[Supplementary Discussion]
The discussion states: Luoli (scrofula) and other diseases are all caused by wind-heat qi and toxic factors accumulating in the zang-fu organs, clashing with the liver meridians and sinews. Since the Liver governs the sinews, when toxic vapors attack the sinews and vessels, they stop and knot in areas where the muscles are deficient, forming Luoli. It mostly occurs between the neck and axilla. Its appearance is like a cluster of nodules, connected continuously like plums or plums, called Luoli.
Prescriptions
Jiuku Shengling Dan (also known as Jiuku Huajian Tang, from *Jisheng Bachui Fang*) treats Luoli, Madao Chuang (scrofulous abscesses resembling machetes), and Ying (goiter). The lesions start from below or behind the ear, extending down the neck to the shoulder, or entering the Quepen (ST12) area; this is the distribution field of the Hand-Shaoyang Sanjiao Meridian and Foot-Shaoyang Gallbladder Meridian. Those Luoli located at the lower head, even reaching the Jiache (ST6) point, belong to the distribution field of the Foot-Yangming Stomach Meridian, arising from pathogenic factors affecting the Heart and Spleen. Now, these two conditions are treated together.
Huangqi (Astragali Radix, 1 qian): Protects the exterior and interstices of the skin and hair. If the interstices are deficient, it invigorates blood vessels. It is also a sacred medicine for treating sores and ulcers, capable of tonifying primordial Qi on the surface to eliminate weakness, harmonizing Yang Qi, and draining fire pathogens.
Renshen (Ginseng Radix, 3 qian): A herb that tonifies Lung Qi. Use it if there is shortness of breath and irregular breathing with panting.
Gancao (Prepared Glycyrrhizae Radix et Rhizoma, half qian): Harmonizes the herbs, drains fire, and benefits Stomach Qi.
Shengma (Cimicifugae Rhizoma, 1 qian)
Gegen (Puerariae Lobatae Radix, half qian)
Zhen Loulu (Euphorbiae Humifusae Radix, half qian): If unavailable, omit this herb. These three herbs are specific to the Foot-Yangming Stomach Meridian.
Lqiao (Forsythiae Fructum, 1 qian): This herb dissipates blood stasis and Qi stagnation in the twelve meridians; it is indispensable in sore-treating formulas.
Mudanpi (Moutan Cortex, 3 qian): Removes stagnant blood in the intestines and stomach.
Danggui (Angelicae Sinensis Radix, 5 qian)
Shudihuang (Prepared Rehmanniae Radix, 5 qian)
Shengdihuang (Rehmanniae Radix, 5 qian): These three herbs harmonize, generate, and cool the blood among various medicines.
Baishaoyao (Paeoniae Radix Alba, 5 fen): Sour in taste and cold in nature, it tonifies the Middle Jiao and benefits Lung deficiency. It must be used for treating abdominal pain.
Rougui (Cinnamomi Cortex, 3 fen): Dissipates accumulated Yin and yin-sores/ulcers. It should be used in small amounts. One theory suggests "treating cold with heat," using acrid-hot properties to cover Yin-cold and dissipate floating frozen Qi. Omit if the patient is dry and irritable. It must be used for Yin-type sores.
Chaihu (Bupleuri Radix, 8 fen, used with Lqiao as a guiding herb for the Shaoyang Meridian). If there is no Madao or Ying, the disease is not in the Shaoyang Meridian; omit Chaihu.
Shuniuzi (Arctii Fructum, 5 fen): Do not use if there is no swelling; use if swelling is present.
Kunbu (Sargassum, 5 fen): Salty in taste; suitable for hard masses because salt softens hardness.
Ezhu (Curcumae Rhizoma, 5 fen, fire-processed)
Jingsanleng (Sparganii Rhizoma, 5 fen, roasted): Use for very hard sores; do not use if not very hard.
Yizhiren (Alpiniae Oxyphyllae Fructum, 3 fen): Use for vomiting saliva or food due to Cold in the upper Stomach. Omit if absent.
Damaimian (Hordeum Vulgare Farina, 1 qian): Treats abdominal contraction and urgency; also aids digestion and tonifies the Stomach.
Shenqu (Medicated Leaven Powder, 5 fen, stir-fried): For indigestion.
Huanglian (Coptidis Rhizoma, 5 fen): Treats irritability and oppression.
Another version omits Shengdihuang. Grind the above herbs into fine powder. Soak in boiling water, mix with steamed wheat dough, knead evenly, shape into cakes, sun-dry, and crush into rice-grain-sized pellets. Take 3 qian or 2 qian each time, washed down with boiling water. Adjust dosage according to the patient's deficiency or excess, ensuring the dose does not hinder appetite. This is the general principle of treatment.
If accompanied by stiffness and pain of the spine (Hand and Foot Taiyang meridians), inability to look back, waist feeling like breaking, and neck feeling like pulling, add Duhuo (0.5 qian), Qianghuo and Fangfeng (1 qian each). Fangfeng is acrid and warm; even without Taiyang symptoms, if the sore is above the diaphragm, it should be used to disperse Wind in the upper body. Contraction and spasm indicate Wind. If there is Heat or weakness in the legs, add Chao Huangbo (0.5 fen). If there is dryness, irritability, and desire to undress, indicating latent Fire in the Kidney, add more Huangbo. Omit if these symptoms are absent. If abdominal distension occurs after taking medicine, add Chao Houpo (0.5 fen); omit if unavailable. If Qi is stagnant, add Chenpi (Citri Reticulatae Pericarpium) and Muxiang (Vladimiriae Root).
For Luoli in the Yangming distribution, omit Chaihu and Shuniuzi. For Madao/Ying in the Shaoyang distribution, omit Duhuo, Loulu, Shengma, Gegen, and add Qumai (3 fen).
Generally, for sores and other diseases, if the person is weak, omit bitter-cold herbs and increase Renshen, Huangqi, Gancao, etc., to tonify Yuan Qi before draining fire.
If the pathogen is in the Upper Jiao, add Huangqin (half wine-washed, half raw). In the Middle Jiao, add Huanglian (half wine-washed, half raw). In the Lower Jiao, select Jiushui Huangbai, Jiushui Zhimu, Fangji, etc.
If stool is unsmooth due to excessive pathogen, add Jiuda Huang (wine-washed Rhizoma Rhei) to promote bowel movement. If due to Blood dryness causing dry stool, add Taoren (Peach Kernel) and Jiuda Huang. If due to Wind-dryness obstruction, add Maziren (Cannabis Seed) and Huanglian. If due to Wind-astringency causing constipation, add Weijiaoren (Roasted Gleditsia Sinensis Seed), Da Huang, and Qinjiao (Radix Gentianae Macrophyllae) to promote movement. If Wind-pulse is astringent with Qi-stagnation and urinary difficulty, add Yuliren (Prunus Semen) and Da Huang to remove Qi-dryness.
For Yin-Cold diseases with cold-clumping closure and constipation, use Banshu Wan from the Bureau Formulas, or decoct with Fuzi (Prepared Aconiti Lateralis Radix) and Ganjiang (Zingiberis Rhizoma), taken cold.
Sandu Kuijian San (from *Jisheng Bachui Fang*) treats Madao Chuang, where knots are hard as stone, located from below the ear to Quepen, or to the shoulder and hypochondrium, involving the Hand and Foot Shaoyang meridians, and Luoli spreading over the jaw to Jiache, hard but unruptured, involving the Foot-Yangming meridian. Or if both sores have ruptured and discharged pus, treat accordingly. Take medicine before sleep, judging dosage based on diet and stool consistency.
Longdancao (Gentianae Radix, 0.5 liang, wine-washed and stir-fried four times)
Gancao (Prepared, half qian)
Shengma (6 fen)
Chaihu (0.5 qian)
Jiegeng (Platycodonis Radix, 0.5 liang)
Lqiao (3 qian)
Gegen Baishaoyao Huanglian (2 qian each)
Kunbu (0.5 liang)
Zhimu (Zhi Mu Rhizoma, 0.5 liang, wine-washed twice)
Danggui Shao (Angelicae Sinensis Radix tip, 0.5 qian)
Gualougen (Trichosanthis Radix, 0.5 liang, cut finely, wine-washed)
Huangqin Shao (Scutellariae Radix tip, 1.5 qian, wine-washed and stir-fried, half raw)
Jingsanleng (3 qian, wine-washed, slightly roasted)
Ezhu (3 qian, finely wine-washed and stir-fried)
Huangbai (Phellodendri Cortex, 0.5 liang, wine-stir-fried four times)
Cut the above herbs. Take 6 qian or 7 qian each time, with 2 cups of water, soak for half a day, decoct to 1 cup, strain, and take hot. While lying down, elevate feet, keep head slightly low, hold one mouthful of liquid, swallow in ten portions until finished. Rest frequently to allow medicine to accumulate in the chest. Prepare half the dose as fine powder, honey-pill size of mung beans. Take 100-150 pills each time. Wash down with a mouthful of the decoction. Add Haizao (Sargassum, 0.5 liang, stir-fried).
Quangu Wan (from *Shenghui Fang*) treats Luoli.
Guangu Gu (Stork Bone), Li Gu (Fox Bone), Shegan (Belamcandae Rhizoma), Xuanshen (Scrophulariae Radix), Shengma (Prepared), Qingmuxiang (Aquilariae Lignum Resinatum), Chenxiang (Aucklandiae Radix), Xijiao (Rhino Ceratostigma, all prepared), Dingxiang (Caryophylli Flos), Lingyangjiao (Saiga Tataricae Cornu, powdered), Danshen (Salviae Miltiorrhizae Radix), Gancao (Prepared, 4 qian each), Renshen, Shashen (Adenophorae Radix, 3 liang each), Tagan (Otis Liver, 6 fen), Liaoqiao Zi (Forsythiae Fructus, 6 fen), Guangming Sha (Cinnabaris, 3 fen, ground well). Grind and sieve. Honey-pills. Take 15 pills with drink, twice daily, gradually increasing to 20 pills. Take on an empty stomach. Diarrhea patients avoid raw/cold, greasy, blood foods, hot meat, Haizao, Songcai (Chinese Cabbage), sticky/stale foods. Another formula includes Shexiang (Moschus).
Heiqian San (aka Shenmi San, Dasheng San, Qianniuan, from *Renji Zhizhi Fang*) treats chronic Luoli.
Banmao (Mylabris, any amount, remove head/legs, stir-fry with rice until black/rice yellow, remove rice, remove fire toxicity by placing on paper on ground, grind to powder, reserve rice for later use)
Baican (Bombyx Batryticatus, select straight ones, stir-fry to remove silk)
Heiqian Niu (Semen Kochiae, use first-run powder)
Jingjie Sui (Schizonepetae Spica, top). Grind the above three separately, mix equal parts.
Take 1 qian of Banmao powder, plus 1 qian combined Baican, Qian Niu, Jingjie Sui. Mix with 1 cup of liquid. On the day of treatment, skip dinner to empty the stomach. First, take 2 qian Hutong powder mixed with Jingjie Decoction, heavy dose 1 cup. At first watch (9-11 PM), take another cup. Shortly after, take 1 cup of the four herbs mixed with non-intoxicating wine, then lie down. If urine is not collected in a vessel, expect discharge resembling mucus or lumps. Count the number of Luoli on the neck; the number of lumps in urine should match. Rat-shaped ones usually heal. Repeat if necessary. If urination is severely difficult, take 1-2 qian Hupo (Amber) mixed with Dengcao (Juncus Decoction) to expel evil matter. Jingjie + Hutong also works. If medicine causes blockage, take Wuling San with Black Bean Decoction. Pregnant women forbidden. If post-medication urinary retention occurs, take Dengcao + Hupo. Then use subsequent medicine.
For unruptured sores: Use Sangshen (Mulberry Fruit), extract juice, boil into paste (drop test: does not spread). Mix with Wei Danfan (Calcined Alum), apply on paper to the sore.
For ruptured sores: Apply Chen Sha (Cinnabar) and Danfan powder lightly. Avoid hot/toxic/windy foods.
For non-healing ruptured sores, use topical powder:
Zhizhu (Spider), Banmao, Hong Niangzi (Lytta Minor), Shuiwa (Water Frog), Didan (Blister Beetle Larva), Yuanhua (Genkwa Flos), 0.5 qian each. Roast on new tile until dry, grind. Wash sore with Scallion White Decoction, dry with soft silk. Apply with good rice vinegar. Three times daily. When fluid dries up, use flesh-generating medicine.
Xuejie (Dragon's Blood), Longgu (Fossilized Bones), Rushiang (Frankincense), Mingfan (Alumen), Gaoyaosh (Gypsum Fibrosum), Huangdan (Lead Tetroxide), 1 qian each (calcined/pass through fire). Grind. When yellow pus/blood is gone, sprinkle dry. Naturally generates flesh.
Huadu San treats Luoli.
Danggui, Baishaoyao, Chishaoyao (Red Peony Root), Zhimu, Zhuma Gen (Ramie Root), Baijiaoxiang (Elemi Resin), Gancao, Beimu (Fritillariae Bulbus), Qinjiao, Shengdihuang, Baigui (Tortoise Plastron), Chaihu, Qianhu (Peucedani Radix), Guanzi Gui (Cassia Twig), Sheng Huang (Raw Astragalus), Shu Huang (Prepared Astragalus), 1 liang each.
