The son (referring to Wei Zhixiu's son) administered the medication with a decoction of jujube. Upon initial administration of five fen, there was no change in condition; another five fen was given, and shortly thereafter, severe abdominal pain ensued. A jujube decoction was then drunk to alleviate this, followed by five or six bowel movements. At that time, it was approximately late afternoon. By midnight, a large amount of accumulated fluid—several dou—was finally discharged, and the accumulation was resolved. During the evacuation, the patient experienced extreme adverse reactions (mingxuan), with cold hands and feet, and briefly lost consciousness before recovering. The entire family wept bitterly, blaming the harshness of the medicine. Alas, how can medicines be used lightly?
One person had overconsumed fruits and vegetables during summer, resulting in incessant diarrhea, severe pain in the epigastrium (Zhongwan point area), irritability, thirst, and a desire to drink. He self-administered Tianshui San (Sky Water Powder) and Xiangru Yin (Elsholtzia Decoction). Upon pulse diagnosis, the right Guan and Cun positions were both deep and hidden. Thus, it was diagnosed as cold-damp stagnation. Xiangsha Liujunzi Tang (Aucklandia and Amomum Six Gentlemen Decoction) was prescribed with processed ginger and thick bark magnolia stem. After one dose, the pain and thirst ceased, and recovery was achieved through regulation with Weiling Tang (Stomach and Lingzhi Decoction).
Gong Zicai treated Prefect Xu, who suffered from hematemesis and expectoration due to excessive consumption of distilled liquor and five-spice medicinal wine. His six pulses were rapid and urgent. This indicated that alcohol toxicity and accumulated heat had penetrated into the bone marrow, making tonification inadvisable. Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxicity) was modified with Anemarrhena Rhizome, Fritillaria Bulb, Gypsum, Forsythia Fruit, Scrophularia Root, Trichosanthes Root, Kudzu Root, Trichosanthes Fruit, Platycodon Root, and rhubarb steamed with wine, administered morning and evening. After more than a hundred days, Liuwei Wan (Six-Ingredient Pill with Rehmannia) combined with the detoxifying decoction was taken concurrently with the previous herbal soup. It took another hundred days for full recovery. However, after retiring home for over a year, he died still plagued by alcohol intoxication.
Lu Zuyu treated the mother of Wu Wuzu, who had been a widow since youth and maintained a strict vegetarian diet, possessing a very deficient constitution. While eating glutinous rice flour food, she was mistakenly informed that her son had failed the county examination, leading to shock and chest stuffiness, resulting in internal injury. Others administered excessive digestive-promoting herbs, causing severe middle-jiao distension. Pulse diagnosis revealed irregular, intermittent pulses in both hands, profuse watery diarrhea, an open mouth, and head sweat pouring like rain, indicating impending collapse of vital qi, yet the chest remained tender to palpation. With deficiency evident in the pulse but excess in symptoms, tonification should precede purgation. Fuzi Lizhong Tang (Aconite and Ginger Regulate the Middle Decoction) was urgently administered. After two doses, the pulse gained some root. Subsequently, Zhishi Lizhong Tang (Immature Bitter Orange and Ginger Regulate the Middle Decoction) was given, and the accumulation gradually moved, with the pulse showing spirit. Later, Runzi Wan (Moistening Pill) was administered at five fen per dose, delivered with the previous decoction. Once the accumulation was expelled and the body harmonized, regulation led to recovery. (The sequence of medication is worthy of emulation.)
The son of Xu Xiaoyuan, having recently married, had excessive phlegm and injured spleen qi, compounded by an external contraction. He presented with alternating chills and fever, stiffness and pain in the neck and back, and headache, exhibiting both exterior and interior signs. Some diagnosed malaria and hastily used antimalarial drugs, leading to thirst. He further consumed raw and cold foods, developing vomiting and diarrhea. Chai Ling Tang (Bupleurum and Poria Decoction) proved ineffective. Diagnosis revealed cold limbs, unconsciousness, blue-yellow complexion, and a faint, nearly absent pulse in the left three positions and right Chi position, while the right Guan position was slippery and forceful. Shenfu Lizhong Tang (Ginseng and Aconite Regulate the Middle Decoction) with Immature Bitter Orange, Thick Bark Magnolia Stem, and Hawthorn Seed was prescribed. After three doses, the pulse recovered. However, internal injury symptoms manifested as high fever, scorched tongue with prickles, tenderness around the umbilicus, and continuous sweating on the limbs, indicating complete lower-jiao excess syndrome. Only small amounts of Immature Bitter Orange, Thick Bark Magnolia Stem, and prepared rhubarb, guided by iron rust water, were used to expel three or four pieces of dry feces. As the condition persisted, Immature Bitter Orange, Thick Bark Magnolia Stem, Hawthorn Seed, and Coptis were added to Xiao Chaihu Tang (Minor Bupleurum Decoction) with a little Ginseng. Every four to five days, five fen of Runzi Wan was given, resulting in one bowel movement. After more than eighty days, the interior syndrome cleared, and the six pulses regained spirit. Previously, he had tidal fever in the late afternoon, closed stomach qi, dry mouth, and abdominal fullness. Removing Ginseng from the previous formula and administering four doses significantly reduced the condition. Some argued that if Shaoyang syndrome persists beyond twenty-one days, it becomes a complicated case. The Classics state: "Those who can eat prosper; those who cannot eat perish." Now, near a hundred days without any food intake, how could there be life? Moreover, with no fever, no tongue coating, and only slight late-afternoon heat, this must be due to yin deficiency. Urgent nourishment of yin and blood, along with opening the stomach and strengthening the spleen, would lead to natural recovery. One dose was given, followed by half a cup of rice gruel and a cup of longan juice. That night, however, he again had high fever, heart pain, extreme irritability, and developed tongue coating. Re-diagnosis showed deep, solid pulses in the right Guan and Chi positions. Immature Bitter Orange, Coptis, Polyporus, Malt, Hawthorn Seed, and Thick Bark Magnolia Stem were used to deliver fifteen fen of Runzi Wan. One smooth bowel movement occurred, and all symptoms vanished immediately. The formula was changed to Liujunzi Tang (Six Gentlemen Decoction), and appetite returned. Over one hundred and thirty days, his hair fell out completely, and he did not leave his house for over a year.
Chen Mengzhao went to teach during the New Year, accidentally opened an empty room, saw a coffin, felt a sudden palpitation, and had a nocturnal emission that night. The next day, he forced himself to write essays. In the evening, after eating meat noodles, he developed headache, fever, a bowl-sized mass in the right hypochondrium, pain, and alternating chills and fever, suspecting a carbuncle. Diagnosis indicated internal injury with external contraction. He disbelieved this. The surgeon applied external plasters and internal detoxifying agents without effect. Another practitioner gave tonics, leading to stupor, irritability, and delirious speech resembling madness. Re-evaluation showed extremely rapid and flooding pulses, attributed to erroneous tonification. Treatment reverted to internal injury from diet, using Green Tangerine Peel, Tangerine Peel, Immature Bitter Orange, Thick Bark Magnolia Stem, Hawthorn Seed, and Coptis. Wheat bran was stir-fried and applied hot to the abdomen. He slightly revived, and five fen of Runzi Wan was given. After several doses, old turbidity was expelled, pain decreased, and the formula shifted to Ginseng, Atractylodes, Angelica, White Peony Root, Ophiopogon, Tangerine Peel, Poria, and Licorice. Regulation over a month led to stability.
Shen Zhenyu suffered from a yin syndrome presenting with yang-like symptoms and recovered using Wenjing Yiyuan Tang (Warming Meridians and Benefiting Origin Decoction). Shortly after recovery, consuming mantou (steamed buns) and mutton caused chest and abdominal distension and blockage. Medication for ten days was ineffective. Thirst, irritability, worse in the late afternoon, and constipation persisted. Niter, rhubarb, Immature Bitter Orange, Thick Bark Magnolia Stem, Areca Seed, Hawthorn Seed, Hemp Seed, Green Tangerine Peel, Safflower, Angelica, Rehmannia, Scutellaria, and Coptis were all tried without success. Lu stated: Severe illnesses require powerful prescriptions. Restricting dosages to one or two qian results in weak efficacy. For Yuanmingfen (Mirabilite) and Areca Seed, five qian are necessary; for Immature Bitter Orange, Rehmannia, Angelica, and Scutellaria, one liang is required; for Safflower, three qian. Additionally, use four to five liang of Hawthorn Seed decocted in water to replace normal water for decocting other herbs. Add half a wine glass of iron rust water before administration to naturally expel turbidity. Following this advice, one dose caused abdominal movement and constant rumbling. Within two hours, half a bucket of old turbidity was discharged, bringing immediate relief, and recovery followed through regulation.
In the past, a Brahmin monk from the East said to those eating noodles, "This is excessively heating; why do you eat it?" Seeing radishes in the food, he said, "Fortunately, this exists to neutralize its nature." Since then, it has been transmitted that one must eat radishes when consuming noodles. Novels also record: Someone poisoned by wheat flour dreamed of a woman in red singing sadly, with the line, "There is a radish roasting my palace." (Yi Shuo Xu Bian, Bencao.)
Bian Que said: Excessive drinking corrodes the intestines and stomach, soaks the marrow, steams the sinews, damages the spirit, and shortens lifespan. A guest visited Zhou, offering two shi of fine wine. Zhou drank two dou, and the guest drank eight dou. The next day, Zhou was unharmed due to habitual tolerance, but the guest had died. Examination showed the guest's ribs pierced and intestines protruding, clearly violating Bian Que's warning against exceeding capacity. (Ibid.)
Wang Haicang treated Qin Sheng, who habitually consumed Tian Sheng Cha (natural tea) and cold foods, accumulating chronic cold. The pulse was neither deep nor floating; throughout the body, pressing yielded extreme force, hard and unyielding, protruding above the skin surface, moving too fast to count and too vast to describe. This is the "drum-beating pulse" of yin syndrome. Wanderfire gathered in the chest, constrained by cold, creating strong pulsation. Zhenwu Tang (True Warrior Decoction), Sini Tang (Reverse Cold Decoction), and Lizhong Tang (Regulate the Middle Decoction) pills were given, assisted by White Peony Root and Fennel, formed into pills with wine paste to prevent ascending rebellion. Hundred pills were taken continuously day and night. After eight to nine days, half a jin of pills was consumed, leading to sweating and recovery. This is rare in the world. (Yin Zheng Lue Li, Yi Shuo Xu Bian.)
Xue Lizhai treated a man who ate zongzi (sticky rice dumplings), experiencing irritability, thirst, and difficulty defecating. Digestive promoters failed. Fermented rice wine cake, fried and powdered, was mixed with warm wine and taken at two qian. Soon, abdominal sounds occurred, the zongzi passed, and he recovered. Another person ate too many crystal dumplings, causing abdominal distension and pain, also cured by this method.
A man ate fish, developed abdominal pain and dysentery, unresponsive to various medicines. Equal parts Tangerine Peel and Atractylodes Macrocephala Rhizome, powdered, taken with aged rice water, cured him. Another person had abdominal pain every time he ate crab, relieved by strong decoction of Perilla Leaf.
A woman suffered from food stagnation and fullness. Using Renshen Yangwei Tang (Ginseng Nourishing Stomach Decoction) and Muxiang Binglang Wan (Aucklandia and Areca Pill) caused diarrhea, phlegm vomiting, and abdominal masses. Erchen Tang (Two Old Ones Decoction), Coptis, and Thick Bark Magnolia Stem worsened the bloating and anorexia. This was due to deficient stomach qi unable to digest. Buzhong Yiqi Tang (Supplement the Middle and Benefit Qi Decoction) with Poria and Pinellia was given for over fifty doses, strengthening the spleen and stomach and resolving all symptoms.
Dou Cai treated a man who ate cold foods in summer, causing severe chest and abdominal oppression. Jin Ye Dan (Golden Elixir Pill) was administered at one hundred pills, followed by Quan Zhen Dan (Complete True Pill) at one hundred pills shortly after. Gas descended, and he recovered.
Another man drank alcohol after every meal, damaging lung qi, causing chest diaphragm distension and suffocating breathlessness. Zhuru Fen (Crystal Powder) and Wuge San (Five Diaphragms Powder) cured him. For severe cases, moxa at Zhongfu (LU1) is effective; cooling medicines would cause difficult-to-treat middle-jiao fullness.
A child ate raw apricots, injuring the spleen, causing fatal distension. Moxa was applied to Left Mingmen (GV4) for twenty cones, curing him. He also took fifty Quan Zhen Dan pills.
A man addicted to tea was advised by a Daoist to fill new shoes with tea and eat them until empty, repeating three times. Men should use women's shoes, and women men's shoes. This cured him. (Ji Jiu Fang.)
Lizhai treated a man who had intercourse in summer and ate ice fruit, causing abdominal pain. Fuzi Lizhong Tang cured him. Another with the same condition disbelieved, used Scutellaria, Coptis, and Erchen Tang, and died.
Lin Guanzi treated a man who traveled far after intercourse, drank fresh spring water while hungry and thirsty, and fell ill upon return. Doctors gave exterior-resolving and digestive herbs, causing cold extending past knees and elbows, external heat, restlessness, kicking off covers, incoherent speech, and a thread-like pulse at the Cun position, with no other pulse. Urgently, Ginseng, Ginger, and Aconite, with Scallion White and Fresh Ginger, were decocted in large doses, cooled, and poured in. He slept, agitation ceased. Scallion and Ginger were removed, and after several doses, sweating occurred, liver pulse harmonized, and other medicines regulated him to safety.
Zhu Danxi treated a man who drank heavily, leading to lower bleeding, abdominal pain, tenesmus, and dysentery. Halves liang of Talcum, two qian each of Forsythia, Scutellaria, Akebia Stem, White Peony Root, Immature Bitter Orange, and Atractylodes Macrocephala Rhizome, five fen of Licorice, and twenty-one Peach Kernels were divided into four doses.
Someone injured by worry and food stagnation had accumulation in the intestines and stomach, desiring vomiting and diarrhea. From winter solstice to summer, the accumulation erupted, causing violent diarrhea for days. Yu Han Jing states: Dysentery recurring annually or monthly indicates accumulation; purgation is appropriate. Wenpi Tang (Warm the Spleen Decoction) is best. If difficult to purge, Ganjiang Wan (Ginger Pill) assists: equal parts Dried Ginger, Croton Seed, Rhubarb, and Ginseng. Follow with Baizhu San (Atractylodes Powder): equal parts Atractylodes Macrocephala Rhizome, Aucklandia, Aconite, and Ginseng. Powder finely, take two qian with one cup of water, three slices of ginger, and one jujube, warming and taking six parts.
A man drank heavily, causing chronic diarrhea unrelieved. Self-administering Aconite and Sichuan Pepper worsened anorexia and diarrhea. Talcum and Scutellaria, halves liang each, with Dried Ginger, Coptis, and Toona Sinensis Bark, formed into pills with porridge, taking one hundred pills per dose.
Scholar Xu treated Imperial Clan member Zhao Yancai, who had lower bleeding, waxy complexion, and anorexia, attributed to alcohol disease. Zijin Dan (Purple Gold Pill) was prescribed. Completing the course stopped bleeding, restored facial luster, and doubled appetite. A scholar from Xin'an had similar results with three hundred pills in one dose. Alum three qian, yellow beeswax two liang, jujubes fifty. In a clay pot or silver/stone vessel, boil three sheng of good wine. Add alum and jujubes first, simmer slowly for half a day. Remove jujubes, peel, and pit. Add wax, simmer slowly for one to two hours until paste-like. Add two liang of wax tea, mix, and form pills sized like paulownia seeds. Take twenty to thirty pills with tea or wine as desired.
Sun Zhao treated Academician Zhang Juyi, who loved wine and lived freely, suddenly developing fever and headache. Academy physicians treated him for ten days with worsening results. Scholars summoned Sun. Sun diagnosed: Others say shaoyang, but this is phlegm. Zhang loves wine, has much phlegm, and food injury. Phlegm is not shaoyang; right hand pulse is very rapid, left is peaceful. This is headache from wine/food injury. Use Shi Yao Wan (Food Medicine Pill) five to seven pills. Wait half a day to see progress. Sun administered the pill. After digestion, chest widened, headache decreased. Further diaphragm-clearing medicines brought peace. Generally, yang pathogenic factors causing headache for ten days will inevitably turn to fever and madness. Thus, it is not shaoyang but food disease.
Zhu Danxi treated Lady Hu, who ate cold noodles and meat, causing headache, spontaneous sweating, chest stuffiness, and dark urine. Three and a half qian of Atractylodes Macrocephala Rhizome, one and a half qian of Tangerine Peel, five fen each of Akebia Stem, Ligusticum Wallichii Rhizome, and Scutellaria Root, decocted with ginger water, swallowed with fifty grains each of Caodoukou Wan (Cardamom Pill), Asarum Pill, and Baohe Wan (Harmonize the Stomach Pill).
Luo Qianfu said: In winter of Ding Si year, returning from military service to Bianliang, actor Li Ren'ai told me: My eldest son, since July this year, worked hard, drank cold tea while thirsty, and ate cold rice. He felt epigastric stuffiness. One dose of medicine caused two bowel movements, reducing symptoms. Days later, he ate cold foods again, causing recurrent epigastric and abdominal fullness, vomiting, nausea, loss of taste, refusal to eat, limb fatigue, and reluctance to speak. The previous doctor was consulted again, saying: This is easy to treat; a few more bowel movements will bring relief. Youyou San (Carefree Powder) mixed with Kansui powder was taken in white water. Evening came, abdominal thundering and cramping pain ensued. Soon, diarrhea returned, persistent thirst and irritability, uncontrollable eating desires, and occasional stupor. The doctor was called again, found the pulse unmanageable, and withdrew. Soon, cold sweat poured, breath from nose and mouth grew cold, and he died. The servant regretted deeply and asked. I said: I haven't seen it personally, don't know why, but left. Someone else said: I saw it personally. Was it the medicine's fault? I said: Not the medicine's fault, but failure to observe dosage limits. Medicines without basis become qi thieves. Neijing says: Regulating a prescription is like tying a bag. If the bag is full and not tied, contents leak; if not tied, spirit and qi do not gather together. Thus, Zhang Zhongjing, using Guizhi Tang for wind strike, said: If one dose causes sweating and cure, stop subsequent doses, do not finish the decoction. Dachengqi Tang for severe fullness, said: Change clothes, stop subsequent doses, do not finish. Such caution is a great warning, following the sage's principle of tying the bag. Treating disease requires seeking the root. Li played drama, exhausting spirit and qi, causing mid-abdominal pain. Due to summer heat, he drank cold tea. Spleen and stomach qi were weak, unable to transform, causing fullness. Purging with medicine further weakened him. Carelessness damaged yang. Deficiency followed by purging, yin contends inside, yang disturbs outside, soul sweat not stored, cold limbs arise internally. Zhongjing said: One reversal delays death; two reversals hasten it. Is this not failure to observe dosage? Neijing warns: Superior doctors regulate qi; middle doctors disrupt pulses; inferior doctors exhaust qi. Must be cautious.
Zhang Zihe treated a tenant farmer addicted to tea, with accumulation in the left hypochondrium. Said: Similar to Feiqi (Fat Qi), but without malaria attacks, it is not Feiqi. Though sick for ten years, not laboring a day. Both hands deep and thin, due to accumulation. My treatment avoids acupuncture pain; only medicine extends life. First, Chatiao San (Tea Regulating Powder) induced vomiting of several sheng of old tea. Then, using a wooden tool, induced vomiting of several more sheng, all tea-colored. Next, Sanhua Shenyao Wan (Three Flowers Divine Protection Pill) ten-plus pills caused over twenty bowel movements that night, mixing pus, water, dry feces, and blood. Next day, dampness-draining medicines for ten days relieved all suffering, restoring clear spirit. (Yi Shuo Xu Bian.)
Quan Gong, Supervising Censor of Eastern Zhejiang, drank over ten cups of Araki (distilled spirit) every morning before regular wine. In June, severe fever developed. Zhang Yizhi treated him by placing ice on the heart and abdomen. After ice melted, fever increased. Internal medication cured him in three days. (Yaoyao Huowen.)
A wealthy young man in his twenties had fever in April, sought Zhao Yi-de. Pulses were weak floating and sunken, with fluctuating deficiency heat, tidal fever without fixed time, and intermittent forceful, rapid pulses advancing and retreating with heat. Not external heat, likely retained heat from alcohol, exacerbated by sexual exhaustion and qi/blood deficiency. Inquiry confirmed drinking Araki daily in January and eating dog meat for a month. Knowing this, qi/blood tonics plus Kudzu Root to dissipate alcohol toxin were given. One dose caused slight sweating, but weakness increased, fever remained. Realizing qi/blood were too weak to tolerate Kudzu's dispersing action, Chicken Claw Fruit (Jiju Zi) was needed to resolve toxicity. A small amount of dried fruit was added, and fever resolved.
Tang Sheng, due to drinking milk water and eating raw foods, had purple-black bloody diarrhea over ten times. Spleen and stomach affected by cold-damp toxin. Liushen Pingwei San (Six Spirits Harmonize Stomach Powder) halves liang plus three qian Atractylodes Macrocephala Rhizome to promote blood circulation in waist and umbilicus. One dose cured him.
Zhou Zigong treated Wang Jingli, who felt light and floating, as if walking in emptiness. Seventy doctors changed, all diagnosing wind-deficiency, giving heat-inducing medicines which worsened condition. Zhou said: This is alcohol toxicity. Cooling medicines dispelled it, and he recovered. (Ji Yi Shanfang Ji.)
Zhang Zihe treated an alcoholic patient with headache, fever, aversion to cold, resembling shaoyang. Pulses were flooding and large in both hands. No bowel movement for two to three days. Fangfeng Tongsheng San (Saposhnikovia Unblocking the Healthy Decoction) about one liang, one bowl of water, twenty-plus slices of ginger, twenty scallions, a large handful of fermented soybeans, boiled three times, strained, slightly warmed, divided into two doses. Took most of the first dose, soon stimulated throat with hairpin, vomited about one liang of old wine, still fragrant. Head sweat poured like rain. Took remaining small portion, instantly cured. Neijing says: For fire stagnation, vent it. Vent means inducing sweating and dispersion.
Zhu Danxi treated a drinker with chest fullness, fever, night delirium, resembling shaoyang. Right pulse discordant, left large. Buzhong Yiqi Tang (Supplement the Middle and Benefit Qi Decoction) minus Astragalus, Bupleurum, and Cimicifuga, plus Pinellia. Astragalus tonifies qi, Bupleurum and Cimicifuga ascend, thus removed. Condition cured. Heart pain from cold food resolved by adding Cardamom grains to previous medicine. (Zhi Fa.)
Lizhai treated Cao Quan, who drank Fenjiu, causing anal swelling/pain and constipation with forceful pulse. Jingfang Baidu San (Aucklandia and Saposhnikovia Dispelling Toxicity Powder) failed. Huanglian Neishu Tang (Coptis Interior-Purging Decoction) cured him.
Zhang Zihe treated Vice-Prefect Qin Shuixin of Suzhou, initially with middle qi deficiency-cold, compounded by heavy paperwork and social disturbances, leading to fatigue, poor appetite, slight surface heat, and inability to work. Diagnosis started with warming, then tonifying, eventually adding Ginseng to one liang and Aconite to three qian per dose. Diet and spirit improved. Due to bitter taste of long-term decoctions, pills were made for maintenance, generally supporting yang and suppressing yin. Suddenly, symptoms recurred, worse than before, with spontaneous sweating, dizziness, and reluctance to speak. Urgent consultation questioned previous large-dose warming/tonifying decoctions, assuming pills were too slow. Diagnosis concluded: Symptoms same as before, but medicine different. Likely after mental exertion and anger, re-injured by diet, causing failure to receive and transform, with excess heat stagnating in Taiyang and Yangming meridians. As Wang Dao'an said, within dietary/labor fatigue, there remains excess and deficiency. Now differs from past; Ginseng and Aconite strictly forbidden. Only clearing and guiding to eliminate heat is suitable. Vice-Prefect agreed, treated accordingly, and recovered.
