing the disease process.
Longgu (Fossilized Mammoth Teeth/Ostreae Concha equivalent in context often implies mineral sedatives like Dragon Bone), Guizhi Mu (Cinnamomi Ramulus Wood), Renshen (Ginseng Radix et Rhizoma), Muli (Concha Ostreae), Sheng Baishao (Raw Paeoniae Radix Alba), Shuqi (Ephedrae Herba variant/alternative name in specific contexts, often associated with resolving phlegm/nodes).
During the Yin and Mao hours (5 AM - 9 AM), both hands and arms are cold. This is due to the failure of Spleen and Stomach deficient Yang to rotate and transport. By mid-morning, warmth returns because Yang Qi is vigorous at noon. Although thin porridge is consumed, epigastric fullness persists. It is proposed to administer Zhizhong Tang (Regulate the Middle Decoction) to strengthen and transport the Middle Jiao, preventing Liver pathogens from damaging the Earth element. As it is said, malaria and dysentery diseases often arise from Spleen weakness.
Renshen (Ginseng Radix et Rhizoma), Banqu (Medicated Leaven), Wumei (Mume Fructus), Fuling (Poria), Yizhiren (Alpiniae Oxyphyllae Fructus), Guangpi (Tangerine Peel), Mugua (Chaenomelis Fructus), Zexie (Alismatis Rhizoma).
After malaria, Stomach Qi is weak, making it prone to rebellious Qi. Vomiting of saliva and lack of appetite indicate the need for a method to nourish the Stomach and control the Liver.
Renshen (Ginseng Radix et Rhizoma), Banxia Qu (Pinelliae Medicated Leaven), Guizhi Mu (Cinnamomi Ramulus Wood), Longgu (Fossilized Bones), Wumei (Mume Fructus), Chenpi Bai (Tangerine Peel White), Sheng Baishao (Raw Paeoniae Radix Alba), Muli (Concha Ostreae).
Pulse is wiry and slow, body feels cold, spirit is fatigued. Originating from worry, thought, fear, and fright, the defensive Yang Qi was suddenly fractured, allowing Yin-cold evil Qi to invade. Malaria is likely to occur. The disease mechanism requires quiet rest and protection of Yang, so that external pathogens do not penetrate deeply.
Danggui Jianzhong Tang (Angelica Build the Middle Decoction): remove Ginger, add Muli (Concha Ostreae).
Complexion, form, pulse, and symptoms all indicate deficiency. Chills and fever have condensed and consumed Stomach Yin. There is no sensation of hunger in the epigastrium, and urination and defecation feel unsatisfactory. Merely administering clearing-heat herbs to attack the Middle Jiao will only lead to a deadlock. Consulting ancient texts on summerheat disease: if unresolved after ten days, sour purgation should be used to correct Yang and revive Stomach juices. In patients with primordial deficiency, cloying tonics may alter the disease course.
Guizhi Mu (Cinnamomi Ramulus Wood), Sheng Muli (Raw Concha Ostreae), Chao Wumei (Stir-fried Mume Fructus), Sheng Baishao (Raw Paeoniae Radix Alba), Chao Shuqi (Stir-fried Shuqi), Dazao (Jujubae Fructus).
Another prescription: Remove Dazao (Jujubae Fructus), add Longgu (Fossilized Bones).
For constitutional deficiency with warm malaria, the treatment must harmonize righteous Qi and resolve pathogens. Dispersants like Chaihu (Bupleuri Radix) and Jingjie (Schizonepetae Herba) and heavy laxatives that injure righteousness are prohibited.
Guizhi Mu (Cinnamomi Ramulus Wood), Huangqin (Scutellariae Radix), Xingren (Armeniacae Semen Amarum), Huafen (Trichosanthis Radix), Sheng Baishao (Raw Paeoniae Radix Alba), Banxia Qu (Pinelliae Medicated Leaven), Juhong (Citri Reticulatae Pericarpium Rubrum), Doukou (Cardamomi Fructus Rotundus).
Chills and fever from the extremities force and seize Stomach Yin, resulting in anorexia and lack of hunger during convalescence, along with incessant sweating. The treatment should nourish Stomach Yin and generate fluids, waiting for recovery.
Renshen (Ginseng Radix et Rhizoma), Juanxin Zhuye (Heart of Bamboo Leaf), Sheng Shaoyao (Raw Paeoniae Radix), Fuling (Poria), Maimendong (Ophiopogonis Radix), Mahuang (Hemp Seed/Ma Ren).
Warm malaria with Yin injury presents with hot feet and hyperactive Yang, with early onset of attacks.
Liuyun Wan (Six Flavor Rehmannia Pill): remove Shanzhuyu (Corni Fructus) and Shanyao (Dioscoreae Rhizoma), add Renshen (Ginseng Radix et Rhizoma), Sheng Baishao (Raw Paeoniae Radix Alba), Sheng Biejia (Testudinis Carapax et Plastrum).
Heat pathogens entering the Lung constitute warm malaria.
Guizhi Baihu Tang (Cinnamon Twig and White Tiger Decoction).
Only heat without chills, cough gradually worsening with vomiting, and generalized body ache. This is termed warm malaria. The latent pathogen has been present for a long time, originating from the Lung meridian. Following Zhang Zhongjing's Guizhi Baihu Tang (Cinnamon Twig and White Tiger Decoction), two doses should suffice for resolution.
Guizhi Baihu Tang (Cinnamon Twig and White Tiger Decoction), added with Maidong (Ophiopogonis Radix).
Dan malaria (Febrile Malaria) with pathogens in the Lung presents with thirst and painful joints.
Guizhi Baihu Tang (Cinnamon Twig and White Tiger Decoction).
Lung malaria presents with cough, air reversal, and urge to vomit.
Lugen Zhi (Reed Rhizome Juice), Huafen (Trichosanthis Radix), Xingren (Armeniacae Semen Amarum), Banxia Qu (Pinelliae Medicated Leaven), Juhong (Citri Reticulatae Pericarpium Rubrum).
Chronic consumption and weakness for one year, recently with head distension, tidal fever, and thirst. This is summerheat penetrating deeply to become Dan malaria. The *Jin Gui Yao Lue* states: "When Yin Qi is injured first and Yang Qi acts alone, disease occurs." There is no need for dispersing or guiding out methods that further injure righteous Qi. Instead, use sweet-cold herbs to generate fluids and harmonize Yang, ensuring Ying and Wei are harmonized so that heat extinguishes itself.
Beishashen (Glehniae Radix), Zhimu (Anemarrhenae Rhizoma), Sheng Biejia (Testudinis Carapax et Plastrum), Maidong (Ophiopogonis Radix), Wumei (Mume Fructus), Sheng Baishao (Raw Paeoniae Radix Alba).
Summerheat stagnating for a long time becomes Dan malaria. Excessive heat damages Lung and Stomach Yin, causing five-center heat and frequent coughing. Therefore, light flavors and clear nourishment will naturally lead to recovery. The sweet-cold heat-clearing and fluid-generating formula is under discussion.
Mai Dong (Ophiopogonis Radix), Huafen (Trichosanthis Radix), Zhuye (Bamboo Leaf), Shashen (Glehniae Radix/Tetrapanacis Caulis), Tian Xingren (Sweet Apricot Kernel), Gancao (Licorice Root).
Summerwind entering the Lung constitutes Dan malaria. The *Jin Gui* says Yang Qi acts alone. Jiayan stated: "With a constitution inherently deficient in Yin, the latent summerheat, over time, mixes into the Blood. Yin deficiency leads to Yang floating, obscuring the clear orifices of the Upper Jiao. Stomach Yang loses harmony, leading to poor intake and fullness. Ultimately, the latent pathogen remains. All bitter-acrid methods to vent stagnation are contraindicated."
Since there is upper excess and lower deficiency with lingering hostile pathogens, heavy settling methods are inappropriate. Following Li Zhicai's example of "light substances removing excess," differentiate between Qi and Blood, using ventilation and expulsion methods.
Qing Da Zhuye (Large Green Bamboo Leaf), Lianqiao (Forsythiae Fructus), Xijiao (Rhino Horn/Rhinocerotis Cornu - substitute clinically), Xian Heye Zhi (Fresh Lotus Leaf Juice), Yuan Shen (Scrophulariae Radix), Tongcao (Tetrapaninis Medulla).
Pulse is rapid. In childhood, Yin Qi is injured first, and Yang Qi acts alone, causing tidal fever at night, easing at dawn. This results from latent summerheat internalizing and manifesting in autumn. The disease is named Dan Malaria. Complexion is pale, body is emaciated, prolonged fever leads to deficiency. Sweating, purging, and guiding out are contraindicated to avoid further injuring Yin and Yang. The tongue edges are red, center coating is greasy, accompanied by choking cough. Heat scorches the Upper Jiao, affecting the Lung. The method should nourish Yin to control Yang, assisted by clearing summerheat and肃ifying the Upper Jiao. Use Zhang Jingyue's Yunu Jian (Jade Woman Decoction).
Xian Shengdi (Fresh Rehmanniae Radix), Shigao (Gypsum Fibrosum), Sheng Gancao (Raw Glycyrrhizae Radix et Rhizoma), Maimendong (Ophiopogonis Radix), Zhimu (Anemarrhenae Rhizoma), Zhuxinye (Bamboo Leaf Heart).
Heat intensity has halved, but the pulse remains forceful and rapid. The pathogen latent since summer emerges from the interior. Sweating does not fully resolve it. In children with Yang excess and Yin deficiency, the disease worsens at night. Follow Liu Hejian's method of clearing the Triple Burner simultaneously.
Ganlu Yin (Sweet Dew Drink).
Pulse is rapid and large on the right. Thirst with drinking, mental confusion, hearing sounds as if in a jar. Tongue edges are red, coating has prickles. Latent summerheat likely combines with dampness to transform into malaria. Heat steaming forces fluid damage, and fluids do not recover, causing constant epigastric fullness. In heat diseases, preserving Yin is paramount. Having had malaria for half a month, interior symptoms must be considered. It is proposed to clear the Stomach and generate fluids, following Zhang Jingyue's Yunu Jian (Jade Woman Decoction).

Constitution is Yin deficient. Summerheat pathogens penetrate deeply. With heat accumulation and increasing thirst, sweating can resolve it. This corresponds to Zhang Zhongjing's statement: "Yin Qi is injured first, Yang Qi acts alone," named Dan Malaria. Misusing bitter-acrid draining methods seizes Stomach Yin, dulling Qi and causing loss of appetite.

Ying fluids are exhausted, pathogens penetrate the Chest Center (Shanzhong). Consequently, heart heat causes panic and distress. Zhu Danxi stated: "Ascending Qi originates from the Liver." Since prior cold limbs also indicate Liver pathology, use Zixue San (Purple Snow Powder) for its aromatic penetrating action to prevent interior pathogens from binding. Decoct the herbal soup as a drink, utilizing light and clear properties to avoid stagnation, resolving only the accumulated heat above the diaphragm. It is proposed to use Zhang Jingyue's Yunu Jian (Jade Woman Decoction).

Pulse is strong on the left, indicating pathogens remaining in the Blood. Chills and fever have reduced but persist. Nourish and clear interior heat, waiting for thorough clearance. Do not over-treat.

Zhang Zhongjing said: "Whenever Yuan Qi is injured and heat pathogens remain, sweet herbs should be given." The human body consists of nothing but Yin and Yang Qi. Knowing that Yang deficiency affects the Middle, and Yin deficiency requires Fumai (Restore the Pulse) method is certain. This formula entirely uses Fumai sweet herbs to protect the Yin fluids of the body. Liu Hejian added Three Stones, naming it Ganlu Yinzi (Sweet Dew Drink). These nourishing and clearing Yin herbs can save Yin fluids and expel deeply latent heat pathogens. The Upper Jiao is like mist; Huashi's sweet and bland nature drives it out. The Middle Jiao is like fermentation; Shigao's sweet, acrid, and cold nature clears it. The Lower Jiao is like a ditch; Hanshui Shi's sweet, salty, and cold nature drains it. Thus, eliminating pathogens does not damage true Yin, unlike Bupleurum and Turtle Shell combinations. Named Yinzi (Drink), it takes heavy medicines administered lightly, entering Yin without stagnation, attacking pathogens without damaging righteousness, differing significantly from decoctions and powders. Summer months are most suitable for this discussion, yet many people ignore it, hence this clarification.
Fumai Tang (Restore the Pulse Decoction) added with Three Stones (San Shi).
Spring bleeding indicates failure of Winter storage, undoubtedly a fundamental Yin deficiency disease. Since minor recovery, the One Yin of Summer Solstice failed to return, and blood disorders recurred, reverting to the original disease. Now, the right pulse is urgent and rapid, double that of the left. Face is oily and shiny, sweat is profuse, nasal discharge is turbid, tongue is dry with white coating, irritability and thirst with desire to drink. From noon to afternoon (Wu and Wei hours), steaming heat rises, head distends, whole body twinges and pains, panting and frequent coughing. Heat eases late at night, allowing slight sleep. If discussing consumptive disease, these manifestations should not exist. Consulting the *Jin Gui*: "Yin Qi is injured first, Yang Qi prevails alone, causing excessive heat and vexation," named Dan Malaria. Know that heterogeneous pathogens enter via mouth and nose, follow the Lung system through the Mu Yuan (Master Source), travel straight down the central pathway, distribute to Ying and Wei. When pathogens and righteous Qi meet cyclically, they contend, causing fever. Fluids are seized and consumed daily. Fire heat punishes Metal, making cough and panting severe. This is distinctly different from the underlying consumptive disease. Zhang Zhongjing's established rule is to first address the new exterior pathogen, fearing that tonification would aid the pathogen and harm righteousness. Therefore, relying on the right pulse diagnosis, it is proposed to use acrid-sweet agents to drive out summer-damp pathogens, ensuring heat reduction, then discuss adjusting the underlying disease. Do not fear deficiency and nurture pathogens, causing harm. This is crucial.

The previous method greatly cleared the Qi level and unblocked Ying and Wei. Shigao assisted by Guizhi primarily clears the Lung. The rest nourish and clear Stomach heat, still retaining the intent of generating fluids.
Diagnosing both hands as equal and small-rapid. Towards the end of the Wei hour (1 PM - 3 PM), heat intensity seems lighter than yesterday. The right pulse is not urgently pounding, but symptoms of heart heat, vexation, and thirst remain as before. Examining the tongue: dry white coating, edges excessively red. In a Yin-deficient constitution, heat pathogens invade the Triple Burner via deficiency. Regarding winter diseases caused by wind-cold, the Six Channels must be investigated; summer heat-warmth diseases must be traced through the Triple Burner. Liu Hejian created this in the *Xuan Ming Lun*, it is not my subjective opinion. Whenever heat clears briefly, it is proposed to administer one dose of Ganlu Yinzi (Sweet Dew Drink), and re-evaluate after five days of administration.

First, boil two cups of clear water until half remains, add herbs, boil forty to fifty times, clarify, and take cold.
Before illness, the appearance was already thin. After illness, night fever and morning coolness. Movement is still comfortable, unlikely to be a severe cold disease. However, after eight or nine days, upon minor recovery, eating rice dumplings and fish/flour foods follows the principle of "food-induced fever resolves by reducing food intake," a concept Zhang Zhongjing did not originally prescribe formulas for. This leads to abdominal pain and watery diarrhea. Food stagnation blocks the intestines and stomach, and the Fire Bowels fail to transform. Investigating the root, pediatric constitutions possess pure Yang. Emaciation before illness is also due to Yang hyperactivity consuming essence. Zhang Zhongjing stated: "Dan malaria presents with only heat, no chills." In this clause, "Yin Qi injured first, Yang Qi acting alone, heat scorching and vexing, causing muscle wasting," no formula was set, only stating "manage with diet." Jiayan advocated sweet-cold generation of fluids for cure, emphasizing the Post-Heaven Stomach Qi. Frequent watery diarrhea further injures fluids; multiple doses of bitter-cold herbs fail to stop heat. Nighttime delirium, knowing chest/diaphragm rejection pain and abdominal disharmony, all result from treating mild disease with heavy medicine, exhausting Yin and Yang Qi. The method should stop medication and provide grain, reasonably administering sweet-acid flavors to relieve vexation and thirst, allowing for careful consideration.
Malaria comes with vomiting, bleeding forms clots and is abundant. This is chronic labor injury accumulating stasis. Due to cold-heat attacking and moving Stomach collaterals, turbid stasis floods. After bleeding, cold limbs indicate Yangming deficiency. But malaria pathogens still come, with thirst and chest fullness. Although heat pathogens are not fully resolved, using bitter-cold Zhi (Immature Bitter Orange) and Po (Magnolia Bark) again to attack Stomach Qi is likely inappropriate.

Summer malaria onset often involves warm-heat. Purging moves the interior; interior injury leads to pathogen sinking, transforming into dysentery with severe pain. Pulse is large and rough on the right, tongue is white, nasal passages are dry and black, no hunger, no eating. Due to latent summer-damp, new coolness from outside causes malaria. Sweating resolves the exterior, but latent Qi fills the chest, residual heat knots in the Qi level. Seeking Lung transformation, like autumn descending weather, clear and肃ifying commands execute. Obese people have more phlegm and dampness. Summerheat received in summer, deep autumn coolness arrives, latent heat then erupts. Profuse sweating does not resolve it, unrelated to exterior cold. Irritability, thirst, and desire to drink are all pathological. Lung fails to descend causing chest fullness, dampness excess causes shivering.
